The gut microbiota and Type 1 Diabetes.

Type 1 Diabetes (T1D) is a multifactorial, immune-mediated disease, which is characterized by the progressive destruction of autologous insulin-producing beta cells in the pancreas. The risk of developing T1D is determined by genetic, epigenetic and environmental factors. In the past few decades there has been a continuous rise in the incidence of T1D, which cannot be explained by genetic factors alone. Changes in our lifestyle that include diet, hygiene, and antibiotic usage have already been suggested to be causal factors for this rising T1D incidence. Only recently have microbiota, which ar

Typhoid transmission: a historical perspective on mathematical model development.

Mathematical models of typhoid transmission were first developed nearly half a century ago. To facilitate a better understanding of the historical development of this field, we reviewed mathematical models of typhoid and summarized their structures and limitations. Eleven models, published in 1971 to 2014, were reviewed. While models of typhoid vaccination are well developed, we highlight the need to better incorporate water, sanitation and hygiene interventions into models of typhoid and other foodborne and waterborne diseases. Mathematical modeling is a powerful tool to test and compare diff

Temperomandibular joint ankylosis in children.

Temperomandibular joint (TMJ) ankylosis or hypo mobility involves fusion of the mandibular condyle to the base of the skull. Impairment of speech, difficulty in mastication, poor oral hygiene, rampant caries, and acute compromise of the airway pose a severe psychological burden on the tender minds of children. The treatment of TMJ ankylosis poses a significant challenge because of technical difficulties and a high incidence of recurrence. This report describes a case of 7-year-old with inability to open mouth, diagnosed with unilateral right bony TMJ ankylosis. The surgical approach consisted

Localized juvenile spongiotic gingival hyperplasia: a report of 3 cases.

Localized juvenile spongiotic gingival hyperplasia (LJSGH) is a recently described benign condition that affects the gingiva of children and young adults. Clinically distinctive, LJSGH presents as a localized area of erythema on the attached gingiva, with a subtly papillary surface architecture. The lesions are generally biopsied because of the lack of resolution with conservative oral hygiene therapeutic measures and esthetic concerns. The histopathology has a characteristic appearance of subtle papillary epithelial hyperplasia, acute inflammation, and numerous engorged capillary vascular spa

Changing a urethral or suprapubic catheter: the patient's perspective.

Understanding the patient's perspective in any area of health care is now recognised as being crucial to good practice, but little is known about patients' views on living with an indwelling urinary catheter. In this study, 36 people living with either a urethral or suprapubic catheter across the UK were interviewed. After thematic analysis, the authors interpreted what they learned. Many issues were raised, including the need to know who was responsible for changing the catheter, the importance of continuity of care, 24-hour access to services, and good hygiene. Patients valued nurses with tr

Skin microbiota: overview and role in the skin diseases acne vulgaris and rosacea.

As the first barrier to environmental exposures, human skin has developed an integrated immune system to protect the inner body from chemical, physical or microbial insults. Microorganisms inhabiting superficial skin layers are known as skin microbiota and include bacteria, viruses, archaea and fungi. The microbiota composition is crucial in the instruction and support of the skin's immune system. Changes in microbiota can be due to individual, environmental or behavioral factors, such as age, climate, hygiene or antibiotic consumption, which can cause dysbiosis. The contribution of skin micro

[Geriatric urology: special features of palliative long-term urinary bladder drainage in the context of the Geriatric I's].

In situations at the interface of urology and geriatric medicine the palliative management of geriatric patients with urinary incontinence of voiding dysfunction occupies a central position. Influencing factors that need to be considered by the surgically active urologist are not only the high age and the thus associated vulnerability of the geriatric patient but also other aspects such as the patient's wishes, the expectations of care-giving relatives and the sexual situation. In this light a strict and individually adapted indication for placing a palliative indwelling catheter is mandatory.

Ipsilateral idiopathic gingival enlargement and it's management using conventional gingivectomy and diode laser: A recurrent case after 15 years.

Idiopathic gingival fibromatosis is a relatively rare condition characterized by the proliferation of the gingival tissues resulting in masticatory, esthetics, phonetics and psychological disturbances. The severity of the overgrowth can range from a solitary isolated mass to a more generalized and diffused enlargement. The etiopathogenesis of this bizarre condition is poorly understood and has been attributed to various factors. It can present as a single disorder or may manifest as part of a syndrome. This case reports an ipsilateral diffused idiopathic gingival enlargement in a middle aged a

Diagnosis and molecular characterization of Trichomonas vaginalis in sex workers in the Philippines.

Trichomonas vaginalis is a pathogenic protozoon which causes the sexually transmitted infection, trichomoniasis. The absence or non-specificity of symptoms often leads to misdiagnosis of the infection. In this study, 969 samples consisting of vaginal swabs and urine were collected and screened from social hygiene clinics across the Philippines. Of the 969 samples, 216 were used for the comparative analysis of diagnostic tools such as wet mount microscopy, culture, and PCR utilizing universal trichomonad primers, TFR1/2 and species-specific primers, TVK3/7 and TV1/2. PCR demonstrated higher sen

Nonsurgical recovery of interdental papillae under supportive periodontal therapy.

We observed nonsurgical improvement of interdental papillae in a patient undergoing supportive periodontal therapy. The patient was a 47-year-old Japanese man presenting with widespread gingival recession at Daniele's papilla presence index level 3 and Miller Class I recession affecting the facial aspect of tooth number 42. Initial periodontal therapy for periodontitis was performed, included oral hygiene instruction, scaling and root planing, resulting in a reduction in inflammation. Use of an interdental brush was then suspended to allow the interdental papillae to recover. The type of tooth

Housing shortage and communal politics in European cities around 1900: the cases of Basel 1889 and Belgrade 1906.

In the second half of the nineteenth century, European cities faced a problem well known in postindependence India: the population escalated due to immigration from the rural areas causing rapid and considerable housing shortage. This forced large parts of the poorer classes into miserable living conditions. Lack of space, money and hygiene facilitated the epidemic spread of diseases such as tuberculosis and diarrhoea. The town authorities were called upon to stop speculation and to launch state financed housing projects. However, in reality the situation was very different depending on the pl

[Dentifrices:an evaluation based on the current literature].

Toothpastes remain a frequently used auxilary in oral hygiene. Although some new components have been introduced, the base composition does not differ much between toothpaste types. Concerning toothpastes there is a thin border between cosmetics and drugs and it becomes more and more difficult for oral health care workers to prescribe the right toothpaste for the right patient. Production companies are frequently using commercial advertisements to promote their product. In this way, it's getting more and more difficult for the dentist (and patient) to find the suitable product. This literature

Sleep Health Issues for Children with FASD: Clinical Considerations.

This article describes the combined clinical experience of a multidisciplinary group of professionals on the sleep disturbances of children with fetal alcohol spectrum disorders (FASD) focusing on sleep hygiene interventions. Such practical and comprehensive information is not available in the literature. Severe, persistent sleep difficulties are frequently associated with this condition but few health professionals are familiar with both FASD and sleep disorders. The sleep promotion techniques used for typical children are less suitable for children with FASD who need individually designed in

[Prevention of catheter-related bloodstream infections in the operation room].

Catheter-related bloodstream infections (CRBSIs) are recognized as an important and serious problem, especially in an intensive care unit (ICU), since they have far higher infection rates compared to those for other type of intravascular devices. However, in the operation room, there seems to be little concern among anesthesiologists regarding this problem. It is important for anesthesiologists to understand that CRBSIs can be prevented or reduced by evidence-based interventions such as hand hygiene, education in hand washing and alcohol-based hand rubbing, sterile catheter care techniques, pr

[Breastfeeding and infectious diseases: state of the art].

Despite the well-known nutritive, psychological, immunological and economical benefits of breast-feeding, some contraindications exist, such as some mother infectious diseases transmitted through the breastfeeding itself. The risk of transmitting an infectious agent through breast milk seems to be relatively low, except for some virus diseases (CMV HIV), for some invasive bacteria forms (Salmonella typhimurium and Brucella) and for the presence of abscesses and mastitis. In some mother infectious disease, a correct hygiene allows the continuation of breastfeeding without risks for the infant,

Dirty hands: bacteria of faecal origin on commuters' hands.

Although many studies have investigated bacteria on the hands of health-care workers and caregivers, few have looked at microbiological contamination on the hands of the general adult public. This study investigated faecal bacteria on the hands of commuters in five UK cities. Of the 404 people sampled 28% were found to have bacteria of faecal origin on their hands. A breakdown by city showed that the proportion of people with contaminated hands increased the further north the city of investigation (P<0.001), an effect which was due in large part to a significant trend in men but not in women.

Prevention and management of gastrointestinal infections in infants from a nutritional perspective.

This article considers infections of the gastrointestinal (GI) tract. This is a complex organ, which exists in a range of environments. Despite containing defence mechanisms against microorganisms, GI infections are common throughout infancy; however, the risk of infection can be reduced through careful hygiene and the encouragement of breast-feeding. Although research into the role of dietary factors in preventing or treating GI infection is in its early days, there is some evidence for the use of prebiotics and probiotics. The role of health care professionals is to give parents and carers a
