Case report on antipsychotic induced catatonia in an incarcerated patient.

Antipsychotic-induced catatonia is an iatrogenic and debilitating adverse reaction, but there is a dearth of recent documented cases. This report describes a 35-year-old incarcerated Korean-American male with a history of unspecified psychosis who presented for antipsychotic induced catatonia after administration of haloperidol decanoate intramuscular (200 mg across the span of 1 week). Neurologic workup was performed including MRI, lumbar puncture, and electroencephalography. Despite an approximate month long hospitalization, benzodiazepine challenge, benztropine trial, and amantadine adjunct

Reversal Agents in Sedation and Anesthesia Practice for Dentistry.

Reversal agents are defined as any drug used to counteract the pharmacologic effects of another drug. Several pharmacologic antagonists serve as essential drugs in the contemporary practices of sedation providers and anesthesiologists. Reversal or "antidote" drugs, such as flumazenil and naloxone, are often used in unintentional overdose situations involving significant benzodiazepine- and/or opioid-induced respiratory depression. Within the context of skeletal muscle relaxation, neostigmine and sugammadex are routinely used to reverse the effects of nondepolarizing neuromuscular blocking agen

Clinical Pearls on Sleep Management in Atopic Dermatitis.

Multiple etiologies contribute to sleep disturbance in atopic dermatitis (AD) patients, including learned scratching behavior and increased monoamines, cutaneous blood flow, inflammatory cell activities, and cytokines, as well as decreased melatonin, anti-inflammatory cytokines, and skin barrier function. Insomnia impairs cognitive development in children with AD, leading to behavioral problems and learning disabilities. Insomnia in adults with AD impedes work productivity. In this article, we discuss pearls on improving insomnia through both nonpharmacologic modalities, such as environmental

Differential substance abuse patterns distribute according to gender in heroin addicts.

This study attempts to analyse potential gender differences among a group of heroin addicts seeking treatment at a university-based medical centre. The central modality of treatment at this centre is the use of methadone maintenance. Among those patients entering this program there seems to be an emerging pattern of males who tend to use heroin as their opiate of choice, and are more likely to combine it with cannabis, while females are more likely to use to street methadone, with adjunctive use of ketamine, benzodiazepines, hypnotic drugs and/or amphetamines. Women are at higher risk of abusi

Video feedback with peer ratings in naturalistic anxiety-provoking situations for social anxiety disorder: Preliminary report.

The present study aimed to examine how video feedback can affect perceived performance and anticipatory anxiety in various naturalistic social anxiety-provoking situations among clinical patients diagnosed with social anxiety disorder (SAD) and to examine predictors that might influence response to video feedback. Participants were 52 consecutive patients with DSM-IV SAD who participated in a group-based CBT program. Our results demonstrated that video feedback was associated with a decrease in the underestimation of own performance as well as the perception of feared outcomes. Moreover, antic

[Novel insomnia drugs, including drugs currently under development].

Insomnia has mainly been treated with the hypnotic benzodiazepine (BZ). Recent studies have revealed the role and mechanisms of BZ receptors and have led to the development of non-BZ hypnotics. The chemical structures of non-BZ hypnotics differ from that of BZ; these hypnotics selectively bind to the omega 1 receptor of the gamma-aminobutyric acid (GABA)-BZ receptor complex. Because the omega 2 BZ receptors have adverse effects such as muscle relaxant actions, non-BZ hypnotics have lesser adverse effects than BZ. Antipsychotics, antidepressants, and antihistamines are also used for the treatme

The effectiveness of anticonvulsants in psychiatric disorders.

Anticonvulsant drugs are widely used in psychiatric indications. These include mainly alcohol and benzodiazepine withdrawal syndromes, panic and anxiety disorders, dementia, schizophrenia, affective disorders, bipolar affective disorders in particular, and, to some extent, personality disorders. A further area in which neurology and psychiatry overlap is pain conditions, in which some anticonvulsants, and also typical psychiatric medications such as antidepressants, are helpful. From the beginning of their psychiatric use, anticonvulsants have also been used to ameliorate specific symptoms of

Pharmacotherapy for performance anxiety disorders: occasionally useful but typically contraindicated.

Pharmacotherapy is an effective part of treatment for most anxiety disorders, but not for specific phobia or performance anxiety. In them it is contraindicated, because it interferes with the effectiveness of exposure therapies and the extinction of fear responses. Interference with exposure-extinction is a drug side effect that should rarely if ever be tolerated in treating them. This article reviews the many indications for pharmacotherapy in treating most anxiety disorders and contrasts its usefulness in treatment of anxiety disorders with its relatively rare usefulness in treating specific

Allosteric modulation in spontaneously active mutant gamma-aminobutyric acidA receptors.

Tryptophan substitutions were made in the second transmembrane domain of the gamma-aminobutyric acid(A) (GABAA) receptor alpha and beta subunits and the resulting mutant receptors, containing alpha2(S270W) and/or beta1 (S265W), were expressed in Xenopus oocytes. Mutation of either or both subunits resulted in receptors that exhibited enhanced sensitivity to agonist and were spontaneously active in the absence of GABA. The spontaneous activity was blocked by picrotoxin or bicuculline. The enhancement of GABA-induced currents by pentobarbital, by the neurosteroid 5alpha-pregnan-3alpha-ol-20-one,

Drug-facilitated sexual assault ('date rape').

In the past few years, drug-facilitated sexual assaults have received widespread media coverage. In addition to alcohol, the most frequently used date-rape drug, flunitrazepam (Rohypnol), a fast-acting benzodiazepine, and gamma-hydroxybutyrate (GHB) and its congeners are among the most popular drugs used for this purpose. The latter drug is easily procured at some gymnasiums, popular bars, discos, and rave clubs, as well as over the Internet. Perpetrators choose these drugs because they act rapidly, produce disinhibition and relaxation of voluntary muscles, and cause the victim to have lasting

[The use of chlorazepate dipotassium (Tranxene) in the states of restlessness and agitation].

Clorazepate dipotassium (Tranxene), benzodiazepine of retarded anxiety-relieving, sedative and sleep-inducing action was used in an open sample of 36 patients for fast control of anxiety and aggression in the course of schizophrenia, schizoaffective psychosis and other psychotic disorders. The intensity of aggressive behaviors was assessed as 6-7 items on the CGI scale. After intramuscular injection of clorazepate complementing the so-called basic treatment in the mean daily dose of 150 mg, several hours lasting sedative effect was achieved. No unfavourable interactions between the applied clo

Social phobia: everyone's disorder?

Several findings suggest that serotonin dysfunction may play at least a partial role in the etiology of social phobia. The cortisol response to fenfluramine, a serotonin agonist, is enhanced in patients with social phobia. Serotonin may be a common denominator between the blushing commonly seen in social phobics and the cutaneous flushing occurring in patients with carcinoid syndrome, although this is unlikely. Drugs that have demonstrated effectiveness in social phobia include the serotonin selective reuptake inhibitors (SSRIs), clonazepam (a benzodiazepine that potentiates serotonin function

Eyelid myoclonia with typical absences: an epilepsy syndrome.

Five unrelated patients are described with the clinical and electrical features of eyelid myoclonia with absences (EMA). In this syndrome brief, typical absences occur with rapid eyelid myoclonia associated with retropulsive movements of the eyeballs and occasionally of the head. The seizures are of shorter duration than in childhood absence epilepsy, and are accompanied by less profound impairment of consciousness. The electroencephalogram demonstrates high amplitude discharges consisting of spikes, multiple spikes and slow waves at a fluctuating frequency of 3-5 Hz and following eye closure,

Can severe cardiorespiratory dysregulation induced by clozapine monotherapy be predicted?

Severe orthostatic and cardiorespiratory dysregulation may occur during institution of clozapine therapy both during concomitant benzodiazepine medication and on reinstitution of clozapine after a washout period. Extensive medical and neurological workup before and after the trials can nevertheless be normal. The absence of similar previous reactions to other drugs, adhering to monotherapy with clozapine, increasing dosage slowly or performing a single test for orthostatic reactions after initiating clozapine are insufficient precautionary measures. Cardiorespiratory complications can occur wi

The role of the lateral septum in anxiolysis.

Behavior of rats in the water-lick conflict test was examined during stimulation, and after lesions of the lateral septal nucleus. Continuous low-current stimulation resulted in an anxiolytic effect, an increase in the number of licks, and hence in the number of shocks, during a signaled, punished period. This effect is similar to the one seen with peripheral administration of benzodiazepine and other anxiolytic agents. Rewarding medial forebrain stimulation did not have this effect. Conversely, septal lesions resulted in an anxiogenic effect, a significant decrease in the number of licks duri

GABA activity in dispersed bovine pineal cells: effect on serotonin release and calcium and chloride uptake.

The presence of gamma-aminobutyric acid (GABA) and peripheral and central benzodiazepine receptors in the mammalian pineal gland prompted the examination of GABAergic transmission in dispersed bovine pineal cells. The effect of GABA on 3H-serotonin (5HT) release was examined in bovine pineal cells. GABA, by acting through GABA B receptor subtype, decreased 5HT release and by acting through GABA B, and presumably through GABA A receptor subtypes, inhibited depolarization-induced 45Ca2+ uptake in bovine pinealocytes. GABA, by acting on GABA B as well as on GABA A receptors, prevented the 5HT2- o

Novel GABAA receptor alpha subunit is expressed only in cerebellar granule cells.

GABA (gamma-aminobutyric acid), the major inhibitory neurotransmitter in the vertebrate brain, mediates neuronal inhibition by opening a chloride channel integral to the GABAA receptor. This action is potentiated by both benzodiazepine and barbiturate drugs. Since the isolation of cDNAs encoding GABAA receptor alpha 1 and beta 1 subunits, a further eight subunits have been identified. These subunits show GABAA receptor heterogeneity, unpredicted from classical pharmacological studies. I now report the isolation of a mouse cDNA clone encoding a novel GABAA receptor alpha subunit. The striking f

Double-blind study on the hypnotic and antianxiety effects of zopiclone compared with nitrazepam in the treatment of insomnia.

The effects of zopiclone (7.5 mg/per os/day), a new non-benzodiazepine drug, on anxiety levels, time of sleep induction, hours of sleep, number of nocturnal arousals, quality of sleep and of daytime arousal were evaluated by using psychometric ratings. A random double-blind cross-over study versus nitrazepam (5.0 mg/per os/day) was performed on 20 patients affected by insomnia and generalized anxiety disorder (DSM III-R). The effects were evaluated by using the Hamilton Rating Scale for Anxiety, the Toulouse-Pieron Attention Test and a time-signed quantitative scale for anxiety. The results of

Effects of the beta-carboline FG 7142 on saccharin preference and quinine aversion in water-deprived rats.

Three groups of water-deprived male rats were given the choice of 0.05% sodium saccharin solution and water; 0.0005% quinine solution and water; water in both containers. N'-Methyl-beta-carboline-3-carboxamide (FG 7142), a benzodiazepine receptor inverse agonist, dose-dependently (2.5-10 mg X kg-1 i.p.) reduced intake of fluid in each group. The saccharin-choice group were most sensitive, showing a significant reduction at 2.5 mg X kg-1. Preference for the saccharin solution was reduced by FG 7142 but it did not increase aversion to the quinine solution. Hence, FG 7142 did not generally increa
