Robust human genetic evidence supporting causal effects of FGF21 on reducing alcohol consuming behaviours.

INTRODUCTION: Alcohol use disorder (AUD) represents a tremendous societal burden, yet few efficacious therapies are available and widely used. Pre-clinical and human observational data support fibroblast growth factor 21 (FGF21) as a promising therapeutic target for the treatment of AUD. Here, we identify a robust genetic instrument for FGF21 agonism and leverage it in the Mendelian randomization paradigm to explore the effects of FGF21 agonism on AUD and related traits, as well as metabolic outcomes more widely. METHODS: We first compared associations with the positive control outcomes of li

Radiotherapy for Liver-Confined Hepatocellular Carcinoma in Elderly Patients with Comorbidity.

Background: Globally, the incidence rate of hepatocellular carcinoma (HCC) has increased among elderly patients. Elderly patients often present with multiple comorbidities that affect treatment tolerance and outcomes, and the optimal management strategy for this population has not yet been established. Therefore, we assessed comorbidities in elderly patients and investigated the treatment outcomes of radiotherapy (RT) to liver-confined HCC. Methods: We retrospectively reviewed 40 elderly patients aged ≥70 years with liver-confined HCC, who were treated with RT between 2015 and 2023. Comorbidit

Epidemiology and clinical outcomes of injuries in motorcycle delivery riders in the United Arab Emirates: a four-year trauma registry analysis.

BACKGROUND: Motorcycle delivery riders represent a growing and vulnerable population of road users, particularly in urban centres experiencing rapid e-commerce expansion. Despite global reductions in road traffic injuries, this group remains at high risk due to occupational hazards and limited applications of traffic laws and protective measures. We examined the epidemiology, injury patterns, and clinical outcomes of motorcycle delivery riders admitted with trauma in Al Ain, United Arab Emirates (UAE), from 2020 to 2023. METHODS: We conducted a retrospective analysis of prospectively collecte

Anticancer Molecular Mechanisms of Epigallocatechin Gallate: An Updated Review on Clinical Trials.

Human physiology is a complex process encompassing various biochemical mechanisms, regulated by hundreds of chemical mediators, thus making it more complicated to understand. Balance between these mediators is mandatory to continue the helm of life; however, an imbalance can disrupt normal physiological functions and lead to chronic health conditions. Cancer is one such chronic state that occurs due to an imbalance or overproduction of reactive oxygen and nitrogen species (RONS), which are highly unstable and damage other chemical constituents in the body. The mutilation at the cellular level

Placebo effects improve sickness symptoms and drug efficacy during systemic inflammation: a randomized controlled trial in human experimental endotoxemia.

BACKGROUND: Systemic inflammation triggers a wide range of sickness symptoms, including bodily discomfort and affective symptoms, which are relevant to numerous health conditions. While extensive research in the placebo field demonstrates that positive expectations can improve symptoms, it remains unclear if interventions designed to augment positive treatment expectations can alleviate sickness symptoms in the context of immunomodulatory drug therapies. METHODS: In this randomized, controlled, fully balanced 2 × 2 factorial placebo design, N = 124 healthy volunteers received either active ib

Impact of Dapagliflozin on Cardiometabolic Outcomes After Acute Myocardial Infarction According to Baseline Glycemic Status and Body Mass Index: Subanalyses of the DAPA-MI Trial.

BACKGROUND: Dapagliflozin improved cardiometabolic outcomes following myocardial infarction in patients without prior type-2 diabetes (T2DM) in the DAPA-MI (dapagliflozin in patients with myocardial infarction) trial. The effect of glycemic status and body mass index (BMI) post-myocardial infarction requires elucidation. METHODS: Participants with T2DM diagnosis, without baseline hemoglobin A1c, or not receiving any study medication, were excluded. Eligible participants were categorized, according to baseline hemoglobin A1c, as normoglycemic (<5.7% [39 mmol/mol]) or prediabetes (5.7 to <6.5%

Baseline and 10-year change in the number of ideal cardiovascular health metrics and sudden cardiac death in the community.

AIMS: Adherence to an ideal cardiovascular health (CVH) might contribute to lower the burden of sudden cardiac death (SCD) in the community. We aimed to examine the association between the number of ideal CVH metrics at baseline and of its change over 10 years with the risk of SCD. METHODS AND RESULTS: The Copenhagen City Heart Study is a community-based prospective cohort study. The number of ideal CVH metrics (range 0-6; non-smoking and ideal level of body mass index, physical activity, untreated glucose, untreated systolic blood pressure, and untreated total cholesterol levels) at baseline

Care provided to women during and after a pregnancy complicated by hyperglycaemia: the impacts of a multi-component health systems intervention.

BACKGROUND: Aboriginal and Torres Strait Islander women experience a disproportionate burden of hyperglycaemia in pregnancy. A multi-component health systems intervention aiming to improve antenatal and postpartum care was implemented across Australia's Northern Territory (NT) and Far North Queensland (FNQ) between 2016 and 2019. Components included clinician education, improving recall systems, enhancing policies and guidelines, and embedding Diabetes in Pregnancy (DIP) Clinical Registers in systems of care. This program was evaluated to determine impacts on clinical practice and maternal hea

PROTOCOL: Effectiveness of Interventions for the Prevention and Treatment of Obesity in Children and Adolescents From Low- and Middle-Income Countries: A Systematic Review and Meta-Analysis.

Childhood obesity represents a major and growing public health challenge, with a disproportionate burden in low- and middle-income countries (LMICs). This systematic review will address a critical evidence gap by synthesising existing research on the effectiveness of interventions for the prevention and treatment of childhood obesity in LMICs. By focusing exclusively on interventions implemented within LMIC contexts, the review will account for the unique socio-cultural, economic, and environmental determinants that influence intervention delivery and effectiveness in these settings. The effects of transcranial direct current stimulation on serum superoxide dismutase levels in ambulatory ischaemic stroke patients: A randomised pilot study.

INTRODUCTION: Stroke is one of the leading causes of disability worldwide, imposing a significant public health and economic burden. Oxidative stress, which results from an imbalance between free radical production and antioxidant defences, exacerbates stroke outcomes via its role in promoting neuroinflammation and tissue damage. Superoxide dismutase (SOD), a critical antioxidant enzyme, plays a key role in mitigating oxidative stress. This study evaluates the effect of transcranial direct current stimulation (tDCS) on serum SOD levels in ambulatory ischaemic stroke patients with mild cognitiv

Chronic Traumatic Encephalopathy Neuropathologic Change is Infrequent Following Isolated Moderate-Severe Traumatic Brain Injury.

Chronic traumatic encephalopathy-neuropathologic change (CTE-NC) has been primarily studied in contact sport athletes with repetitive head impacts (RHI). An association with isolated traumatic brain injury (TBI) is less clear. We systematically reviewed the autopsied cohort of Late Effects of TBI (LETBI), characterized primarily by isolated TBI but also including RHI, for features of CTE-NC. A consecutive series of 44 brains underwent comprehensive neuropathologic evaluation, exceeding recommended CTE consensus protocols. Of the 44, 6 (13.6%) (age range, 3 rd -7 th decades; median, 6 th decade

Beyond the Gut Feeling: A Retrospective Analysis of Adult Foreign Body Ingestion, Associated Risk Factors, and Treatment Outcomes.

Background Adult foreign body ingestion (FBI) shows different patterns from pediatric cases and poses variable needs for endoscopic or surgical intervention, depending on object type, risk factors, and anatomical location. International guidelines recommend urgent removal for high-risk objects, such as sharp items, magnets, or batteries, and early endoscopy for selected esophageal impactions. To our knowledge, this study, for the first time in the United Arab Emirates (UAE), investigated the epidemiology, risk factors, management strategies, and outcomes of adult FBI cases at a tertiary hospit
