Efficacy and safety of invasive laser acupuncture (650 and 830 nm) on knee osteoarthritis: A pilot randomized clinical trial.

UNLABELLED: This pilot study explored the safety and efficacy of invasive laser acupuncture (ILA) using two wavelengths (650 and 830 nm) compared with a sham laser in patients with knee osteoarthritis (KOA). This randomized, parallel-arm, patient- and outcome assessor-blinded, placebo-controlled, single-center clinical trial was designed with three groups (control, 650, and 830 nm), and 45 participants were randomly allocated in a ratio of 1:1:1. All participants were treated with ILA on the more affected side at SP9, SP10, ST34, ST35, EX-LE2, EX-LE4, and GB34 for 10 minutes per session, twice

Impact of a patient-centred levothyroxine decision support tool on rapid achievement of target dosage and improved patient outcomes: protocol for a randomised controlled trial.

INTRODUCTION: The hormone replacement drug levothyroxine is the most common therapy for people with reduced function of the thyroid gland. The optimal dosage varies considerably between individuals, and reaching the correct dosage is often a time-consuming task. To reduce the time needed for dose adjustments, we have developed a model for calculating each patient's optimal dosage and an associated decision support tool (DST). We present the study protocol for a randomised controlled trial using the DST in initiation and adjustment of levothyroxine therapy for thyroidectomised patients. The aim

Foslevodopa/Foscarbidopa Subcutaneous Infusion Safety and Efficacy in Patients with and Without Prior Deep Brain Stimulation.

INTRODUCTION: As Parkinson's disease (PD) advances, limitations of oral medication include pill burden, increasing "Off" time, and "On" time with bothersome dyskinesia. Deep brain stimulation (DBS) can improve motor complications, but disease progression may warrant further intervention later. An approved nonsurgical option, continuous subcutaneous infusion of foslevodopa/foscarbidopa (LDp/CDp), has been shown to improve motor fluctuations, though the impact of prior DBS on its efficacy and safety is unknown. METHODS: Post hoc analysis of a 52-week open-label phase 3 trial (NCT03781167) evalu

Efficacy of adjunctive Yoga Nidra in patients with functional dissociative seizures receiving structured psychoeducation (YOGA-FDS): a pilot randomised controlled trial.

BACKGROUND: Functional dissociative seizures (FDS) are paroxysmal seizure-like events with no electrographic abnormalities. Structured psychoeducation is considered a standard component of FDS management. Yoga Nidra (YN) is a guided meditative relaxation technique that has shown benefit in other neuropsychiatric conditions, but its adjunctive role in FDS has not been studied. OBJECTIVE: To evaluate whether adjunctive YN provides additional benefit over structured brief psychoeducation alone in reducing monthly FDS frequency. METHODS: This open-label, randomised controlled pilot trial enrolle

Protocol for an open-label, randomised, controlled trial to evaluate the efficacy and safety of sotatercept add-on therapy compared with pulmonary vasodilator-based standard of care for pulmonary vasodilator-resistant pulmonary arterial hypertension associated with unrepaired congenital shunts (atrial septal defect, ventricular septal defect or patent ductus arteriosus), including Eisenmenger syndrome: the SuMILE trial.

INTRODUCTION: Eisenmenger syndrome and pulmonary arterial hypertension (PAH) due to unrepaired congenital shunts, including atrial septal defect (ASD), ventricular septal defect (VSD) and patent ductus arteriosus (PDA), remain life-threatening conditions despite advances in congenital heart disease (CHD) care. In this population, vasodilator-based therapies effective in other forms of PAH have shown limited benefit, and no disease-modifying treatment has been established. Sotatercept, an activin-signalling inhibitor, improved exercise capacity and haemodynamics in phase 2/3 PAH trials; however

Electroacupuncture Versus Auricular Acupressure Versus WaitliSt Control for Endocrine Therapy-Related Joint Pain in Patients With Breast Cancer (EASE): Study Protocol for a Pragmatic Multicenter Randomised Controlled Trial.

BackgroundThe incidence of breast cancer has been steadily increasing in recent years. Endocrine therapy, as a first-line treatment, is typically required for 5 to 10 years. However, joint pain is one of the most common adverse effects, affecting 50%-70% of patients and significantly compromising treatment adherence and quality of life. Current management strategies have limited efficacy. Electroacupuncture has been shown to alleviate joint pain intensity and improve related symptoms, but the need for frequent hospital visits imposes a logistical burden on patients and their families, which ma

Efficacy and safety of a selective partial agonist for nociception/orphanin-FQ peptide (NOP) receptors in patients with insomnia disorder.

AIMS: Insomnia is a common sleep disorder, affecting up to 20% of the world population and adversely impacting productivity, health, and overall well-being. Although pharmacologic options exist to treat insomnia, the health-related quality of life for patients who are prescribed hypnotics is no higher than for those who are not, revealing a significant treatment gap. Sunobinop, an oral, selective, potent, partial agonist of the nociceptin receptor (NOP), was evaluated for efficacy and safety in the treatment of Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) insomn

Study protocol of a randomized controlled superiority trial of Huddinge Online Prolonged Exposure therapy (HOPE) for adults with posttraumatic stress disorder.

BACKGROUND: Cognitive behavioral therapy (CBT) is considered a first-line treatment for posttraumatic stress disorder (PTSD) but is rarely available in regular care. Action is clearly needed to increase treatment availability. A possible solution to improve access to treatment would be the use of remotely delivered digital intervention. Internet-delivered CBT may also carry several other advantages compared to traditional psychological treatments (e.g., less therapist time per patient, bridging geographical distances between therapist and patient, and provision of a standardized intervention).

Structured Personalized Oxygen and Supportive Therapies for Dyspnea in Oncology (SPOT-ON): A personalized randomized clinical trial protocol.

BACKGROUND: Low-flow supplemental oxygen (LFSO), high-flow nasal cannula (HFNC), and non-invasive ventilation (NIV) are often used for palliation of dyspnea in hospitalized patients. However, it is unclear how patients prefer to use these modalities and how they can be personalized for optimal dyspnea relief. The objective of this randomized clinical trial is to compare the effect of a personalized respiratory therapist (RT)-led intervention, termed SPOT-ON, and enhanced usual care on dyspnea intensity in both hypoxemic and non-hypoxemic hospitalized patients with cancer. METHODS: In this two

A Pilot Randomised Non-Inferiority Trial of Auto-Titrating Versus Fixed Continuous Positive Airway Pressure for Obesity Hypoventilation Syndrome With Severe Obstructive Sleep Apnoea.

BACKGROUND AND OBJECTIVE: Fixed CPAP (fCPAP) is the preferred first-line ventilatory therapy in patients with obesity hypoventilation syndrome with severe obstructive sleep apnoea (OHS + sOSA). Auto-titrating CPAP (APAP) is not recommended but has not been systematically evaluated in this population. Our pilot study aimed to provide feasibility data for a larger comparative effectiveness trial to determine whether APAP is non-inferior to fCPAP for improving hypercapnic respiratory failure. METHODS: Participants with OHS + sOSA were randomised to either APAP or fCPAP for 3 months. The primary

The role of neuromodulation in heart failure with preserved ejection fraction.

BACKGROUND: Heart failure with preserved ejection fraction (HFpEF) is a clinical syndrome with limited treatment options to improve long-term outcomes such as quality of life, exercise capacity, and mortality. Neuromodulation-based therapies have emerged as potential interventions to address autonomic dysregulation in HFpEF. This review discusses the long-term efficacy and safety of four key neuromodulation therapies: Renal Denervation (RDN), Baroreceptor Activation Therapy (BAT), Vagus Nerve Stimulation (VNS), and Greater Splanchnic Nerve (GSN) Ablation. Each therapy shows promise, but variab
