Microfilaria in kidney biopsy: A report of two cases.

Filariasis is traditionally diagnosed on finding microfilaria in peripheral blood smear and in cases of occult filariasis by immunological tests. Clinically, it presents with a wide range of symptoms and signs. We present two cases of filariasis presenting with renal symptoms warranting kidney biopsy. Histopathology of both revealed microfilaria in the glomerular capillary loop. First case was a 23-year-old male who presented with complaints of passage of milky urine. Second case was a 36-year-old female patient who presented with complaints of gross hematuria. Urine examination revealed nephr…

Characterization of septin expression in normal and fibrotic kidneys.

Chronic kidney disease (CKD) is characterized by the loss of nephrons and worsening organ-fibrosis that leads to deterioration and ultimately the total breakdown of kidney function. Renal fibrosis has become a major public health problem worldwide and necessitates hemodialysis and kidney transplantation in affected patients. CKD is mainly characterized by the activation and proliferation of interstitial fibroblasts and by excessive synthesis and accumulation of extracellular matrix components, causing the disruption of the normal tissue architecture of the kidney. Septins are GTPase proteins a…

The Future of IL-1 Targeting in Kidney Disease.

Interleukin (IL)-1α and IL-1β are proinflammatory cytokines that play a role in many diseases such as rheumatoid arthritis, juvenile rheumatoid arthritis, gout, and periodic inflammatory syndromes, including familial Mediterranean fever and Muckle-Wells syndrome. Drugs targeting IL-1 such as recombinant IL-1Ra (anakinra), neutralizing anti-IL-1β antibodies (canakinumab) and IL-1β traps (rilonacept) are in clinical use to treat these diseases. Additionally, experimental evidence suggests a role of IL-1 in kidney disease and hypertension and targeting IL-1 showed promising results in high cardio…

Hemodialysis and Kidney Transplantation as Predisposing Conditions to Onychomycosis.

BACKGROUND/AIM: Onychomycosis (OM) is one of the commonest superficial fungal infections. Patients undergoing hemodialysis (HD) treatment and kidney transplant recipients (KTR) are considered at risk of contracting fungal infections, but the few published data do not reach the conclusion of whether they are predisposed to OM. This study aimed to determine the prevalence and etiology of OM in these patients and to determine the antifungal susceptibility profile of the isolated fungal species. METHODS: We recruited 149 HD patients, 187 KTR, and a control group comprising 174 patients attending …

Choice of fluids in the perioperative period of kidney transplantation.

Normal saline has traditionally been the resuscitation fluid of choice in the perioperative period of kidney transplantation over balanced potassium solutions. However, the problems arising from hyperchloraemia triggered by the infusion of normal saline have led to studies being conducted that compare this solution with balanced solutions. From this narrative review it can be concluded that the use of balanced crystalloids containing potassium in the perioperative period of kidney transplantation can be considered safe. These solutions do not affect serum potassium levels any more than normal …

Therapy of acute kidney injury in the perioperative setting.

PURPOSE OF REVIEW: The current review analyzes the current pharmacologic approaches in cardiac surgery-associated acute kidney injury and renal replacement/support therapies. RECENT FINDINGS: Hemodynamic management and promising therapies, including atrial natriuretic peptide, calcium sensitizer inodilators, and mesenchymal stem cells have been discussed. Encouraging results from pre-emptive renal replacement therapies have been also addressed. SUMMARY: Cardiac surgery is responsible for the highest risk of renal dysfunction with respect to other surgical settings. A number of different and …

Metabolomic Profiling in Individuals with a Failing Kidney Allograft.

BACKGROUND: Alteration of certain metabolites may play a role in the pathophysiology of renal allograft disease. METHODS: To explore metabolomic abnormalities in individuals with a failing kidney allograft, we analyzed by liquid chromatography-mass spectrometry (LC-MS/MS; for ex vivo profiling of serum and urine) and two dimensional correlated spectroscopy (2D COSY; for in vivo study of the kidney graft) 40 subjects with varying degrees of chronic allograft dysfunction stratified by tertiles of glomerular filtration rate (GFR; T1, T2, T3). Ten healthy non-allograft individuals were chosen as …

What happens to the heart in chronic kidney disease?

Cardiovascular disease is common in patients with chronic kidney disease. The increased risk of cardiovascular disease seen in this population is attributable to both traditional and novel vascular risk factors. Risk of sudden cardiac or arrhythmogenic death is greatly exaggerated in chronic kidney disease, particularly in patients with end stage renal disease where the risk is roughly 20 times that of the general population. The reasons for this increased risk are not entirely understood and while atherosclerosis is accelerated in the presence of chronic kidney disease, premature myocardial i…

[Pathogenesis and treatment of anemia in chronic kidney disease].

Chronic kidney disease leads to dysfunction of renal erythropoietin-producing cells, resulting in a decrease in erythrocyte production. A decrease in oxygen delivery to vital organs due to anemia results in worse quality of life and is associated with poor prognosis of the patients. Treatment with human recombinant erythropoietin or its improved version of erythropoiesis-stimulating agent (ESA) with a longer half-life is effective. However, some patients show hyporesponsiveness to ESA. Hyporesponsiveness to ESA, which can be induced by factors such as inflammation and uremic toxins, is associa…

A Case of Kidney Involvement in Primary Sjögren's Syndrome.

BACKGROUND Sjögren's syndrome is an autoimmune disorder caused by the infiltration of monocytes in epithelial glandular and extra-glandular tissues. Hallmark presentations include mouth and eye dryness. Although renal involvement is uncommon in primary Sjögren's syndrome (pSS), patients may experience renal tubular acidosis type I (RTA I), tubulointerstitial nephritis, diabetes insipidus (DI), nephrolithiasis, and Fanconi syndrome. However, it is atypical to see more than 1 of these manifestations in a single patient. CASE REPORT We present the case of a 24-year-old woman with polyuria and pol

[Obesity and kidney disease - renal consequences of an "epidemic"].

Overweight and obesity are widespread in the German population, affecting not only adults but also a significant number of children and adolescents. The risk to develop chronic kidney disease is markedly increased in overweight or adipose children, adolescents and adults.Overweight and obesity induced risk factors have a direct impact on the development of chronic renal disease (obesity-associated focal segmental glomerulosclerosis). They accelerate the progression of coexistent nephropathies (diabetic or hypertensive nephropathy, primary glomerulonephritides) and are independent risk factors …

Chikungunya in kidney transplant recipients: A series of cases.

Chikungunya (CHIK) is a mosquito-borne virus (CHIKV) infection that recently appeared in the Americas and thousands of confirmed cases have been reported in Brazil since the first autochthonous cases were reported in September 2014. We reported four cases of CHIK in kidney transplant recipients. The diagnosis was confirmed by positive CHIKV real-time polymerase chain reaction in two cases and positive CHIKV-IgM serology in two patients. The time between transplantation and CHIKV infection ranged from 2 to 11 years. All of them had arthralgia, and 3 of them had fever. Other symptoms were mild c…

Development of a Pig Model for Laparoscopic Kidney Transplant.

OBJECTIVES: Our aim was to describe a standardized laparoscopic kidney transplant procedure in a pig model. MATERIALS AND METHODS: Ten pigs underwent laparoscopic kidney autotransplant. A right-hand assisted nephrectomy was performed through a Pfannenstiel incision. After the graft was washed with Ringer lactate, it was transplanted into the right iliac vessels by pure laparoscopy. To maintain cold ischemia, a gauze-wrapped ice slush was placed below the allograft. The ureteroneocystostomy was performed through the Pfannenstiel incision. The contralateral ureter was ligated at the end of the …

[The efficacy and safety of bisphosphonate for patients with kidney disease].

Bisphoshonate is largely used for treatment of osteoporosis in general population. The gastrointestinal absorption is poor and it can be excreted via only kidney. Therefore, CKD patients can accumulate a higher dose of bisphosphonate in their body. Although the primary action of bisphosphonate is the inhibition of bone resorption, it secondary suppresses bone formation, which can often lead to adynamic bone disease in patients with CKD. In CKD patients who still have normal parathyroid hormone, calcium, and phosphate levels, randomized trials showed the similar effects as in non-CKD patients;h…
