Early postcholecystectomy type I Mirizzi syndrome with cystic duct stump leak and retained stones.

A 37-year-old woman underwent laparoscopic bailout subtotal cholecystectomy at a county hospital on December 20, 2025, for acute cholecystitis with cholelithiasis. Dense Calot's triangle adhesions prevented safe ductal exposure. Suspected cystic-duct impaction prevented intraoperative cholangioscopic entry; the remnant was sutured (no clip/stapler documented), and no formal intraoperative cholangiography was performed. MRCP showed a long, low-inserting cystic duct, a 6-mm confluence calculus and upstream biliary dilatation. Tissue friability and persistent obstruction may have contributed to t…

Gallbladder Perforation Due to Ascaris lumbricoides in a Pregnant Woman and 6 Year Old Girl from Afghanistan: Case Report.

Ascariasis is an important and prevalent geo-helminth infection, especially in underdeveloped countries such as Afghanistan, where public health problems and devastation of hygienic infrastructures usually lead to the higher incidence of the infection. Here, we report two cases of intestinal obstruction that accompanied with biliary ascariasis due to Ascaris lumbercoides in pregnant women and a 6-year-old girl from Afghanistan. Biliary ascariasis can be associated with cystic duct obstruction, gallbladder dilation, acute cholecystitis, acute cholangitis, obstructive jaundice, and biliary colic…

[Case of Adenocarcinoma with Neuroendocrine Carcinoma Component in the Gallbladder].

A 73-year-old man visited our hospital complaining of abdominal pain. An abdominal CT scan revealed diffuse wall thickening of the gallbladder with a gallstone. Urgent laparoscopic cholecystectomy was performed with the diagnosis of acute cholecystitis. Histopathological examination indicated adenocarcinoma with a small neuroendocrine carcinoma component invading the subserous layer. Additional gallbladder bed resection and lymphadenectomy were performed. The final pathological diagnosis was adenocarcinoma with NEC, pT2N0M0, pStage II . The patient is alive without any recurrence 7 months afte…

Reversible IgA deficiency after severe Gram-negative bacteria infection in a patient with systemic sclerosis.

A 43-year-old woman with systemic sclerosis (SSc) developed IgA deficiency (IgAD) after cholecystitis. The severe decrease of IgA (<10 mg/dl) partially recovered after 5 years. She had repeated episodes of infection during IgAD. Anti-IgA antibody was not detected. Flow cytometric analysis showed that peripheral CD19(+)IgA(+) and CD38(+)IgA(+) cells were normally present. Although the mechanism of secondary IgAD is still vague, its association with autoimmune diseases including SSc and also with bacterial infection is discussed.

Hepatic evisceration after cholecystectomy in a superobese patient.

Gallbladder pathology, in general, and cholelithiasis, in particular, are more common in the morbidly obese. Obesity is a risk factor for conversion to open surgery in laparoscopic cholecystectomy. Obesity is also a risk factor for evisceration after laparotomy in adults. Hepatic evisceration after cholecystectomy is rare. We describe a case of right liver lobe evisceration diagnosed by abdominal computed tomography in a superobese patient (body mass index 57 kg/m(2)) after emergency laparoscopic surgery for acute calculous cholecystitis converted to open surgery.

Double gallbladder-a laparoscopic management.

A rare case of gallbladder duplication, an unusual biliary anomaly is reported in a young female patient presenting with acute cholecystitis. After a confirmed diagnosis of double gallbladder was made by sonography, endoscopic retrograde cholangio-pancreaticography (ERCP), and magnetic retrograde cholangio-pancreaticography(MRCP), both gallbladders were removed laparoscopically. On histology both gallbladders showed cholesterolosis. Detailed preoperative investigations are required for an accurate preoperative diagnosis before considering laparoscopic cholecystectomy to avoid inadvertent damag…

[Difficult laparoscopic cholecystectomy].

The study analyses different situations encountered throughout the authors personal operative experience, susceptible to generate technical and tactical difficulties during laparoscopic cholecystectomy such as: postoperative perivisceritis, obesity, difficult liver mobilization, lipomatosis, acute and scleroatrophic cholecystitis, "porcelain gallbladder". They also point out different causes capable of generating important bleeding during the operation: cirrhosis, accidental adhesion tearing, anatomical arterial variations. Above all, the study presents the possible technical and tactical solu…

[The sequelae of a mild closed craniocerebral trauma and the prognosis of its late outcomes in patients with a unfavorable preinjury background].

Combined catamnestic studies were made in 310 individuals 16 to 80 years old with a history of mild closed craniocerebral injury (MCCCI) for their health status. Of these, 200 subjects had, prior to MCCCI, arterial hypertension, chronic first-second degree cerebrovascular insufficiency, gastric and duodenal ulcer, chronic cholecystitis, hepatocholecystitis, gastroduodenitis. In a catamnestic follow-up 1 year following MCCCI many patients exhibited abnormal psychoneurologic and vegetovisceral changes. Prognostic factors have been derived for late outcomes of MCCCI using A. Val'd's sequential an…

Laparoscopic cholecystectomy was performed on 65 unselected and consecutive patients, regardless of age, weight, history of abdominal surgery or presence of acute cholecystitis. All procedures were completed successfully, with only two patients converted to an open cholecystectomy. There were no intra-abdominal intraoperative complications; n o intraoperative transfusions were required. There were no intra-abdominal injuries, and no patient required repeat surgery for postoperative complications. Hospital stays averaged 30 hours, and the average time until patients resumed normal activities wa…

Biliary tract infections: a guide to drug treatment.

Initial therapy of acute cholecystitis and cholangitis is directed towards general support of the patient, including fluid and electrolyte replacement, correction of metabolic imbalances and antibacterial therapy. Factors affecting the efficacy of antibacterial therapy include the activity of the agent against the common biliary tract pathogens and pharmacokinetic properties such as tissue distribution and the ratio of concentration in both bile and serum to the minimum inhibitory concentration for the expected micro-organism. Antimicrobial therapy is usually empirical. Initial therapy should …

Shoeshine maneuver for cystic duct dissection: a simple technique to make Calot-triangle dissection smooth.

PURPOSE: Laparoscopic cholecystectomy, introduced in 1985 by Prof. Dr. Erich Mühe, has become the gold standard for treating chronic symptomatic calculous cholecystopathy and acute cholecystitis, with an estimated 750,000 procedures performed annually in the United States of America. The risk of iatrogenic bile duct injury persists, ranging from 0.2 to 1.3%. Risk factors include male gender, obesity, acute cholecystitis, previous hepatobiliary surgery, and anatomical variations in Calot's triangle. Strategies to mitigate bile duct injury include the Critical View of Safety and fundus-first dis…

Should prophylactic cholecystectomy be performed in patients with concomitant gallstones after endoscopic sphincterotomy for bile duct stones?

BACKGROUND: Endoscopic sphincterotomy (EST) is the standard treatment for biliary duct stones. However, performing additional cholecystectom … ever, additional biliary events of cholangitis were similar in the two groups, except for a case of cholecystitis (Log-rank, p = 0.998). CONCLUSIONS: Cholecystectomy after EST for biliary duct stones fails to reduce additional recurrent cholangitis but reduces additional cholecystitis.
