Acute and long-term management of food allergy: systematic review.

BACKGROUND: Allergic reactions to food can have serious consequences. This systematic review summarizes evidence about the immediate management of reactions and longer-term approaches to minimize adverse impacts. METHODS: Seven bibliographic databases were searched from their inception to September 30, 2012, for systematic reviews, randomized controlled trials, quasi-randomized controlled trials, controlled clinical trials, controlled before-and-after and interrupted time series studies. Experts were consulted for additional studies. There was no language or geographic restrictions. Two revie

Looking ahead in dermatology: skin and allergy.

The prevalence of allergic diseases has increased tremendously over the last decades. Clinically they comprise a spectrum of many different conditions caused by specific immunological hypersensitivity in response to a mostly apathogenic substance. Environmental factors such as lack of immune-stimulating contacts (infection, vaccination) and exposure to allergy-enhancing anthropogenic pollutants from tobacco smoke or traffic exhaust particles are suspected to be involved in the increase of allergies. Recently it has been shown that pollens are not only allergen carriers but also secrete highly …

Food allergy: recent advances in pathophysiology and treatment.

Food allergies, defined as an adverse immune response to food proteins, affect as many as 6% of young children and 3%-4% of adults in westernized countries, and their prevalence appears to be rising. In addition to well-recognized acute allergic reactions and anaphylaxis triggered by IgE antibody-mediated immune responses to food proteins, there is an increasing recognition of cell-mediated disorders such as eosinophilic gastroenteropathies and food protein-induced enterocolitis syndrome. We are gaining an increasing understanding of the pathophysiology of food allergic disorders and are begin

[Natural latex allergy. Patient management: from clinic to prevention. A review].

The widespread use of latex devices has been followed, in the last 25 years, by an increase in IgE mediated sensitization. The clinical manifestations of latex allergy affect the skin (urticaria and angioneurotic oedema), the lower and the upper respiratory tracts (rhinoconjunctivitis, asthma and glottis oedema), and the cardiovascular system (anaphylaxis). There is also an anaphylactic risk during surgery and invasive diagnostic procedures. Vegetable food cross-reacts with latex so that more than half of the patients show specific IgE against some food. Further than traditional groups at risk

[Latex allergy].

Immediate hypersensitivity to natural rubber latex has increased since the early 1980s. High prevalence of latex sensitization and allergy are observed among healthcare workers, atopic individuals and children who had undergone multiple surgical operations (spina bifida, congenital anomalies). Presenting symptoms are polymorphous: contact urticaria, rhinitis, conjunctivitis, asthma, and anaphylaxis. Corn-starch-latex particles released in the air after powdered gloves manipulation may be inhaled and lead to occupational asthma. The diagnosis may be made by a focused clinical history, prick-tes…

Adverse reactions to foods.

Allergic reactions to foods represent a prominent, actual and increasing problem in clinical medicine. Symptoms of food allergy comprise skin reactions (urticaria, angioedema, eczema) respiratory (bronchoconstriction, rhinitis), gastrointestinal (cramping, diarrhea) and cardiovascular symptoms with the maximal manifestation of anaphylactic shock. They can be elicited by minute amounts of allergens. The diagnosis of food allergy is done by history, skin test, in vitro allergy diagnosis and--if necessary--oral provocation tests, if possible placebo-controlled. Avoidance of respective allergens f

A mixed immediate and delayed allergy to oral vitamin D supplementation: a case report.

BACKGROUND: Vitamin D deficiency is common worldwide and of particular concern for populations at higher latitudes, including those in devel … immunoglobulin levels. The patient was referred to an allergy clinic where a diagnosis of vitamin D hypersensitivity, with features of both immunoglobulin-E-mediated (type 1) and T-cell (type 4)-mediated pathways, was made. A desensitization program was commenced using a modified Australian protocol. During the 18-week course, the patient was exposed to increasing …

Taking a Rational Approach to a Reported Antibiotic Allergy.

Up to 10% of hospitalized patients have an antibiotic allergy label in their medical file, most frequently concerning penicillins. However, the vast majority of reported allergies to antibiotics does not represent a "true" allergy but are due to drug intolerance, idiosyncratic reactions or symptoms of the concurrent infectious disease. Since antibiotic allergy labels result in deviation from first-choice antimicrobial therapy, tackling the issue of incorrect antibiotic allergy labelling, already at young age, is a core element of antibiotic stewardship. In this article, we describe the structu

Immediate and cell-mediated reactions in parasitic infections by Anisakis simplex.

BACKGROUND: Anisakis simplex is responsible for allergic symptoms after repeated ingestion or contact with parasitized fish. OBJECTIVE: To further analyze type I and IV immunologic mechanisms in a group of patients with allergic reactions to A simplex, we performed prick-by-prick testing with A simplex larvae and patch tests with live, cooked, or frozen larvae of the A simplex parasite. METHODS: Thirty-eight patients underwent skin prick test and radioallergosorbent test with inhalant allergens, foods, and A simplex. Prick-by-prick tests with A simplex and patch tests with live, cooked, and

Food sensitivities, taste changes, aphthous ulcers and atopic symptoms in allergic disease.

Abnormalities in taste and buccal sensation, and production of abdominal symptoms, are associated with ingestion of specific foods by atopic individuals as shown by Food Sensitivity Analysis. Of 61 atopic patients all had abnormalities in buccal sensation and up to 87% related nausea or other abdominal symptoms to ingestion of specific foods. Eighty-two percent had positive skin tests to foods, 56% had a history of aphthous ulcers and 18% could associate development of the ulcers with ingestion of specific foods. Application of the responsible allergen to the ulcer base produced severe pain in

Pathogenesis and causations of systemic contact dermatitis.

Systemic contact dermatitis (SCD) describes a hypersensitivity reaction following systemic re-exposure of the inciting allergen in previously sensitized individuals. Plants, drugs, and metals are the most common causes of SCD. In individuals with obstinate dermatitis, it is imperative to deliver history-focused patch testing with subsequent avoidance and elimination of the compound.

Erythema multiforme.

An active duty male Soldier presents to your clinic with concerns of blister-like lesions on both hands and feet several weeks after receiving immunizations. He is diagnosed with erythema multiforme (EM), a hypersensitivity reaction that is typically self-resolving. This article reviews the etiologies, pathophysiology, course, diagnosis, and treatment of erythema multiforme.
