Network-based pharmacological and experimentally validated study on the therapeutic effects and mechanisms of Polygonatum Rhizoma Ginseng Formula in immunocompromised mice.

ETHNOPHARMACOLOGICAL RELEVANCE: Polygonatum Rhizoma Ginseng Formula (HJRS) is a traditional Chinese medicine preparation commonly used to enhance immunity. Traditionally, it is believed to "moisten dryness and nourish yin, tonify the spleen, and benefit qi." However, a systematic evaluation of its efficacy and mechanism of action is still lacking. AIM OF THE STUDY: To explore the pharmacodynamic effects and underlying mechanisms of HJRS in cyclophosphamide (CTX)-induced immunocompromised model mice. METHOD: To explore the immunomodulatory effects of HJRS, we used cyberpharmacology combined w

Endoscopic features of cytomegalovirus disease of the upper gastrointestinal tract between transplant and non-transplant patients.

BACKGROUND/AIMS: Cytomegalovirus (CMV) disease in the upper gastrointestinal (UGI) tract frequently occurs in immunocompromised patients. However, data regarding UGI CMV disease in non-transplant patients compared with those in transplant recipients are limited. Therefore, we compared the clinical characteristics, endoscopic findings, and outcomes of UGI CMV disease in non-transplant patients with those in transplant recipients. METHODS: We reviewed the medical records of patients diagnosed with UGI CMV disease between May 1999 and January 2022. UGI CMV disease was defined as symptoms or sign

Risk of cytomegalovirus infection in solid organ transplant recipients: A population-based cross-sectional study.

BACKGROUND: Relative to healthy individuals, in severely immunocompromised transplant recipients, the presentation of cytomegalovirus (CMV) is characterized by delayed clearance, more recurrent episodes, and faster changes in their condition over time. This study investigates CMV occurrence in solid organ transplantation (SOT) recipients, addressing a gap in epidemiological research. METHODS: Using National Health Insurance Research Database data, a cross-sectional study observed SOT recipients (2003-2019) with propensity score matching. The case group was 11,028 SOT recipients and 44,112 pat

Toxoplasma gondii infection diagnosed by tNGS in a patient with acute lymphoblastic leukemia: a case report.

BACKGROUND: Toxoplasma gondii (T. gondii) is an obligate intracellular protozoan parasite. In the context of hematological malignancies, the immunosuppression induced by chemotherapy and the underlying disease state render patients highly vulnerable to T. gondii reactivation. The consequences of such infections can be extremely severe, often leading to life-threatening complications such as toxoplasmic encephalitis (TE). There are few reports on the detection of T. gondii infection in cerebrospinal fluid (CSF) by targeted next-generation sequencing (tNGS). CASE PRESENTATION: A 5-year-old girl

Vaccination in adults at-increased risk of herpes zoster in Canada: insights from a multidisciplinary panel consensus.

BACKGROUND: Risk of herpes zoster (HZ) increases with age (notably at ≥50 years), with greater risk also apparent in immunocompromised populations. The use of the recombinant zoster vaccine (RZV) in adults aged ≥50 years is established in Canada. However, while licensed RZV use was expanded in 2021 to include individuals ≥18 years of age who are or will be at increased risk of HZ due to immunodeficiency or immunosuppression caused by known disease or therapy, there remains some uncertainty for clinicians regarding which patients should be offered vaccination. RESEARCH DESIGN AND METHODS: To a

[Immunosuppression, pregnancy and listeriosis].

In this case report on pregnancy-related listeriosis, two women with inflammatory bowel disease and immunosuppression are presented. Both were treated with intravenous ampicillin for three weeks and gentamicin for three days. Continuous fetal monitoring was performed every 12 hours and ultrasound every 3-4 days. Both infants, born at 37 and 33 weeks, required brief CPAP but were clinically unaffected and survived without sequelae. Infant 1 received antibiotics for one week as the mother was two weeks into her treatment regimen, while infant 2 was not treated as the mother had completed her reg

Physical exercise, the immune system and infection risk: implications for prehabilitation and rehabilitation for solid organ transplantation candidates and recipients.

PURPOSE OF REVIEW: Solid organ transplantation recipients have an increased risk of infection, exacerbated by immunosuppressant medications that need to finely balance suppression of the immune system to prevent allograft rejection while avoiding over-suppression leading to infections and malignancy. Exercise modulates immune functions, with moderate-intensity activities particularly associated with enhanced antiviral immunity and reduced infection incidence. However, investigations of the effects of exercise and physical activity on immune function and infection risk posttransplantation are s

Disseminated sporotrichosis presenting as left nasolacrimal duct obstruction.

Sporotrichosis, a subacute or chronic mycosis caused by the Sporothrix schenckii complex, is commonly seen in tropical or subtropical parts of the world, especially in individuals who are in contact with cats or engage in gardening activities. Sporotrichosis generally presents with skin ulceration at the site of inoculation and follows a lymphocutaneous pattern; however, it can present in various confounding ways. Here, we present a case of disseminated sporotrichosis in an immunocompromised patient who did not seem to have any of the commonly associated risk factors and initially presented wi

Vitexin-2-O-rhamnoside improves immunosuppression, oxidative stress, and phosphorylation of PI3K/Akt signal pathway in cyclophosphamide treated mice.

Vitexin-2-O-rhamnoside (VR) is an important active substance in hawthorn, which is widely used as a food or functional food raw material; however, its immunomodulatory activities have not been extensively studied. In this study, BALB/c mice immunocompromised by cyclophosphamide (CY) were used as models to explore the effects of VR on the immunity and antioxidant capacity of mice. The results revealed that VR can restore weight to the immunosuppressed mice to varying degrees, improve spleen and thymus injury, and restore peripheral blood levels. Furthermore, it can effectively promote the proli

Chitosan Protects Immunosuppressed Mice Against Cryptosporidium parvum Infection Through TLR4/STAT1 Signaling Pathways and Gut Microbiota Modulation.

Cryptosporidium parvum infection is very common in infants, immunocompromised patients, or in young ruminants, and chitosan supplementation exhibits beneficial effects against the infection caused by C. parvum. This study investigated whether chitosan supplementation modulates the gut microbiota and mediates the TLR4/STAT1 signaling pathways and related cytokines to attenuate C. parvum infection in immunosuppressed mice. Immunosuppressed C57BL/6 mice were divided into five treatment groups. The unchallenged mice received a basal diet (control), and three groups of mice challenged with 1 × 106

Evaluation of Two Commercial Kits on the Automated ELITe InGenius PCR Platform for Molecular Diagnosis of Toxoplasmosis.

Molecular diagnosis of toxoplasmosis is essential for establishing the diagnosis of congenital contaminations and for primary infection or reactivation of immunocompromised patients. An integrated extraction and real-time PCR-based system is of particular interest in this context. Commercial kits for automated extraction and amplification steps are now available. Herein, we assessed two commercial PCR assays for this diagnosis, those of Bio-Evolution and Elitech, on the ELITe InGenius platform. The Bio-Evolution assay showed a specificity and a sensitivity of 100% on clinical samples, but a lo
