Necrotizing streptococcal myositis of an extremity: a rare case report.

Necrotizing myositis is a severe and very rare streptococcal soft tissue infection involving the superficial fascia and muscle. Its clinical symptoms are nonspecific until the appearance of a fulminant clinical course with soft tissue destruction and septic shock. A high mortality and morbidity rate has been reported in the few cases over the last century. Despite several attempts to better define the different entities causing this necrotizing soft tissue infection, no clear treatment has been outlined. We present the case of a 47-year-old woman who had an acute necrotizing myositis after a s

The management of cubitus varus and valgus using the Ilizarov method.

Cubitus varus and valgus are the most common complications of supracondylar and lateral condylar fractures. Various combinations of osteotomy and fixation have been described to correct these deformities but each is associated with significant complications. In this study, we used distraction osteogenesis and Ilizarov frame fixation to treat 24 elbows in 23 patients with cubitus varus or valgus. Their clinical outcome was evaluated using the protocol of Bellemore et al. The mean time to follow-up was 18.3 months (10 to 36) and the mean time to frame removal was 13.5 weeks (8 to 20). The mean c

Nonoperative treatment for minimally and nondisplaced lateral humeral condyle fractures in children.

The radiographic and clinical records of 95 children with nondisplaced or minimally displaced fractures of the lateral humeral condyle treated on a nonoperative protocol were reviewed. Acute nondisplaced or minimally displaced fractures were defined as < 24 h old on initial evaluation and displaced < 2 mm in three radiographic planes (anteroposterior, lateral, and internal oblique). Closed treatment and close follow-up in a long-arm cast or splint resulted in a union rate of 98% in 3-7 weeks. Two of the fractures displaced required open reduction and internal fixation and then subsequently wen

Alteration of Baumann's angle by humeral position: implications for treatment of supracondylar humerus fractures.

To determine the effects of limb positioning on Baumann's angle, we made a controlled radiographic study of a 6-year-old cadaver upper extremity specimen. Radiographic measurement of Baumann's angle was obtained at 10 degrees increments of humeral rotation from 40 degrees of internal rotation to 40 degrees of external rotation. These measurements were obtained with the humerus parallel to the x-ray cassette and with the humerus flexed 30 degrees from the cassette. Measurement of Baumann's angle with radiographs obtained with the humerus parallel to the x-ray cassette was associated with less v

Displaced fractures of the apophysis of the olecranon in children who have osteogenesis imperfecta.

Seven fractures of the apophysis of the olecranon in five children who had mild osteogenesis imperfecta were treated with open reduction and internal fixation followed by immobilization in a cast. All of the fractures healed uneventfully. The patients were examined after an average duration of follow-up of thirty months (range, twelve to sixty months); at that time, no patient reported pain or limitation of function. In the four elbows that had been followed for three years or more, there had been no observable disturbance of growth.

[Possibilities for treating chronic radius head dislocations].

Thirteen patients with neglected luxation of the proximal radius who were treated with different surgical procedures at the Orthopaedic Department of the Kantonsspital in St. Gallen (Switzerland) returned for evaluation at an average of 7 years postoperatively. The luxation was traumatic in 11 cases (7 Monteggia-fractures) and in 2 cases due to a dysplasia of the forearm. In 7 patients the deformity recurred and in 1 a synostosis between radius and ulna was noticed. In 4 cases the wrist showed a relative overlength of the ulna. Proximal osteotomy of the ulna in combination with open reduction

Localized chondrocalcinosis in traumatized joints.

The radiographs and medical records were reviewed of 76 patients who presented with chondrocalcinosis over a period of two years. Two groups of patients could be distinguished. The first group consisted of 58 mostly elderly patients (age 53-89 years [mean 71.8]). In these, chondrocalcinosis was most likely to be caused by an underlying disease such as calcium crystal deposition disease or it was present in association with osteoarthritis. In most cases of this group the findings were bilateral. The second group consisted of 18 patients who were significantly younger (age 15-69 years [mean 43.1

Applying the munster cast: optimal motion control for scaphoid fractures.

The scaphoid is the most frequently fractured carpal bone, with fractures of the middle third accounting for 80% of all scaphoid fractures (1-3). These fractures are commonly treated with a thumb spica, long arm cast that immobilizes both the wrist and the elbow. This type of cast is cumbersome and creates economic hardship for some patients.

Haemophilic synovitis: is rifampicin an alternative?

Rifampicin synoviorthesis has been empirically used for the treatment of haemophilic synovitis for some time. This paper reports on the experience of three Latin American centers with this treatment and compares it with radioactive synoviorthesis results. Chemical synoviorthesis with rifampicin is best indicated in younger patients (< 15 years) and small joint (ankles and elbows).

Luxatio erecta.

A case of luxatio erecta associated with a fracture of the greater tuberosity and a mixed nerve lesion is presented. The arm is abducted with the elbow flexed and the hand above the head, and there are creases on the shoulder. Reduction may be accomplished by upward traction on the extended arm with countertraction on the top of the shoulder and carrying the arm through an arc to the side of the body.

Clinical effects of Sizi Powder external application combined with TDP therapy for upper limb lymphedema after breast cancer surgery: A retrospective cohort study.

Upper limb lymphedema is a common complication after breast cancer surgery and may substantially impair upper limb function and quality of life. In clinical rehabilitation practice, external application of Sizi Powder combined with TDP therapy has been used as an adjunctive intervention; however, the available clinical evidence remains limited. This study aimed to evaluate the efficacy and safety of Sizi Powder external application combined with TDP therapy in patients with upper limb lymphedema after breast cancer surgery. This single-center retrospective cohort study included 86 patients wit

Fragment Incarceration as a Key Determinant of Functional Outcomes After Operative Fixation of Pediatric Medial Epicondyle Fractures: A Single-center Retrospective Cohort Study.

BACKGROUND: Medial epicondyle fractures account for a substantial proportion of pediatric elbow injuries, but the determinants of functional outcome after operative fixation remain incompletely defined. This study sought to describe functional and radiographic outcomes after open reduction and internal fixation (ORIF) of pediatric medial epicondyle fractures and to identify clinical, injury-related, and treatment-related parameters associated with these outcomes. METHODS: We retrospectively reviewed consecutive patients 16 years or younger who underwent ORIF for isolated medial epicondyle fra

Dual 500-μs wide pulse neuromuscular electrical stimulation enhancing sensorimotor cortical excitability.

BACKGROUND: Neuromuscular electrical stimulation (NMES) is an effective tool to improve motor activation of patients with motor dysfunction. However, to enhance the cortical activities induced by NMES, the corresponding strategies should be carefully designed with optimal stimulation parameters. The aim of the present study is to investigate whether the pulse assignment with wide-pulse-based Variable Frequency Trains improves sensorimotor cortical excitability. METHODS: A block-designed experiment was conducted with NMES delivering current to right biceps brachii muscle in nine healthy right-
