Biliary tract disease in Thailand.

One hundred and eighty-nine patients were documented as having biliary tract disease. Two hundred and nine lesions were found. The prevalence of malignant disease of the biliary tract was represented by 30.1 per cent, and cholangiocarcinoma was a leading cause. The carcinogenesis of cholangiocarcinoma has been speculated. Acute cholecystitis represented 55 per cent of biliary tract disease in Thailand, which consisted of acute calculous cholecystitis and acute noncalculous cholecystitis, 94.8 and 5.2 per cent, respectively. Among acute calculous cholecystitis, the prevalence of cholelithiasis,

Diagnostic utility of cholescintigraphy and ultrasonography in acute cholecystitis.

When faced with a patient with possible acute cholecystitis, technetium-99m-HIDA scintigraphy should be the primary diagnostic procedure performed. If scintigraphy reveals a normal gallbladder, acute cholecystitis is excluded. If the scintigram fails to visualize the gallbladder, ultrasonography is deemed advisable to exclude potential false-positive scintigrams and confirm the presence of cholelithiasis.

Evaluation of the chemopreventive potentials of ezetimibe and aspirin in a novel mouse model of gallbladder preneoplasia.

Gallbladder stones (cholecystolithiasis) are the main risk factor for gallbladder cancer (GBC), a lethal biliary malignancy with poor survival rates worldwide. Gallbladder stones are thought to damage the gallbladder epithelium and trigger chronic inflammation. Preneoplastic lesions that arise in such an inflammatory microenvironment can eventually develop into invasive carcinoma, through mechanisms that are not fully understood. Here, we developed a novel gallbladder preneoplasia mouse model through the administration of two lithogenic diets (a low- or a high-cholesterol diet) in wild-type C5

Hepatobiliary disease in cystic fibrosis: a survey of current issues and concepts.

The incidence of hepatobiliary complications of cystic fibrosis (CF) has been increasing in parallel with the rate of survival. Detection of hepatic involvement remains a problem, as liver function tests, serum bile acid determinations, and ultrasonography do not permit an early diagnosis. The pathogenesis of cholelithiasis has been elucidated in the past few years. However, the mechanism leading to the pathognomonic CF lesion, focal biliary cirrhosis, is still unknown. There are indications that mucus plugging may be the ancestral lesion and the triggering factor. The possibility that correct

Identification of PEG10 and TSG101 as carcinogenesis, progression, and poor-prognosis related biomarkers for gallbladder adenocarcinoma.

PEG10 is a transcriptional factor while TSG101 is involved in numerous cellular processes, including apoptotic resistance. Overexpression of PEG10 and TSG101 were observed in a variety of human cancers. However, their expression and clinical significance in gallbladder cancer (GBC) have not yet been identified. To understand the tumor biology of GBC at the molecular level, we examined PEG10 and TSG101 expression in 108 adenocarcinomas, 15 gallbladder polyps, 35 chronic cholecystitis tissues, and 46 peritumoral tissues by using immunohistochemistry. Overexpression of PEG10 and TSG101 was detect

Primarily Gasless Robot-Assisted Cholecystectomy in Dogs: A Cadaveric Feasibility Study.

Gallbladder disease is a common cause of morbidity in dogs, and cholecystectomy remains the definitive treatment in many cases. Although minimally invasive approaches offer recognized advantages, their adoption is limited by technical complexity and by the physiological effects of carbon dioxide (CO₂) pneumoperitoneum. This cadaveric study evaluated the feasibility of performing primarily gasless robot-assisted cholecystectomy under abdominal wall suspension in five canine cadavers. A normobaric operative field was established using a suspension device combined with the VersiusTM robotic platf

Survival benefit of adjuvant radiotherapy for gallbladder cancer patients with stage IIIA: a population based, propensity score-matched study.

BACKGROUND: The optimal adjuvant treatment for advanced gallbladder cancer (GBC) remains controversial. This study aimed to evaluate whether adjuvant radiotherapy (ART) after surgery confers a survival benefit in patients with advanced GBC, particularly focusing on stage IIIA disease. METHODS: We retrospectively analyzed data from the Surveillance, Epidemiology, and End Results (SEER) database (January 2010-December 2015). Patients were divided into ART and non-ART groups. Overall survival (OS) and cancer-specific survival (CSS) were evaluated using Cox proportional hazards models and Kaplan-

When Checkpoint Inhibition Goes Off Target: A Rare Case of Pembrolizumab-Induced Acalculous Cholecystitis.

Immune checkpoint inhibitors (ICIs) have revolutionized cancer therapy by improving survival across multiple malignancies by enhancing antitumor immune responses. However, increasing use of ICIs has been accompanied by a growing spectrum of immune-related adverse events (irAEs). Gastrointestinal irAEs most commonly include colitis and hepatitis, whereas biliary complications remain rare and poorly characterized. ICI-associated cholecystitis is an uncommon entity affecting patients treated with ICIs. It often presents as an acalculous disease and occurs months after therapy initiation. This rep

The Value of Routine Histopathology After Laparoscopic Cholecystectomy: A Single-Centre Retrospective Analysis.

Introduction Laparoscopic cholecystectomy is among the most common general surgical procedures globally. While many centers routinely send all gallbladder specimens for histopathological examination, the clinical and cost-effectiveness of this practice is debated. This study evaluates the utility of a routine approach compared to a selective one. Methods We conducted a retrospective observational study of 171 patients (n=171) who underwent elective, expedited, or urgent laparoscopic cholecystectomy at Warwick Hospital between 1 July 2023 and 30 January 2024. Data were extracted from the theatr

GLP-1 receptor agonists: trend, necessity, or blessing?

The recent endorsement of glucagon-like peptide-1 (GLP-1) receptor agonists, such as semaglutide (Wegovy), by the American College of Cardiology as first-line therapy for weight management marks a paradigm shift in cardiometabolic care. Semaglutide offers significant benefits, including improved glycemic control and substantial weight loss, with emerging data demonstrating its impact even in individuals without diabetes. However, GLP-1 receptor agonists' growing popularity raises important concerns regarding long-term safety, access equity, and health care priorities. Although common adverse e

Gut microbiota promotes cholesterol gallstone formation through the gut-metabolism-gene axis.

BACKGROUND: Gallstone disease, arising from the interplay between host metabolism and gut microbiota, represents a significant health concern. Dysbiosis of the gut microbiome and disruptions in circadian rhythm contribute to the pathogenesis of gallstones. This study conducted a comprehensive analysis of gut microbiota and metabolites derived from stool and serum samples of 28 patients with cholesterol gallstones (CGS) and 19 healthy controls, employing methodologies such as 16S rRNA sequencing, metaproteomics, metabolomics, and host genetic analysis. Additionally, a retrospective cohort study

The present and the future of immunotherapy in hepatocellular carcinoma and biliary tract cancers.

Hepatobiliary malignancies encompass a spectrum of invasive carcinomas arising in the liver [hepatocellular carcinoma (HCC), bile ducts [intrahepatic cholangiocarcinoma (ICC), and extrahepatic cholangiocarcinoma (EHC)] and the gallbladder. These malignancies represent a growing global health burden, with rising incidence and mortality rates and their overall prognosis remains poor because many patients present with advanced unresectable disease at diagnosis. In recent years, significant advancements in understanding HCC immunogenicity have reshaped the therapeutic scenario of advanced HCC with
