Intravascular leiomyomatosis in postmenopausal woman: a case report.

Intravenous leiomyomatosis (IVL) is a rare benign tumor originating from the smooth muscle of the uterus. Tumor cells spread along the veins, which may involve the uterine veins, pelvic veins, inferior vena cava, and even the right atrium. This case report describes a postmenopausal woman with IVL extending into the right heart chambers. The patient successfully underwent surgery and was discharged without complications. This report highlights valuable insights into the preoperative diagnosis and clinical management of IVL.

[An abscess gone astray].

A 69-year-old female patient with known recurrence of esophageal cancer presented with fever and elevated inflammatory markers. In the recent past, a suspicious mediastinal lymph node had been biopsied via bronchoscopy. Consequently, a lymph node abscess developed, leading to blood stream infection (pathogen: Streptococcus anginosus) via a fistula into the superior vena cava. Through a preexisting atrial septal defect, the infection also spread to the brain, causing multiple brain abscesses.

Ascending aortic aneurysm after acute aortic dissection in a case with systemic lupus erythematosus.

A 50-year-old woman with systemic lupus erythematosus and a history of aortic arch replacement surgery for Stanford type A aortic dissection experienced a reoccurrence of an ascending aortic aneurysm and coronary artery occlusion. Computed tomography revealed that the aneurysm was compressing the superior vena cava and right atrium. The patient underwent urgent surgery to repair the aneurysm. This case highlights that aortic aneurysms can reoccur even after total arch replacement in systemic lupus erythematosus patients.

Iatrogenic Budd-Chiari Syndrome Secondary to Transhepatic Hemodialysis Catheterization.

Hepatic vein thrombosis (HVT), or Budd-Chiari syndrome, is a rare disorder resulting from the obstruction of blood flow from the liver to the inferior vena cava and eventually back to the heart. HVT can lead to extensive hepatocellular injury and portal hypertension and may require liver transplantation in patients. We present a rare cause of HVT and subsequent liver failure secondary to transhepatic venous catheterization for hemodialysis (HD) in a patient with end-stage renal disease (ESRD).

Perioperative Venous Excess Ultrasound Score (VExUS) to Guide Decongestion in a Dilated Cardiomyopathy Patient Presenting for Urgent Surgery.

Venous excess ultrasound score (VExUS) is a recently described ultrasound-based scoring system that quantifies systemic congestion using Doppler flow indices of the hepatic and portal vein in addition to inferior vena cava assessment. There are many potential and emerging applications of this modality. We discuss the case of a severely congested heart failure patient presenting for urgent non-cardiac surgery where VExUS parameters were used to monitor and guide his decongestive therapy postoperatively.

Autotransplantation for Threatened Cardiac Rupture After Left Ventricular Repair.

A 26-year-old Japanese woman experienced threatened cardiac rupture after mitral valve replacement and debridement of a left ventricular (LV) posterior wall abscess caused by infectious endocarditis. We performed cardiac autotransplantation using the bicaval technique to allow bench repair of the recurrent abscess. A balloon catheter was inflated in the inferior vena cava to obtain a bloodless anastomotic field. This is the first report of autotransplantation for threatened LV rupture after posterior wall repair.

Real time three-dimensional transesophageal echocardiography imaging of crescent-shaped superior sinus venosus atrial septal defect.

Multi-imaging modalities should be considered in patients with atrial septal defects (ASDs) in which the degree of right ventricular enlargement is not fully explained by the small size of ASD found on conventional transthoracic echocardiography. We report a case of crescent-shaped superior sinus venosus ASD discriminated using real-time three-dimensional transesophageal echocardiography. MDCT confirmed its relationship with the superior vena cava and the absence of combined anomalies, such as the pulmonary venous return anomaly.

Left isomerism syndrome with total anomalous systemic connection.

We present a case of left isomerism with total anomalous systemic venous connection where the inferior vena cava was absent and all other systemic veins connected abnormally to the left atrium. The right atrium was hypoplastic with an intact atrial septum. Blood flow to the lungs was through a large ventricular septal defect. The diagnosis was made with echocardiography, angiography, and computed tomography. Complete repair was performed successfully, and the 7-year-old patient had an uneventful recovery.

Inferior sinus venosus atrial septal defect.

One case of inferior vena cava type sinus venosus defect is reported. It is likely to be missed on transthoracic echocardiography. Transesophageal echocardiography is useful. Unlike superior sinus venosus defect which are easily visible on transesophageal echocardiography, a careful search is needed to detect inferior sinus venosus defects. All patients with unexplained right ventricular volume overload on transthoracic echocardiography should have transesophageal echocardiography to see for sinus venosus atrial septal defects.

[Unusual picture of pacemaker leads in patient after Senning procedure].

We present a case of a 21-year-old man after Senning operation admitted to our hospital for transvenous implantation of a dual chamber pacemaker. The presence of persistent left superior vena cava enabled us to implant the desired dual chamber pacemaker. It is an extremely unusual situation when two pacemaker leads utilise two different routes to the heart: superior caval vein - atrial baffle - ventricle and persistent left superior caval vein - atrium.

Recurrent cerebral embolism associated with indwelling catheter in the presence of anomalous neck venous structures.

Persistent left superior vena cava (PLSVC) is the most common central venous anomaly in the thorax, which may remain asymptomatic throughout life. Indwelling catheters that are increasingly used in oncology practice may result in complications in the presence of PLSVC. The authors report an unusual case of recurrent ischemic attacks each time a portacath was accessed in a patient with breast cancer and PLSVC. This case highlights the importance of suspecting this condition in the presence of clinical and radiographic clues to prevent complications.

A combined endovascular and open ''reverse hybrid'' technique for repair of complex juxtarenal inflammatory aortic aneurysms.

Inflammatory abdominal aortic aneurysms (IAAA) can present significant challenges to surgeons, especially in the juxtarenal location where they may not be amenable to endovascular repair. The dense, inflammatory component of these lesions can encase adjacent structures including the duodenum, ureters, and inferior vena cava putting them at risk for injury during open exposure. We report a novel ''reverse hybrid'' technique using a combined endovascular and open approach for repair of large, juxtarenal IAAA's.

An unusual cause of stroke--the importance of saline contrast echocardiography.

We report a case of a 38-year-old man who presented with a cryptogenic stroke in whom a persistent left superior vena cava (PLSVC) to the left atrium was established as an isolated anomaly by both echocardiography and magnetic resonance angiography. This rare cardiac abnormality creates a systemic right to left shunt and the potential for cerebral abscess or infarction. Echocardiographic diagnosis may be missed unless intravenous saline contrast is performed using a left upper extremity vein.

A very rare venous anomaly in a living liver donor: left hepatic venous connection to the right atrium.

In humans, three main hepatic veins drain the liver into the inferior vena cava below the diaphragm. This report represents the first living donor liver that had a rare anatomic variation of the left hepatic vein draining directly to the right atrium, which was detected preoperatively by routine investigations of the living donor transplantation. This type of anomaly may present potentially fatal challenges to a donor operation if not detected preoperatively, especially when the left lobe is the choice for explantation.

Anomalous right upper lobe venous drainage.

Anomalous pulmonary venous return is a rare congenital anomaly mainly involving the right lung and is often associated with congenital intracardiac malformations as atrial septal defect. We report a case of anomalous right upper lobe venous drainage resulting in two right upper lobe veins draining into the azygous vein and into the confluence between superior vena cava and azygous vein, respectively. Preoperative identification of such an aberrant venous drainage is useful for avoiding unexpected intraoperative bleeding.
