[Spontaneous electromyographic activity. Practical importance].

Spontaneous activities are a major semeiologic sign in electromyography. The present article deals with the different aspects recorded in practice in normal and pathological cases. There are two types of spontaneous activities, those related to motor unit hyperactivity (fasciculations and myokymia) and those related to the hyperactivity of one or more muscle fibers: fibrillations, positive sharp waves, myotonic discharges and complex repetitive discharges. In the first case the lesion is located in the axone and in the second in the membrane of the muscle fibers; All theories related to the ce…

Pathophysiological significance of fasciculations in the early diagnosis of ALS.

In amyotrophic lateral sclerosis (ALS), muscles with preserved strength can have fasciculation potentials (FPs) as the only abnormality. The FPs in strong muscles are predominantly simple and stable, and can often be recruited by a slight voluntary contraction. In weak and atrophic muscles, the FPs are generally complex and unstable, tend to have a slower firing rate and are not recruited by voluntary contraction. Macro-electromyography studies suggest that these FPs are part of a more complex motor unit, as opposed to simple FPs which can represent all the motor unit. FPs driven by transcrani…

Increased threshold sural amplitude after upper limb isometric contraction in complete paraplegics.

OBJECTIVE: To determine whether the enhancement of threshold sural sensory nerve action potentials (SNAPs) occurred in patients with spinal cord injury after upper limb isometric contraction. DESIGN: This prospective study, in which ten paraplegic patients with spinal cord injury were recruited from the Harris County community and served as his/her own control, was performed in the electromyography laboratory at Harris County Hospital District Quentin Mease Hospital. The baseline SNAP was established using ten threshold, signal-averaged stimuli to the sural nerve. With the same stimulus stren…

Alternative treatment of the carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) is a painful disease of the wrist which is usually treated with corticoids locally or surgically. In this study combined infusion therapy with procain, pentoxyphyllin and magnesium sulphuricum in patients with CTS was evaluated retrospectively. 101 manually working patients (65 male, 36 female) aged 42.8 +/- 8.6 years, initially examined at our department to assess professional origin of the disease and who had abnormal conduction to the distal section of median nerve on electromyography (EMG), were entrolled. The group of patients was heterogenous etiologically. 4…

[The clinical features of cervical spondylosis with boule musculaire].

To clarify the pathophysiology of boule musculaire associated with cervical spondylosis, we investigated the 13 patients (11 males and 2 fem … cases of cervical myelopathy. MRI obtained from 3 patients demonstrated a compressed cervical cord. Electromyography obtained from 7 patients demonstrated mild neurogenic changes such as polyphasic or prolonged motor unit potential (MUP) in the boule musculaire and in the atrophic regions. However, there was no giant MUP, fasciculation, fibrillation, nor positive …

Cable grafting of the spinal accessory nerve after radical neck dissection.

BACKGROUND: From January 1981 through March 1996, 20 patients with head and neck cancer underwent radical neck dissection with sacrifice of the spinal accessory nerve and immediate reconstruction of the nerve using a microsurgical technique and a cable graft of the great auricular nerve. METHODS: Postoperative shoulder function was assessed via a subjective questionnaire, objective strength testing, and/or postoperative electromyography. The latter was used to evaluate for the presence and amplitude of voluntary motor potentials, the presence of fibrillation potentials, and nerve conduction l…

Antroduodenal manometry findings in patients with slow-transit constipation.

BACKGROUND: The results of subtotal colectomy for slow-transit constipation are unpredictable. Abdominal pain, distension, and bloating often persist after operation. To ascertain whether patients with slow-transit constipation may have a generalized intestinal motor disorder, we studied the antroduodenal motor activity in 20 consecutive patients with slow-transit constipation. METHODS: All patients underwent symptom registration, whole-gut transit time, anorectal manometry, electromyography of the anal sphincter, the balloon expulsion test, and defecography to characterize their constipation…

Prognostic predictors of acute transverse myelitis.

INTRODUCTION: The role of clinical, MRI and neurophysiological parameters in predicting the outcome of acute transverse myelitis (ATM) is reported. MATERIALS AND METHODS: Thirty-one patients with ATM were subjected to clinical, MRI, somatosensory and motor evoked potential studies in both upper and lower limbs and concentric needle electromyography. The outcome was defined at the end of 6 months into poor (Barthel Index score <12) and good (> or =12). The relationship of various prognostic variables was evaluated by biserial correlation coefficient and stepwise discriminant analysis. RESULTS…

[Sensorimotor polyneuropathy and systemic amyloidosis as paraneoplastic symptoms of a carcinoid-like well differentiated carcinoma of the breast].

HISTORY AND ADMISSION FINDINGS: A 75-year-old woman was admitted because of weakness and paraesthesias in both legs. 15 years earlier a chest radiogram had shown numerous round foci of uncertain cause. Physical examination confirmed muscular weakness in all limbs, especially the legs, as well as abnormal superficial and deep sensory perception. INVESTIGATIONS: Electromyography registered a patchy pattern of intentional muscular activity and marked denervation activity but largely normal nerve conduction. There was no evidence of inflammatory disease in the laboratory tests, including the CNS.…

[Diaphragm kinetics coupled with spirometry. M-mode ultrasonographic and fluoroscopic study; preliminary results].

Most techniques used so far for the evaluation of diaphragm kinetics are either invasive (electromyography, fluoroscopy), or indirect (respiratory pressures, impedance plethysmography). The aim of this study was to determine whether assessment with ultrasound or fluoroscopy differed, and which technique appeared more suitable in the investigation of quantitative hemidiaphragmatic displacement. Six patients (3 female, 3 male, aged 29 to 40) without respiratory disease were studied during systematic X-Ray chest examination, spirometry, and abdominal sonography. The amplitude of the right diaphra…

Chloroquine neuromyopathy.

A 59-year-old woman with rheumatoid arthritis was treated with prednisone and 250 mg of chloroquine diphosphate (CQ) daily. Though there was improvement in her joint symptoms, she began to notice progressive lower limb weakness, later extending to the arms and lasting for 2 months. Electromyography showed fibrillations, polyphasic potentials and high frequency discharges. Biceps brachii biopsy showed that virtually every muscle fiber and multiple small vacuoles surrounded by a basophilic rim. There was variation in fiber diameter and some fibers were atrophic and angulated. ATPase revealed typ…

The respiratory system.

Neurological disorders frequently contribute to respiratory failure in critically ill patients. They may be the primary reason for the initi … ion of central and peripheral nervous system disorders. Phrenic nerve conduction studies and needle electromyography of the diaphragm and chest wall muscles are useful to characterize neuropathies and myopathies affecting the diaphragm. Repetitive phrenic nerve stimulation can assess neuromuscular transmission defects. It is important to identify patients at risk …

Trauma to the cricoarytenoid joint represents a rare but serious complication of endotracheal intubation. Subluxation and luxation of the ar … ologists is mandatory. In addition to indirect and direct laryngoscopy, computerised tomography and electromyography of the larynx play an important role in differentiating arytenoid dislocation from true vocal cord paralysis due to nerve damage. Early operative reposition results in fair prognosis, whereas delayed diagnosis may lead to ankylosis of the cricoaryte …
