The relationship of joint symptoms with exercise performance in older adults.

OBJECTIVES: The objective of this study is to determine if exercise increases joint symptoms in older adults with a history of arthritis or produces symptoms in older adults without such history. In addition, we examine whether joint symptoms explain the large observed variation in strength gain in older adults undergoing vigorous strength training exercise, and report the incidence of musculoskeletal injuries upon initiation of an exercise program. DESIGN: A population-based, single blinded, randomized controlled trial with three exercise groups and one control group. SETTING: A large urban

Management of a Pilon Fracture Complicated by Drug-induced Hypersensitivity Syndrome, Fungal Infection and Limb Length Discrepancy: A Case Report.

AIM AND BACKGROUND: Pilon fractures are rare but severe injuries associated with high-energy trauma, often resulting in significant articular and soft tissue damage. Their management is challenging due to complications such as infection, nonunion, and post-traumatic arthritis, which often require complex staged surgical interventions. Drug-induced hypersensitivity syndrome (DIHS), a life-threatening reaction characterised by rash and systemic involvement, presents additional challenges in orthopaedic contexts due to its association with high-dose corticosteroid therapy and increased infection

Elasticity mapping of gastrocnemius fascia and myotendinous junction of the medial gastrocnemius during passive ankle and knee dorsiflexion.

BACKGROUND AND PURPOSE: Muscle strain is a common side-effect of exercise. In this context, many scholars have focused primarily on the biological characteristics of the muscle, generally overlooking the importance of myofascial elasticity. Therefore, the aims of the present study were: (1) to evaluate the intra- and inter-operator reliability of elasticity measurements in the myofascial and myotendinous junction (MTJ) using ultrasound shear wave elastography (USWE); (2) to establish the association between myofascial elasticity and passive joint angles. METHODS: Twenty-one healthy and physic

Neuromuscular Strategies in Dominant and Non-Dominant Legs in Dancers During Dynamic Balance Tasks.

Introduction: Ballet-based dance training emphasizes the equal development of both legs. However, dancers often perceive differences between their legs during balance or landing. There still needs to be more consensus on the functional difference between dominant (D) and non-dominant legs (ND). Therefore, this study investigated both legs' neuromuscular strategy in single-leg balance and landing based on leg dominance. Methods: Thirteen female dancers (age: 22.2 ± 2.8 years) with no history of ankle injuries in the past year participated in the study. Based on the questionnaire, the dominant l…

Autologous bone graft and periosteal patch for large and recurrent talar osteochondral defect: A prospective study.

BACKGROUND: Osteochondral injuries of the talus (OLT) are common as a variety of mechanisms causes them. Asymptomatic presentation with an underlying damaged bone and the overlapping symptoms with a simple sprain make OLT a potentially career-ending injury. The best surgical intervention aims to alleviate pain and improve long-term outcomes as well. These outcomes can be measured by 3 major clinical measurements: Visual analogue Score (VAS), American Orthopedic Foot and Ankle Society (AOFAS), and Foot and Ankle Disability Index (FADI), in addition to achieving the lowest complication rate poss

Treatment and rehabilitation of subtalar dislocations: A case series and a review of the literature.

INTRODUCTION: Subtalar dislocation is an uncommon orthopaedic pathology, representing 1 % of all traumatic injuries of the foot and 1-2 % of all dislocations, mostly affecting young male adults. While its urgent treatment consisting in reduction and immobilization of the dislocation has been well described, disagreement exists about post-operative management with specific regards to immobilization length and rehabilitation protocols. MATERIALS AND METHOD: A case series of traumatic subtalar dislocations treated with urgent reduction, a mean of 4 weeks immobilization and subsequent rehabilitat

An association between excessive valgus hindfoot alignment and postural stability during single-leg standing in adolescent athletes.

BACKGROUND: Diminished balance is associated with the incidence of ankle and lower extremity injuries in adolescents. Although flexible flatfoot is a common foot condition in pediatric and adolescent populations, the association between balance control and foot morphology remain unclear in adolescent athletes. METHODS: Rearfoot angle in the double-limb standing position, body mass index (BMI), and isometric muscle strength related to the knee joint were retrospectively reviewed in 101 adolescent athletes (75 boys and 26 girls) with a mean age of 14.0 years (range 12-17). Postural stability du

Ground Reaction Forces during Vertical Hops Are Correlated with the Number of Supervised Physiotherapy Visits after Achilles Tendon Surgery.

The objective of this study was to assess the effectiveness of, and the correlation between, an average of 42 supervised physiotherapy (SVPh) visits for the vertical ground reaction forces component (vGRF) using ankle hops during two- and one-legged vertical hops (TLH and OLH, respectively), six months after the surgical suturing of the Achilles tendon using the open method (SSATOM) via Keesler's technique. Hypothesis: Six months of supervised physiotherapy with a higher number of visits (SPHNVs) was positively correlated with higher vGRF values during TLH and OLH. Group I comprised male patie

Repair of acute extensor hallucis longus tendon injuries: a retrospective review.

BACKGROUND: Extensor hallucis longus (EHL) tendon injuries may occur with lacerations sustained over the dorsum of the foot and lead to hallux dysfunction. Primary repair is performed when tendon edges are opposable; however, if a gap exists between tendon edges, then reconstruction with tendon graft or tendon transfer may be necessary to restore hallux alignment and dorsiflexion. We describe the surgical technique and report the results on a large series of patients having undergone primary repair or reconstruction of EHL tendon lacerations. METHODS: We retrospectively reviewed all patients

Childhood obesity is associated with increased risk of most lower extremity fractures.

BACKGROUND: A number of studies have found an increased risk of lower extremity injuries in obese patients. Most studies, however, are unable to provide stable population-based estimates based on the degree of obesity and few assess the risk pertaining to more detailed fracture location in the lower extremities. QUESTIONS/PURPOSES: We therefore investigated the relationship between obesity and lower extremity fractures in different age and fracture locations in a stable population. METHODS: This is a population-based, cross-sectional study from the electronic medical records of 913,178 patie

Ilizarov external frame technique for pirogoff amputations with ankle disarticulation and tibiocalcaneal fusion.

BACKGROUND: The high rates of wound failure, persisting infection, and nonunion of the tibiocalcaneal arthrodesis are the main reasons why the Pirogoff ankle disarticulation is rarely used for limb salvage. Use of the Ilizarov external frame has increased our fusion rate. The purpose of this study was to review our experience with the use of the Ilizarov external frame as a technique for Pirogoff amputations with ankle disarticulation and tibiocalcaneal fusion. METHODS: Twenty-four patients (median age, 57.4 years; range, 29-76 years) underwent a Pirogoff amputation with Ilizarov external fra

Suture-Button Versus Syndesmotic Screw Fixation of Ankle Fractures: A Comparative Retrospective Review Over One Year.

Background Syndesmotic fixation is essential in the management of unstable ankle fractures with syndesmotic disruption. It can be achieved either by rigid fixation with screws or dynamic fixation with suture-button devices. Syndesmosis screws are considered the gold standard for the stabilisation and fixation of distal tibiofibular syndesmotic injuries. We use both types of devices in our unit for the stabilisation of syndesmotic injuries. In our department, Arthrex TightRope® (Arthrex, Naples, FL) is the preferred suture-button device for the dynamic fixation of distal tibiofibular syndesmosi

Weightbearing CT Reveals No Anatomical Features Associated With Ligamentous Lisfranc Injuries: A Matched Case-Control Study.

BACKGROUND: Several radiographic and cadaveric studies have suggested that specific anatomical features of the Lisfranc joint complex, including a shallower medial mortise of the second metatarsal (M2), shorter M2 length, or pes cavus alignment, may be associated with ligamentous Lisfranc injuries. These associations have largely been derived from 2-dimensional imaging or post-injury assessments. Weightbearing computed tomography (WBCT) enables 3-dimensional evaluation of midfoot anatomy under physiologic load and allows a more robust reassessment of these proposed anatomical associations.
