A case of spontaneous hypotension of the cerebrospinal fluid (CSF) is reported. A 32 years-old woman was complaining of intense orthostatic headaches. CT scan showed slit ventricles and the spinal tap showed a pressure of the CSF too low to be measured. A sample of clear CSF could only be obtained by aspiration. The patient's status improved without drugs by simply staying in bed for one week. Few similar cases have been reported. The mechanisms of this disorder still remains unknown.

Replacement of the oesophagus with a colonic graft brought up to a retrosternal or intrathoracic position is usually performed in children with undilatable oesophageal stenosis resulting from caustic burns. However, incidents associated with the oesophagus remaining in situ regularly raise the problem of concomitant or delayed oesophagectomy and therefore thoracotomy. A one-stage surgical technique is described, which includes closed-thorax oesophagectomy and colonic oesophagoplasty in situ in the oesophageal bed. The 3 patients operated upon by this method made an uneventful recovery.

A 20-bed alcohol rehabilitation unit with a high patient retention rate had the experience of a sudden increase in irregular discharges. Administration of the Ward Atmosphere Scale developed by Rudolph Moos showed a drop in various therapeutic aspects of the ward environment that paralleled the increase in irregular discharges. These results were discussed with staff, and soon after the number of irregular discharges diminished. The Ward Atmosphere Scale was readministered and showed a move toward a more therapeutic atmosphere. The reasons for these fluctuations are discussed.

A survey of triplet pregnancies in Linköping, Sweden, 1973-1981, showed a low perinatal mortality (4%) and low morbidity possibly due to prophylactic bed rest and tocolytic treatment. All children were followed at child health clinics. In one child, a persistent growth retardation was recorded at 4 years of age. No psychomotor deviations were found in the group. An interview concerning social and economic factors after delivery was carried out. The answers revealed considerable strain in daily life for all families.

Stomach and pectoralis major myocutaneous flaps have both been effective in providing hypopharyngeal lining and muscle bulk to close large oropharyngeal fistulae. Their robust musculature has also proven an ideal bed for the application of a split-thickness skin graft, when dealing with the replacement of major anterior neck skin defects. In addition, the pectoralis myocutaneous flap, when half tubed, is an excellent method of reconstructing resected tongue base, markedly improving the function of tongue remnant.

An analysis is made of the management and outcome of 35 pregnancies involving triplets, quadruplets, and quintuplets. Bed rest does not seem to increase the gestational age at the time of delivery but definitely improves the fetal outcome. Administration of betamethasone as early as 24 to 26 weeks' gestation to enhance pulmonary maturity is recommended. The method of transcutaneous PO2 measurement is suitable to record the differing oxygen supply, and thereby the blood supply of direct skin flaps. The increasing blood supply in the base as well as in the peripheral part of the flap up to the tenth postoperative day is characterized by increasing PO2 values. A review of the literature concerning the classification, etiology, histology and treatment of congenital pseudarthrosis of the tibia is presented. A method has been elaborated in experiments on animals for the implantation of the autovein into the injured vessel lumen with the aid of a specially made device for saving the transplant ends in the artificially made wall of the recipient-vein circular folds. The internal wall is smooth on its total length, has no weak points on the sutures, preserves the topical character and direction, and the vascular bed is less traumatized. The peripheral arteio-venous shunting made simultaneously due to the accelerated venous blood flow, reduces the transplant postoperative thrombosis to 30%.

In a randomized, blind clinical study, we tested the effect of timolol and Daranide against placebo medications among patients who had intraocular pressures greater than 30 mm Hg on the first postoperative day after aphakic keratoplasty. The study failed to show any significant difference in the pressure course. We concluded that because this type of severe secondary glaucoma is extremely difficult to treat, it should be prevented by using a donor cornea larger in size than the recipient bed.

Certain post-traumatic defects of the lower extremity present difficult problems in therapy and current methods of management do not offer satisfactory solutions. One such defect is an ulcerative lesion of the lower limb eroding the tibia and forming a chronic, rigid-walled cavity. We describe an operation in which the bone cavity is filled by viable tissue which acts as a bed for a skin graft. In chronic stasis ulcers it has been established that the tendo calcaneus will accept a skin graft, provided the paratenon is left undisturbred.

The English literature concerning femoral neuropathy associated with anticoagulant therapy is reviewed. Anatomical dissections were carried out in order to determine how the neuropathy might be produced. It is postulated that hemorrhage occurs within the iliacus muscle and that a pressure neuropathy of the femoral nerve ensues. Conservative management is recommended. The condition is probably more prevalent than previously thought, as evidenced by the fact that two cases were diagnosed in as many years by one orthopedic surgeon in a 300-bed general hospital.

Autologous parathyroid tissue frozen in liquid nitrogen for 6 weeks was transplanted successfully into the forearm muscle of an aparathyroid, uremic patient with renal osteodystrophy. The literature review and a case report of a 25 years old patient who started to suffer from an extemely large abdomen, sever oedema, dyspnea, and uterine prolapse from the 30th week in her third pregnancy because of a very large mucinous cyst. The prolapsed uterus improved with bed rest. She delivered at term with no complication. The cyst was removed three weeks after the delivery with about ten litres of mucoid secretion in it. The patient left hospital on the tenth post operative day.

Diagnostic and therapeutic problems in a patient with ectopic ACTH syndrome caused by a malignant bronchial adenoma are discussed. Persistent Cushing's syndrome was present following apparent total adrenalectomy, but radioactive scanning with 131I-19-iodocholesterol showed the presence of residual adrenal tissue in the right suprarenal bed. This paper reports a case of an extremely rare primary mixed germ cell tumor (MGCT) of the stomach in an infant. The patient, an 8-month-old male infant, presented with “black stool for three days.” Abdominal computed tomography (CT) suggested an abdominal space-occupying lesion, and gastroscopy revealed a huge cauliflower-like protrusion on the lesser curvature of the gastric body.
