The etiology and pathogenesis of idiopathic nephrotic syndrome remain obscure. Several investigations have reported a role for allergy in the development and maintenance of this disease, especially in childhood. Of 73 adult patients attending a clinic specially provided to treat patients with atopic dermatitis, 31 (42%) showed one or more positive patch reaction on contact testing. There was a striking female preponderance in the patch test positive group (26F:5M) in contrast to those with negative test results (9F:17M). A new technique by indirect immunofluorescence on sections of pigeon oviduct, is described as an aid to diagnosis of pigeon breeder's disease. It is easy to do, inexpensive, and permits distinction between affected patients and exposed but unaffected persons. The main allergen, pigeon IgA, participates in the fluorescent responses of the mucous membrane and associated secretions. It has been possible to relate changes in antibody titre with improvement following treatment.

Immunotherapy, widely used in the treatment of atopic disease, poses many problems. In asthma the multiple etiologic factors involved make clinical trials of its efficacy difficult to evaluate and immunological studies are badly needed. Allergen extracts of standard potency are essential. The author feels that practicing allergists must improve their knowledge of and expertise in immunotherapy if the patient is to derive maximum benefit from it.

Allergization under conditions of functioning an inhibitory factor in the organism was studied by the reaction of leukocytolysis with Tsuverkalov's dysenterin and in the reproduction of hypersensitivity of the immediate type on the Schultz-Dale's model. It was shown that in functioning an inhibitory factor in the organism the degree of leukocytolysis reaction with the dysentery allergen decreased in the immunized animals in parallel with the fall in the serum antibody activity. A depression of the activity of macromolecular sensitizing antibodies was also observed.
