Modified technique for resection of the alar folds to nasal paralysis in a mule: Case report.

Equids are obligate nasal breathers. Therefore, conditions that obstruct or impair this respiratory route can compromise animal welfare by causing severe respiratory distress, while also impacting athletic potential. Nasal paralysis results from injury to the facial nerve and its branches and is characterized by the lack of movement of the alar cartilage, leading to nasal collapse. Its occurrence is uncommon in horses and has not been previously reported in mules. This report describes a 9-month-old mule presented with a history of facial trauma and compression caused by a halter. After unsucc

Concussion-Related Cognitive and Lipid Changes in Retired UK Rugby Players Study.

Long-term effects of concussions, particularly in contact sport athletes, have been linked to changes in neuronal health. Lipid dysregulation has emerged as a potential contributor to neuronal injury and may serve as a measurable biomarker of brain pathology. This study investigated cognitive scores and serum lipid biomarkers in retired rugby players with a history of concussion to assess their association with concussion exposure. Serum levels of 24-hydroxycholesterol (24-HC), 25-HC, 27-HC, total triglycerides, and ceramide were compared between retired rugby players with a history of repeate

This article provides a recap of the 10 Questions/10 Experts session at the 2024 American College of Sports Medicine Annual Meeting. Each of the speakers considered the validity of common "myths," while providing evidence-based opinions to support, or, bust, myths addressing the following questions: (1) Would 100 g/hr of carbohydrate be advisable for the Olympic Cycling Road race? (2) Is there an advantage in the marathon of ingesting bicarbonate as a hydrogel product? (3) Can genotyping be used to individualize caffeine supplementation in football? (4) Should low fluid consumers drink more to

A Qualitative Study of Youth Football Coaches' Perception of Concussion Safety in American Youth Football and Their Experiences With Implementing Tackling Interventions.

Due in part to concern about the potential long-term effects of concussion and repetitive head injuries in football, some programs have implemented tackling interventions. This paper explores youth football coaches' perception of football safety and their experiences implementing these interventions aimed at athlete safety. Using a qualitative approach, coaches were interviewed by means of a semi-structured protocol that covered: (a) demographics; (b) background and experiences; (c) personal relevance risks, safety, and benefits of youth football; (d) experiences with tackling technique; (e) e

Exercise for Sport-Related Concussion and Persistent Postconcussive Symptoms.

CONTEXT: Emerging research supports the use of mild to moderate aerobic exercise for treating sport-related concussion (SRC) and persistent postconcussive symptoms (PPCS), yet the current standard of care remains to be strict rest. The purpose of this review is to summarize the existing literature on physical activity and prescribed exercise for SRC and PPCS. EVIDENCE ACQUISITION: PubMed and Embase were searched in April of 2019 for studies assessing rest or prescribed exercise for SRC and PPCS. No specific search syntax was used. STUDY DESIGN: Clinical review. LEVEL OF EVIDENCE: Level 4.

Effects of prophylactic ankle and knee braces on leg stiffness during hopping.

During human movement, the leg can be represented as a mechanical spring, with its stiffness potentially contributing to sports performance and injury prevention. Although many individuals perform athletic activities with joint stabilizers, little is known about the effects of prophylactic lower extremity braces on leg stiffness. The objective of this study was to investigate the effect of ankle and/or knee braces on leg stiffness measured during one-legged hopping at a range of frequencies. Thirteen male participants performed one-legged hopping with their dominant leg at frequencies of 2.2,

Osteochondritis dissecans of the elbow: histopathological assessment of the articular cartilage and subchondral bone with emphasis on their damage and repair.

Osteochondritis dissecans (OCD) of the elbow is a localized injury of the articular cartilage and subchondral bone that is commonly seen in the young athlete. In the present study, the extent of damage and repair on the articular cartilage and subchondral bone was examined histologically using specimens of 25 osteochondral cylinders and seven loose bodies obtained from 25 young athletes who had undergone osteochondral autograft surgery. Terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end-labeling (TUNEL) assays for detecting apoptotic cells and immunohistochemistry of matrix me

Between a rock and a hard place: A case of critical limb threatening ischemia treated with deep vein arterialization.

Deep vein arterialization provides an alternative approach to avoid limb amputations in patients with no other revascularization options. An athletic 28-year-old had an anterior cruciate ligament tear in an on-the-job injury. After an anterior cruciate ligament repair, he was found to have an occluded popliteal artery. An above-knee popliteal artery to posterior tibial vein bypass was performed because of a lack of bypass options. This led to the resolution of his rest pain. This salvage maneuver gave the patient at least 1 more year without amputation, showing that deep vein arterialization c

A comparison of head impact characteristics during elite men's Rugby Union fifteens and sevens match play.

Different forms of rugby may pose distinct risks to head injury. Video of rugby match footage was analyzed using head impact magnitude, frequency, and time interval for 15s and 7s athletes. Impacts were reconstructed in laboratory, and finite element modeling was used to estimate maximum principal strain. No difference was found in impact frequency or time interval between the two forms. Significantly more head impacts of higher severity levels were documented during 7s. These findings provide objective comparisons between 7s and 15s which may guide risk mitigation strategies in managing brain

Nonoperative Treatment of Psoas Tendon Avulsion in a Professional Athlete: A Case Report and Evidence Based Review.

CASE: A 36-year-old male professional soccer player sustained an acute left hip injury during a tournament game while defending the goal. Magnetic resonance imaging demonstrated a complete avulsion of the iliopsoas tendon from the left trochanter. The patient was treated nonoperatively, and after 10 weeks, he was able to return to full activity. After 1 year of follow-up, the patient was completely asymptomatic, and was still playing professional soccer without flexion strength deficit. CONCLUSION: In this professional soccer athlete, nonoperative treatment of complete psoas avulsion resulted

Reconstruction of Chronic Boutonniere Deformity of the Thumb in a Golf Player.

We present the case of a professional golf player who gradually developed a boutonniere deformity of the thumb due to chronic repetitive injury to the thumb and was treated with reconstruction of its insertion onto the proximal phalangeal base. The athlete showed an insertion variation of the extensor pollicis brevis, where some slips attached onto the extensor hood and the other slips ran along with the extensor pollicis longus to the distal phalanx, providing no slip to the proximal phalanx. The slips inserting to the distal phalanx were transferred to the base of the proximal phalanx and sa

Open 4-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome of the Leg.

Chronic exertional compartment syndrome (CECS) is a significant source of lower extremity pain and morbidity in the athletic population. Although endoscopic techniques have been introduced, open fasciotomy remains the mainstay of surgical treatment because of the paucity of evidence in support of an endoscopic approach. The literature on surgical management of CECS is mixed, and overall success rates are modest at best. Optimizing surgical technique, including prevention of neurovascular injury and wound complications, can make a significant impact on the clinical outcome. Here we present our

Presumed prepatellar fibrosis in collegiate wrestlers: imaging findings and clinical correlation.

Knee pain and injury are common complaints of athletes presenting to the orthopedic clinic. Wrestlers are no exception, and may present more commonly with anterior knee pain because of the nature of their sport. Morel-Lavallée lesions and prepatellar bursitis have been described in the prepatellar region of wrestlers. We report a distinctly different prepatellar finding on MRI, focally prominent and discrete fibrosis in three collegiate wrestlers. This characteristic appearance and clinical setting have already been repeated in our clinical practice; thus, other MRI readers are also likely to

Bucket-handle meniscal tear in a 5-year-old child.

Bucket-handle meniscus tears are a common athletic injury that occur frequently in the adult population but are extremely rare in young children. A 5-year-old male patient presented with left knee pain after a minor fall to the ground. Complaints of pain with full weight-bearing, locking of the joint during walking, a significant limp, 45° flexion contracture of the knee, and an inability to bring the leg into full extension were noted during examination. MRI showed a large bucket-handle medial meniscal tear. The patient made a full recovery after undergoing arthroscopic repair surgery. Level

Forefoot injuries in dancers.

Dancers, particularly ballet dancers, are artists and athletes. In dance, the choreographer acts as a sculptor, using the dancer as a medium of expression. This often entails placing the dancer's body in positions that require extraordinary flexibility and movement, which requires controlled power and endurance. Ballet and other forms of dance can be highly demanding activities, with a lifetime injury incidence of up to 90%. Ballet is stressful particularly on the dancer's forefoot. The en pointe position of maximal plantarflexion through the forefoot, midfoot, and hindfoot requires tremendous

Diagnosis and management of traumatic and atraumatic hip instability in the athletic patient.

Although relatively uncommon compared with the shoulder, hip instability can be a source of significant disability and is a commonly unrecognized injury. Hip instability can be traumatic or atraumatic in origin. Our understanding and treatment plan for hip instability due to traumatic events is well established. However, our understanding and treatment modalities for hip instability due to atraumatic events or repetitive motion in high level athletes are not as well defined. In this article, we will review the spectrum of traumatic and atraumatic hip instability and discuss the relevant anatom

Interspinous bursitis in an athlete.

We present a case of L2/3 interspinous bursitis treated with extraspinal injections. No previous investigations have used fluoroscopically guided spinal injections to confirm the clinical relevance of the MRI features of this type of bursae. Autopsy studies have revealed an increased incidence of interspinous lumbar bursal cavities with advancing age. Afflicted patients present with localised, midline lower lumbar pain exacerbated by extension. In young athletes these symptoms can mimic spondylolysis. MRI is useful in detecting soft-tissue injury of the posterior elements. Fluoroscopically gui

Adductor canal outlet syndrome.

On rare occasions, jogging, skiing, and other athletic activities can cause thrombosis of the superficial femoral artery at the outlet of Hunter's canal. We report three cases in which mechanical trauma of the femoral artery in Hunter's canal resulted in intimal tear and a segmentally localized thrombosis. It was confirmed by anatomic dissection that the intrinsic mechanical trauma occurred at the point where the femoral artery crosses the adductor magnus tendon. This tendon has been previously implicated as a cause of atherosclerotic stenosis and thrombosis. An awareness of its possible relat

Return to sport following scaphoid fractures: A systematic review and meta-analysis.

BACKGROUND: Scaphoid fracture is the most commonly fractured carpal bone in the athletic patient, accounting for over 85% of all sport-related carpal bone fractures, and is particularly common in sports involving high impact injuries to the wrist. The management of such injuries comprises both conservative and surgical techniques, as guided by fracture location and type. Athletes demonstrate a unique challenge with regards to the management of scaphoid fractures due to their requirement to return to sport, as soon as able. AIM: To review systemically all studies recording return to sport foll
