Conjunctival swabs and corneoscleral rim cultures from corneal transplantation donors as possible early indicators for posttransplant endopthalmitis.

PURPOSE: To determine by microbiological examinations the rate of conjunctiva and corneoscleral rim contamination of cadaver donor tissues. METHODS: A prospective review of culture results for 98 cadaver donor conjunctival swabs [mean age 76.4 ± 12.9 years (±SD)]. Transplant patients were evaluated both before and after disinfection with gentamicin. Individual parameters evaluated included detection rates of bacteria, variety of detected bacteria, time interval from donor death to tissue harvest and age of donors. RESULTS: Before disinfection, 60 out of 98 conjunctival swabs exhibited microb

Treatment of fungal corneal ulcers with amphotericin B ointment.

PURPOSE: To report two patients with severe fungal corneal ulcers who were treated successfully with topical amphotericin B ointment. METHODS: Two eyes of two patients developed corneal ulcers and hypopyon after corneal foreign body removal. Aspergillus fumigatus and Fusarium solani were isolated in Patients 1 and 2, respectively. By antifungal susceptibility testing, amphotericin B was shown to have the lowest minimal inhibitory concentration. RESULT: Topical administration of amphotericin B ointment resulted in dramatic improvement in fungal corneal ulcers. CONCLUSIONS: Antifungal suscept

Fetal varicella - diagnosis, management, and outcome.

Fetal varicella syndrome (FVS) is due to transplacental infection by the Varicella zoster virus following maternal infection. The risks for the fetus and neonate depend on the timing. When varicella occurs around delivery, it often leads to disseminated neonatal varicella. When varicella occurs during pregnancy, transmission can occur, but is usually asymptomatic; some infants develop zoster postnatally and a few have FVS. Before 20 weeks' gestation, FVS can occur, with an incidence of about 1%. The lesions can affect the skin, limbs, central and autonomous nervous systems, eyes, cause calcifi

[Migrating swellings from Asia: gnathostomiasis].

Two patients suffered from intermittent subcutaneous swellings of the face. A 41-year-old man recalled a recent episode with severe thoracic pain and at that time pleurisy was documented. In this patient there was pronounced eosinophilia. The other was a 32-year-old woman. Both patients had traveled to southeast Asia. Antibodies against Gnathostoma spinigerum were detectable in both patients. The first patient was treated with albendazole 200 b.i.d. for three weeks, but because of recurrent facial swelling, he was treated again with albendazole at a higher dose: 400 b.i.d. for eight weeks, whi

A murine model of herpes simplex virus-induced ocular disease for antiviral drug testing.

Previous studies to determine the efficacy of new antiviral compounds in treating HSV ocular infections have used rabbit models. However, rabbits are expensive to purchase and maintain, and require the use of substantial amounts of test compounds. We have used the currently licensed antiviral trifluorothymidine in a murine model of HSV-induced ocular infection to demonstrate that the less expensive murine model can be used for the in vivo evaluation of potentially useful antiviral compounds. Treatment with TFT reduced the severity of blepharitis, vascularization of the cornea, stromal keratiti

Delayed infectious keratitis in a patient with radial keratotomy and floppy eyelid syndrome treated with simple interrupted stitches.

A 55-year-old male presented with an infectious keratitis in his left eye, affecting one of the incisions of the radial keratotomy he had undergone thirty-four years ago. Suturing the incision with two simple interrupted stitches was key to the resolution of the infection. Floppy eyelid syndrome was also found in this patient. Could this act as a risk factor for infectious keratitis in radial keratotomy? To our knowledge, this is the first reported case describing the association between both conditions, and the second reported case where sutures have been used as an adjuvant treatment in thes

Human Thelaziasis: Emerging Ocular Pathogen in Nepal.

Thelaziasis is an ocular arthropod-borne, zoonotic disease of the eye infecting the conjunctival sac, lacrimal duct, and lacrimal gland caused by a nematode of the genus Thelazia. We report the first case of human ocular thelaziasis in Nepal in a 6-month-old child from a Rukum district, Nepal. The infant presented with conjunctivitis, and his visual acuity and dilated fundal examination were normal. A total of 6 worms were removed for identification. Collected nematodes were identified based on morphological keys as Thelazia callipaeda. The patient's symptoms improved after removal of the nema

Trachoma control: the SAFE strategy.

As a contagious bacterial infection that affects the conjunctival covering of the eye, the cornea and the eyelids, trachoma is controlled by an endorsed integrated strategy consisting of surgery for trichiasis, antibiotic therapy, facial cleanliness and environmental improvement, namely, the SAFE strategy developed by World Health Organization. Developed based on evidence from previous field trials and constantly modified in practice, SAFE strategy has greatly boosted the progress in trachoma control. Regardless of the fact that there are still many pending questions, national program coordina

Pigmented free floating vitreous cyst in a 10 years old child.

Pigmented free-floating vitreous cyst in retrolental space is rare. It can represent its congenital origin after spontaneous detachment from the ciliary body epithelium or after trivial injury. We report a case of pigmented, free floating non-infective vitreous cyst in a 10 year old child who presented with compliant of transient blurring of vision three days after she joined swimming class. A thorough eye examination with ultrasound B scan and ultrasound biomicroscopy was done to rule out any other associated conditions.

A Case of Keratomycosis Caused by Fusarium Solani at Rural Tertiary Care Center.

Keratomycosis is defined as invasive infection of corneal stroma caused by variety of fungi. Most of these cases occur in agricultural workers with history of corneal trauma with vegetative matter contaminated by fungi. Fusarium species which causes keratomycosis receives occasional mention in literature. We report a case of a 27-year-old male, farmer by occupation presented with complains of foreign body sensation, itching and discharge from the right eye and developed white patch on cornea which was progressed gradually to size of diameter of pea which was later diagnosed as keratitis caused

Zika Virus: The Agent and Its Biology, With Relevance to Pathology.

Once obscure, Zika virus (ZIKV) has attracted significant medical and scientific attention in the past year because of large outbreaks associated with the recent introduction of this virus into the Western hemisphere. In particular, the occurrence of severe congenital infections and cases of Guillain-Barré syndrome has placed this virus squarely in the eyes of clinical and anatomic pathologists. This review article provides a basic introduction to ZIKV, its genetics, its structural characteristics, and its biology. A multidisciplinary effort will be essential to establish clinicopathologic cor

Preventing blindness and saving lives: the centenary of vitamin A.

Within 20 years of its discovery 100 years ago, vitamin A was recognized as critical to normal eyes, growth, and survival. Clinical interest subsequently contracted to its importance in preventing xerophthalmia, until this ophthalmologist stumbled, quite accidently, on its role in fighting life-threatening infections. Repeated, large-scale randomized clinical trials eventually convinced (and reminded) the pediatric and nutrition communities of its importance for child survival. Vitamin A distribution programs are now credited with saving the sight and lives of nearly half a million children ev

Orbital cellulitis as a complication of mandibular odontogenic infection.

A 30-year-old man presented with an acute left eye proptosis, pain, and decreased vision. He had been having a dental infection since 3 days. Orbital CT scan showed abscess in the left orbit. On dental X-ray a periapical radiolucency of the mandibular left-second molar was observed, and no obvious sinus involvement was noted. Intravenous antibiotic therapy did not lead to any significant improvement. Therefore, the patient underwent combined dental and orbital surgery to drain the abscess. Following the surgery, the patient's clinical signs and symptoms resolved completely.

Cefixime-induced oculogyric crisis.

Oculogyric crisis is a neurologic adverse event characterized by bilateral dystonic, usually upward, conjugate eye deviations. Cefixime is a third-generation cephalosporin and is widely used in clinical practice in childhood. Confusion, encephalopathy, coma, myoclonus, nonconvulsive status epilepticus, and seizures have been described with the use of cephalosporins. We presented a cefixime-induced oculogyric crisis in a 7-year-old boy during the treatment of urinary tract infection, and this is the first case of cefixime-induced oculogyric crisis whose ocular symptoms gradually disappeared wit

Racemose cysticercosis presenting as cystic neck swelling.

Cysticercosis is a parasitic infection caused by the larvae of the tapeworm Taenia solium which occurs in neural and extraneural forms. Latter commonly involves subcutaneous tissue, skeletal muscles, and eyes. Head and neck sites commonly involved are buccal mucosa, tongue, and lips. It presents as single or multiple submucosal/cutaneous firm nodules. We report a case of cysticercosis of the supraclavicular region of the neck where it presented as a large soft cystic swelling. The diagnosis was made by sonography and confirmed by gross and microscopic examination to be racemose cysticercosis.

Neuro and intraocular cysticercosis: A clinicopathological case report.

Cysticercosis is one of the most common parasitic infestations in humans. Neurocysticercosis (NCC) is the commonest parasitosis of the central nervous system (CNS), endemic in developing countries and is also seen in developed nations with high rates of immigration from prevalent areas. Co-infection of ocular tissue is also significant and often ophthalmologists come across these lesions in their clinical practice. The mode of treatment in NCC and that of intraocular cysticercosis is somewhat different as far as antiparasitic treatment is concerned. This case report highlights the proper manag

Bacterial keratitis early after corneal crosslinking with riboflavin and ultraviolet-A.

We report a case of bacterial keratitis 3 days after corneal crosslinking for keratoconus. The patient complained of increasing pain and redness combined with blurred vision in the treated eye starting on the first postoperative day. Clinical examination showed multiple stromal infiltrations and moderate anterior chamber inflammation. Corneal scraping revealed an Escherichia coli infection, which was successfully treated with fortified tobramycin and cephazolin eyedrops for several weeks. This is the first report of a case of rare postoperative complication resulted in an avascularized corneal

Endophthalmitis due to inadvertent globe penetration during retrobulbar injection of saline solution for laser in situ keratomileusis.

A 31-year-old woman presented with visual acuity of counting fingers and presumed bacterial endophthalmitis in the left eye 10 days after refractive surgery. During the procedure, a retrobulbar injection of balanced salt solution had been performed to assist with globe suction by the microkeratome. A perforation site was identified in the inferonasal retina. Following intravitreal antibiotic injection and surgical intervention, the visual acuity returned to 20/20. Retrobulbar injection to facilitate laser in situ keratomileusis carries risks. Careful monitoring for signs of infection is recomm

Unilateral restrictive ophthalmoplegia and enophthalmos associated with an intraorbital tissue band.

Fibrosis of the extraocular muscles can be congenital or acquired. Acquired causes include trauma, myositis, thyroid eye disease, infection, and metastases. Congenital fibrosis of the extraocular muscles (CFEOM) runs in families and is known to have a genetic basis. It has been classified by Brown, Hansen, and Harley et al into the five following types: general fibrosis syndrome; fibrosis of the inferior rectus (IR) with blepharoptosis; strabismus fixus; vertical retraction syndrome; and unilateral fibrosis, blepharoptosis, and enophthalmos syndrome. In this report, a case of unilateral fibros
