An epidermal inclusion cyst mimicking chronic prepatellar bursitis: a case report.

Soft tissue lesions are common to the prepatellar region, often due to acute or chronic trauma, and most frequently include prepatellar bursitis, lipomas, and ganglion cysts. We report a case of a posttraumatic prepatellar epidermal inclusion cyst to highlight the diagnostic complexities that can arise with soft tissue lesions in this location. On the basis of our case report, treating orthopaedic surgeons should consider the possibility of an epidermal inclusion cyst in patients who present with atypical anterior soft tissue masses with a history of trauma to the anterior knee.

Detection of traumatic arthrotomy of the knee using the saline solution load test.

BACKGROUND: The saline solution load test helps to determine if a wound extends into the knee joint. Little is known about the volume of injected intra-articular saline solution that is needed to effectively rule in or rule out a traumatic arthrotomy of the knee. The purpose of the present study was to determine the appropriate volume and needle location for the diagnosis of a traumatic knee arthrotomy and to assess the effect of associated variables, including knee circumference, body mass index, and sex. METHODS: Fifty-six consecutive patients scheduled for knee arthroscopy were enrolled. A

Aseptic complications after total knee arthroplasty.

Aseptic complications after total knee arthroplasty are occurring less frequently than they did one or two decades ago. This is related in part to technical advancements, design improvements, and changes in perioperative management. Extensor mechanism dysfunction is the most frequent complication and the most commonly cited reason for secondary surgery after total knee arthroplasty. Mechanical wear, tibiofemoral instability, periprosthetic fracture, thromboembolic disease, compromised wound healing, neurovascular problems, and stiffness are less common, but nevertheless troublesome, sources of

[Ultrasound imaging of congenital knee joint dislocation. Value in diagnosis and therapy].

Congenital dislocation of the knee is a uncommon malformation. Frequently there is an association with other congenital deformities including congenital dislocation of the hip. The classification of Leveuf and Pais into three separate groups (Type A, Type B, Type C) is now widely accepted. Between October 1989 and April 1995 we evaluated ten children (five girls and five boys) with twelve dislocated knees. Clinical, radiographic and different ultrasonography examinations were carried out on both knees. The sonographic classification showed Type A in 3, Type B in 5 and Type C in 4 cases. The so

Use of allografts in knee reconstruction: I. Basic science aspects and current status.

Allografts were first used in reconstructive surgery of the knee early in this century. Their widespread use and acceptance paralleled the development of modern tissue banks and our increased understanding of the immune system. Advantages of allogeneic tissue use include less surgical morbidity, shorter surgical time, smaller incisions, and the wider selection of graft sizes and types of tissue. Disadvantages include the risk of disease transmission, a slower biologic remodeling process, and the potential for a subclinical immune response. Allografts can be obtained in several forms, including

[Localized pigmented villonodular synovitis: a primary overlooked diagnosis in a young female patient].

Localized pigmented villonodular synovitis is a rare condition of the knee that may present with symptoms suggesting internal derangement. We report a case of juvenile patient who presented with a 1-year history of signs and symptoms of a loose body in the knee. Radiographic examination and MRI were within normal limits. The lesion was treated arthroscopically by resection.

Transtibial transposition of gastrocnemius muscle and musculocutaneous flaps.

The medial and lateral gastrocnemius muscle flaps have become the workhorses of proximal calf reconstruction since their initial description by McCraw et al. in 1977. Since that time many authors have presented varying and often ingenious techniques by which the range of these flaps can be expanded. This report adds another modification of the standard gastrocnemius muscle flap: transtibial transposition of the muscle through the posterior cortex.

Partial epiphysiodesis to address genu varum or genu valgum.

Two groups of children with excessive genu valgum or genu varum were treated with a partial epiphysiodesis. In group I (16 extremities) a predictive chart was used to determine the appropriate bone age to perform the partial epiphysiodesis, and in group II (15 extremities) the age for the procedure was estimated. The partial epiphysiodesis was performed by either an open or a percutaneous technique. In group I the correction of the genu valgum and genu varum followed the predicted course, whereas group II resulted in a 9.1 degree error. Genu valgum and genu varum in children with adequate rema

Patellofemoral relationships and distal insertion of the vastus medialis muscle: a magnetic resonance imaging study in nonsymptomatic subjects and in patients with patellar dislocation.

The correlation between the insertion level of the vastus medialis muscle and lateral patellofemoral angle (LPA), lateral patellar displacement (LPD) and tilt (LPT), and the height of the patella (LT/LP) was analyzed by magnetic resonance imaging (MRI) in patients with patellar dislocation (n = 10) and in control subjects (n = 10). Images were produced in 0 degrees and 20 degrees of knee flexion. The insertion level to the patella was also analyzed with the knee in extension. No significant correlation was noted between the insertion level and different patellofemoral indexes, but multiple-reg

Anterior cruciate ligament functional instability. A static intra-articular and dynamic extra-articular procedure.

The static and functional results in a series of 50 patients evaluated at least two years after surgery were gratifying. None had any episodes of functional instability. Forty-six of the 49 who participated in sports prior to operation returned to all desired sports, including six patients who returned to intercollegiate and national team sports. The overall subjective and objective results were 30, excellent; 17, good; one, fair, and two, failure. Of the two failures, one had excellent static stability but significant pain due to pre-existing chondromalacia patella, and the other had a postsu

Reconstruction of traumatic complete loss of the medial collateral ligament: a case report.

An unusual problem with traumatic complete loss of the medial collateral ligament in a 23-year-old man with a gunshot injury, was solved by transfer of the medial head of the gastrocnemius. At one-year follow-up the patient had returned to work with satisfactory functional results with respect to stability, strength, and range of motion. The medial head of the gastrocnemius provided a substantial buttress and effectively performed the function of the collateral ligament.

The diagnosis of problems after meniscectomy.

The causes of persistent symptoms after meniscectomy have been assessed clinically, radiologically and arthroscopically in 174 knees. The commonest finding was early degenerative arthritis (chondromalacia) of the femoral condyle (40 per cent). Retained fragments of meniscus were less common than expected (13 per cent), and lesions of the other meniscus were rare (5 per cent). The clinical diagnosis was altered in 42 percent by arthroscopic examination. Arthroscopy was found to be a valuable technique in this group of patients with problems of diagnosis.

Intraoperative Complications in Medial Opening Wedge High Tibial Osteotomy.

Medial opening wedge high tibial osteotomy has been established for treatment of medial symptomatic knee arthrosis with varus malalignment in young and elderly but active patients. To obtain satisfactory results, it is essential for surgeons performing osteotomy to identify, prevent, and treat potential intraoperative adverse events. Lateral hinge fracture (LHF) is the most common intraoperative complication while popliteus artery injury is rare but limb-threatening. Computed tomography is the benchmark to detect LHF, the risk of which increased markedly with the opening gap larger than 11 mm.

Associated ACL risk factors differences during an unanticipated volleyball blocking movement.

The lateral tilt of the arms accompanied by trunk lateral tilt is a typical blocking manoeuvre in volleyball. However, during this unanticipated blocking movement, an associated risk of ACL injury may result. The aim of the present study was to compare associative ACL risk factors at the initial contact and the first and second peak of VGRF during an unanticipated blocking movement with different arm positions. Synchronized kinematic and kinetic data were collected for each trial of each condition. Student paired t-tests and effect size were used to determine differences between two conditions

Posterolateral sling reconstruction of the popliteus tendon: an all-arthroscopic technique.

Injuries to the posterolateral corner of the knee present with variable injury patterns that have produced a number of reconstructive procedures in the literature. We present an all-arthroscopic technique that reconstructs the popliteus tendon using either a semitendinosus autograft or an anterior tibialis allograft. After exposure of the posterior tibia by use of the posteromedial and trans-septal portals for visualization and posterolateral portal as a working portal, the popliteus musculotendinous junction is identified. In preparation for tibial tunnel drilling, a Kirschner wire is passed

Displacement of the common peroneal nerve associated with upper tibial fracture: implications for fine wire fixation.

The constant anatomic position of the common peroneal nerve is relied on when performing fine wire external fixation in the upper tibia. We report the case of a sixty-two-year-old woman with a Schatzker Type V fracture of her right tibial plateau and upper-third diaphyseal fracture associated with displacement and shortening of the upper tibia. She was treated by minimal internal fixation of the intraarticular fracture and application of a Sheffield Hybrid External Fixator. During percutaneous insertion of the reference wire in the fibular head, a distal muscle twitch alerted the surgeon, and
