Conventional parotid surgery has historically relied on indirect anatomical landmarks, which often fail when faced with pathological distortions. This narrative review evaluates the clinical utility of advanced magnetic resonance neurography and 3D modeling for preoperative facial nerve assessment, aiming to bridge the gap between radiological advancements and practical surgical planning. A comprehensive literature search was conducted to evaluate mature clinical tools and experimental techniques. We assessed high-resolution MRI sequences, quantitative diffusion metrics, and radiomic analysis,

BACKGROUND: Mild cognitive impairment (MCI) is a transitional stage between subjective cognitive decline and Alzheimer's dementia, representing a critical window for intervention. We characterize the small-world properties of brain networks in MCI to identify sensitive biomarkers for early detection and assessment.
METHODS: Thirty-one patients diagnosed with MCI were recruited as the experimental group, while 30 healthy elderly individuals served as the normal control (NC) group. Based on diffusion tensor imaging (DTI) and resting-state functional magnetic resonance imaging (rs-fMRI), small-w

Otogenic brain abscess is a rare but life-threatening complication of chronic suppurative otitis media. It usually develops by direct spread through bony erosion of the tegmen tympani or Trautmann triangle, or by thrombophlebitis of the sigmoid sinus. Spread along the anatomical course of the seventh and eighth cranial nerves has been described only once before. We report a 50-year-old woman with moderate intellectual disability living in residential care with long-standing hearing impairment and intermittent ear discharge, who developed sudden ataxia and right peripheral facial palsy 4 days b

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease primarily affecting motor neurons, yet with substantial clinical variability. Furthermore, beyond motor symptoms, ALS patients also show non-motor features, reflecting its classification as a multi-system disorder. The identification of reliable biomarkers is a critical challenge for improving diagnosis, tracking disease progression, and predicting patient outcomes. This review explores macro- and microstructural alterations in ALS, focusing on gray matter (GM) and white matter (WM) as observed through Magnetic Reso

AIM: To define the natural history of diffusion tensor imaging (DTI) in Chediak-Higashi Disease (CHD) participants in relation to normative development.
METHODS: Twenty-five DTI scans from 15 CHD participants were compared with 100 DTI scans from 100 neurotypical controls (NC). Comparisons were evaluated for DTI metrics including fractional anisotropy (FA), and mean, axial, and radial diffusivity (MD, AD, RD, respectively). Correlational tractography was also performed to identify group differences between CHD and NC.
RESULTS: Pediatric CHD participants had DTI metrics similar to NC, however

Prostate cancer (PCa) is the most common malignancy in men. Precise grading is crucial for the effective treatment approaches of PCa. Machine learning (ML) applied to biparametric Magnetic Resonance Imaging (bpMRI) radiomics holds promise for improving PCa diagnosis and prognosis. This study investigated the efficiency of seven ML models to diagnose the different PCa grades, changing the input variables. Our studied sample comprised 214 men who underwent bpMRI in different imaging centers. Seven ML algorithms were compared using radiomic features extracted from T2-weighted (T2W) and diffusion-

PURPOSE OF REVIEW: To evaluate the available studies on structural magnetic resonance imaging (MRI) of post-traumatic headache (PTH).
RECENT FINDINGS: A systematic search of PubMed and Embase databases (from inception to February 1, 2024) identified nine relevant structural MRI studies. These studies included adult participants diagnosed with acute or persistent PTH in adherence with any edition of the International Classification of Headache Disorders. Eight studies focused on PTH attributed to mild traumatic brain injury (TBI), while one examined PTH after whiplash injury. The comparison gr

Generalized Anxiety Disorder (GAD) is characterized by excessive worry and physical symptoms of prolonged anxiety. Patients with subclinical GAD-states (sub-GAD) do not fulfill the diagnostic criteria of GAD, but they often show a disease burden similar to GAD, and the subclinical state may turn into a full syndrome. Neuroinflammation may contribute to changes in brain structures in sub-GAD, but direct evidence remains lacking. We investigated 73 newly recruited sub-GAD patients who had never received pharmacological or psychological treatment and 64 matched non-clinical individuals. We utiliz

BACKGROUND: Carotid endarterectomy (CEA) and carotid artery stenting (CAS) are established treatments for carotid artery stenosis but may cause asymptomatic cerebral infarctions detectable by diffusion-weighted imaging (DWI). This study evaluated the incidence and risk factors for postoperative DWI-positive lesions following CEA and CAS at our institution.
METHODS: We retrospectively analyzed 199 patients who underwent CEA (n = 114) or CAS (n = 85) between January 2019 and December 2023. Preoperative black-blood magnetic resonance imaging (MRI) was performed to determine the plaque-to-muscle

We aimed to determine whether transient global amnesia (TGA) is associated with alterations in central nervous system (CNS) injury biomarkers-serum neurofilament light chain (sNfL) and serum glial fibrillary acidic protein (sGFAP). In a prospective cohort of TGA patients, blood samples were obtained within 24-48 h of TGA onset (t0) and 6 weeks thereafter (t1). We assessed sNfL and sGFAP levels using the highly sensitive single-molecule array assay and calculated Z-scores adjusted for age, gender, and body mass index (BMI). Demographics, electroencephalography (EEG), and cerebral magnetic reson

The diagnosis of Creutzfeldt-Jakob disease (CJD) is particularly challenging because its heterogeneous clinical presentations mimic other rapidly progressive dementias and neurodegenerative disorders (e.g., Hashimoto's encephalopathy, autoimmune encephalitis, atypical Alzheimer's disease). Diffusion-weighted imaging (DWI) is the most sensitive neuroimaging sequence for diagnosing CJD. However, early magnetic resonance imaging (MRI) findings may be subtle or evolving, and autoimmune etiologies often remain in the differential. Therefore, empiric corticosteroids are reserved for cases in which a

Autism spectrum disorder is a common neurodevelopmental condition that manifests as a disruption in sensory and social skills. Although it has been shown that the brain morphology of individuals with autism is asymmetric, how this differentially affects the structural connectome organization of each hemisphere remains under-investigated. We studied whole-brain structural connectivity-based brain asymmetry in individuals with autism using diffusion magnetic resonance imaging obtained from the Autism Brain Imaging Data Exchange initiative. By leveraging dimensionality reduction techniques, we co…

BACKGROUND: The hypothalamus is a key brain structure involved in the pathogenesis of depression, and its abnormal activity is considered an important pathological mechanism for the formation of depression. The presence of abnormalities in the white matter integrity of hypothalamic subregions in mild cognitive impairment with depressive symptoms (D-MCI) remains unknown.
METHODS: In this study, we used diffusion tensor imaging (DTI) to explore the white matter integrity of hypothalamic subregions in D-MCI. On a 3 T magnetic resonance imaging scanner, we collected DTI data from 63 subjects. The

BACKGROUND: Suprasellar hypothalamic-opticochiasmatic glioma (HOCG) and craniopharyngioma (CP) have similar appearances on conventional magnetic resonance imaging (MRI) and are difficult to distinguish. Moreover, these tumors are situated near vital structures like the optic chiasm and hypothalamus, rendering conventional surgery susceptible to significant complications. We mainly discussed the surgical application value and diagnostic value of diffusion tensor imaging (DTI) in HOCG and CP.
METHODS: The retrospective analysis of 13 cases of HOCG and 16 cases of CP was conducted. All patients

A growing body of evidence suggests that in adults, there is a spatially consistent "inferior temporal numeral area" (ITNA) in the occipitotemporal cortex that appears to preferentially process Arabic digits relative to non-numerical symbols and objects. However, very little is known about why the ITNA is spatially segregated from regions that process other orthographic stimuli such as letters, and why it is spatially consistent across individuals. In the present study, we used diffusion-weighted imaging and functional magnetic resonance imaging to contrast structural and functional connectivi

Cerebral energy deficiency is increasingly recognised as an important feature of multiple sclerosis (MS). Until now, we have lacked non-invasive imaging methods to quantify energy utilisation and mitochondrial function in the human brain. Here, we used novel dual-calibrated functional magnetic resonance imaging (dc-fMRI) to map grey-matter (GM) deoxy-haemoglobin sensitive cerebral blood volume (CBVdHb), cerebral blood flow (CBF), oxygen extraction fraction (OEF), and cerebral metabolic rate of oxygen consumption (CMRO2) in patients with MS (PwMS) and age/sex matched controls. Epidermoid tumors are slow-growing, benign, congenital lesions.1 They commonly arise in the cerebellopontine angle, fourth ventricle, suprasellar region, or spinal cord.2 Symptoms may include hearing loss, facial pain, and headaches. The management options include observation or surgical resection. If the patient has symptoms, surgical resection is the treatment option of choice with the goal of gross total resection. In Video 1, we discuss the microsurgical technique for the resection of a right cerebellopontine angle epidermoid tumor. A 22-year-old male patient presented with chronic headach
