Prevalence and factors of frailty among community-dwelling older adults: a multimorbidity perspective.

BACKGROUND: Frailty is a geriatric syndrome that reduces quality of life and increases social burden. Existing frailty research predominantly focuses on hospitalized or nursing home-residing older adults, while surveys targeting community-dwelling older adults with multimorbidity remain scarce. OBJECTIVE: To investigate the prevalence and influencing factors of frailty among community-dwelling older adults with multimorbidity. METHODS: A cross-sectional survey was conducted from May to November 2025. Convenience sampling was adopted to recruit multimorbid older adults from five communities i

Dipeptidyl peptidase IV contributes to the pathogenesis of Cryptococcus neoformans.

We previously identified isoleucine-proline-isoleucine (IPI) as a small molecule associated with extracellular vesicles (EVs) in Cryptococcus. The administration of IPI in vivo resulted in the control of experimental cryptococcal infection through mechanisms that remain unknown. IPI is an inhibitor of dipeptidyl peptidase 4 (Dpp4), an enzyme that regulates insulin and glucose homeostasis in mammals and may also influence pathogenic processes in fungi. Based on these observations, we investigated the biological and pathogenic roles of cryptococcal Dpp4. We demonstrate that Cryptococcus neoforma

Identification of genes promoting fitness of a plant-associated Salmonella Choleraesuis strain on alfalfa sprouts during cold storage.

Consumption of sprouted seeds, such as alfalfa sprouts, has increased in recent years due to their nutritional value and antioxidant content. However, these products have repeatedly been implicated in outbreaks of foodborne pathogens, including Salmonella enterica. Although host-adapted Salmonella serovars are less frequently associated with foodborne illness, infections caused by these serovars often result in invasive and severe outcomes, highlighting the importance of understanding their persistence in food production systems. Moreover, the variability among Salmonella serovars requires cha

Advanced Endoscopic Management of a Full-Thickness Esophageal Perforation From Food Impaction: A Case of Successful Stent-Assisted Healing.

Esophageal perforation is a rare but life-threatening condition with historically high morbidity and mortality. Traditional management has relied on urgent surgical repair, but advances in endoscopic therapy have expanded non-surgical options. We present a case of an 84-year-old male patient with an esophageal perforation caused by food impaction in the setting of a Schatzki ring, successfully managed through a stepwise endoscopic approach using a fully covered self-expanding metal stent (FCSEMS) followed by argon plasma coagulation (APC) and endoscopic clipping to close a residual defect. Catheterization Method and Functional Recovery of Neurogenic Bladder in Spinal Cord Injury.

IMPORTANCE: Neurogenic bladder dysfunction affects up to 80% of individuals with spinal cord injury (SCI), greatly impacting quality of life and health outcomes. Although current guidelines recommend intermittent catheterization primarily for infection prevention, its potential influence on recovery of volitional bladder control remains poorly understood. OBJECTIVE: To determine whether intermittent catheterization, compared with indwelling catheterization, is associated with a higher likelihood of regaining volitional bladder control within 1 year after discharge in individuals with SCI, and

Methotrexate Use in the Management of Ectopic Pregnancies: A Retrospective Study.

Introduction Ectopic pregnancy (EP), most commonly located in the fallopian tube, presents with symptoms such as abdominal pain, vaginal bleeding, and nausea, which often result in delayed diagnosis, increasing the risk of rupture and life-threatening complications. Methotrexate (MTX) is frequently used to treat stable, unruptured EPs, but success rates vary widely, and optimal dosing strategies and predictive factors remain unclear. This study evaluates MTX efficacy in managing tubal EPs and identifies factors that may guide treatment decisions to improve outcomes and standardize care at an u

Exploring sensory alterations and repetitive behaviors in children with autism spectrum disorder from the perspective of artificial neural networks.

BACKGROUND: Restrictive repetitive behaviors (RRBs) and sensory processing disorders are core symptoms of autism spectrum disorder (ASD). Their relationship is reported, but existing data are conflicting as to whether they are related but distinct, or different aspects of the same phenomenon. AIMS: This study investigates this relationship using artificial neural networks (ANN) analysis and an innovative data mining analysis known as Auto Contractive Map (Auto-CM), which allows to discover hidden trends and associations among complex networks of variables (e.g. biological systems). METHODS A

Proximal Humerus Fractures: A Review of Anatomy, Classification, Management Strategies, and Complications.

Proximal humerus fractures are prevalent in older adults, particularly women, primarily due to osteoporosis and increased fall risk. These fractures often result from low-energy falls in elderly patients, while in younger individuals, they are more likely to occur with high-energy trauma, which may involve additional injuries to soft tissue and neurovascular structures. Proper anatomical understanding, including key structures and blood supply, is crucial for effective management and to prevent complications. Several classification systems assist in guiding treatment for proximal humerus fract

A mixed integer programming model for vaccine pricing within a group purchasing organization.

BACKGROUND: Setting prices for life-saving medical or pharmaceutical products needs to consider multiple factors, e.g., affordability and health outcomes across different populations. When a group of buyers (e.g., countries) combine their purchasing power (e.g., via a group purchasing organization), the average procurement price decreases in the total volume. Decisions about what price to then charge to each member in a group are particularly challenging, considering the disparities in their respective ability and willingness to pay. Tiered pricing can be an effective way to set prices for a g

[Design and application of a new transoral gastric tube for extraction and storage of gastric contents].

Difficulty in swallowing is a common symptom in stroke patients, and nasogastric tubes are routinely used to solve the nutritional support problem of these patients. The existing nasogastric tube have the disadvantages of causing aspiration pneumonia and patient discomfort. The traditional transoral gastric tube has no one-way valve switch and gastric content storage device, and cannot be fixed in the stomach, resulting in reflux of gastric contents, inability to fully understand the digestion and absorption of gastric contents, and accidental dislocation of the gastric tube, affecting further

Prediction of Noninvasive Ventilation Failure in COVID-19 Patients: When Shall We Stop?

INTRODUCTION: In coronavirus disease 2019 (COVID-19), there are no tools available for the difficult task of recognizing which patients do not benefit from maintaining respiratory support, such as noninvasive ventilation (NIV). Identifying treatment failure is crucial to provide the best possible care and optimizing resources. Therefore, this study aimed to build a model that predicts NIV failure in patients who did not progress to invasive mechanical ventilation (IMV). METHODS: This retrospective observational study included critical COVID-19 patients treated with NIV who did not progress to

'If we don't assess the patient's vision, we risk starting at the wrong end': a qualitative evaluation of a stroke service knowledge translation project.

BACKGROUND: Visual impairments (VIs) affect 60% of stroke survivors and have negative consequences for rehabilitation and quality of life poststroke. Symptoms of VIs post stroke are difficult to identify for stroke survivors and health care professionals without using a structured vision assessment. In this study, we qualitatively evaluate the implementation outcomes after implementing a structured visual assessment with the Competence, Rehabilitation of Sight after Stroke Vision (KROSS) assessment tool in stroke care services. METHODS: This is a qualitative study comprising four focus group

Reconceptualizing non-pharmacologic approaches to Neonatal Abstinence Syndrome (NAS) and Neonatal Opioid Withdrawal Syndrome (NOWS): A theoretical and evidence-based approach. Part II: The clinical application of nonpharmacologic care for NAS/NOWS.

There has been increasing emphasis on the importance of the development of self-regulatory capacities of the individual as the cornerstone of development. The caregivers' abilities to manage their own attention, emotions, physiology and behaviors influence the development of the child's self-regulatory and interactive capacities, and thereby their overall development. Newborns prenatally exposed to psychoactive substances and/or to other prenatal stressors such as maternal poor nutrition, increased maternal stress, trauma, difficult and/or impoverished environments, in tandem with genetic pred

Theoretical background, first stage development and adaptation of a novel Integrative Adapt Therapy (IAT) for refugees.

AIMS: Refugees are confronted with the task of adapting to the long-term erosion of psychosocial systems and institutions that in stable societies support psychological well-being and mental health. We provide an overview of the theoretical principles and practical steps taken to develop a novel psychotherapeutic approach, Integrative Adapt Therapy (IAT), which aims to assist refugees to adapt to these changes. This paper offers the background informing ongoing trials of IAT amongst refugees from Myanmar. METHODS: A systematic process was followed in formulating the therapy and devising a tre

A novel integrated care concept (NICC) versus standard care in the treatment of chronic cardiovascular diseases: protocol for the randomized controlled trial CardioCare MV.

BACKGROUND: Cardiovascular diseases are the major cause of death globally and represent a major economic burden on health care systems. Positive effects of disease management programs have been shown for patients with heart failure (HF). Remote monitoring and telemonitoring with active intervention are beneficial in atrial fibrillation (AF) and therapy-resistant hypertension (TRH), respectively. For these patients, we have developed a novel integrated care concept (NICC) which combines telemedicine with intensive support by a care center, including a call center, an integrated care network inc

[Restless-legs syndrome].

Restless-legs syndrome (RLS) is a sensorimotor disorder, characterized by an irresistible urge to move the legs usually accompanied or caused by uncomfortable and unpleasant sensations. It begins or worsens during periods of rest or inactivity, is partially or totally relieved by movements and is exacerbated or occurs at night and in the evening. RLS sufferers represent 2 to 3% of the general population in Western countries. Supportive criteria include a family history, the presence of periodic-leg movements (PLM) when awake or asleep and a positive response to dopaminergic treatment. The RLS

Further examination of infants referred from newborn hearing screening.

OBJECTIVES: Universal newborn hearing screening (UNHS) is considered beneficial and is accepted worldwide. However, some problems remain, and administrative systematization has yet to be established in many countries. This study assessed the hearing screening of referred newborn infants and discusses the problems that remain. MATERIALS AND METHODS: Over the two years from July 2001 to June 2003, 98 ears of 49 infants were judged as a "referral" from a newborn hearing screening program, and were subsequently referred to our hospital for further examination using conventional ABR and other audi

Psychological aspects of traumatic injury in children and adolescents.

Each year millions of children are exposed to some form of extreme traumatic stressor. These traumatic events include natural disasters (e.g., tornadoes, floods, hurricanes), motor vehicle accidents, life-threatening illnesses and associated painful medical procedures (e.g., severe burns, cancer, limb amputations), physical abuse, sexual assault, witnessing domestic or community violence, kidnapping, and sudden death of a parent. During times of war, violent and nonviolent trauma (e.g., lack of fuel and food) may have terrible effects on children's adjustment. The events of September 11, 2001

Cancer-related fatigue. Clinical practice guidelines in oncology.

These guidelines propose a treatment algorithm in which patients are evaluated regularly for fatigue using a brief screening instrument, and are treated as indicated by their fatigue level. The algorithm's goal is to identify and treat all patients with fatigue that causes distress or interferes with their daily activities or functioning. Management of fatigue begins with primary oncology team members who perform the initial screening and either provide basic education and counseling or expand the initial screening to a more focused evaluation for moderate or higher levels of fatigue. At this
