Factors affecting the stability of viral vaccines.

The stability of viral vaccines is determined by the rate of loss of "integrity" of the viral antigen during storage. For live vaccines, such as measles, mumps, rubella, canine distemper, stability is equivalent to the preservation of the infectious titres. For inactivated and subunit vaccines, the preservation of the antigenic structure and the correct steric presentation of the relevant epitopes are the parameters which determine their stability. In general, the following factors may have a negative effect on stability: temperature, pH outside the physiological limits, organic solvents, repe…

Compliance to compulsory vaccination: strategies and results.

This report is devoted to analyze the effect that compulsory vaccination has on the compliance of the population, compared with the results obtained by massive campaigns for optional vaccinations. The implementation of a specific software for the management of individual schedules helps to reach a substantial complete coverage of the individuals for the first ones, while improving but incomplete results regard the vaccines against pertussis and measles-mumps-rubella, optional in Italy. The optimization of data management at the local health unit level improves the quality and the satisfaction …

Evaluation of the effectiveness of immunization delivery methods.

Scientific evidence documenting the effectiveness of immunization delivery methods was summarized using the generic approach developed by the Community Health Practice Guidelines Working Group. The delivery methods examined were those for the adult and childhood vaccines of influenza, pneumococcal infection, hepatitis B, measles-mumps-rubella and diphtheria-pertussis-tetanus-polio. Based on a critical appraisal of 54 eligible comparative studies, the effects of different interventions were obtained and pooled effects were calculated for delivery methods oriented to the client, the provider and…

Viral respiratory diseases in children: classification, etiology, epidemiology, and risk factors.

The epidemiology, molecular structure, cell tropism, and pathophysiology of many human disease-causing viruses have been painstakingly and elegantly characterized during the past 50 years. Vaccines and antiviral drugs of varying efficacy were developed and tested. Despite the relegation of smallpox to a freezer chest and the progress in the control of measles and hepatitis B, the viruses that cause respiratory tract infections remain significant causes of illness and death in pediatric populations worldwide. This discussion surveys the virus groups that contain nearly 200 distinct viruses that…

Immunizations for pediatric transplant patients.

Infection remains a major cause of morbidity and mortality in transplant patients. Many infections, however, can be successfully prevented by immunization. This presentation reviews the problems associated with, and the questions that arise concerning the use of routine pediatric vaccines, such as diphtheria-pertussis-tetanus (DPT) and measles-mumps-rubella (MMR). It also reviews the use of special vaccines such as hepatitis B, pneumococcal, and influenza vaccines in transplant patients. Data concerning the use of two experimental, live-attenuated virus vaccines, against cytomegalovirus (CMV) …

The role of epidemic infectious diseases in the discovery of America.

As the world prepares to celebrate the quincentennial events surrounding the discovery of the New World by Christopher Columbus in 1492, a particular interest regarding the influence of epidemic infectious diseases on the history of the conquest of America has emerged. Contrary to popular belief, it was not the European guns or fierce soldiers that conquered the native Americans, but instead it was the common childhood illnesses brought from the Old World by the European conquistadors. Diseases such as smallpox, measles, and typhus annihilated most of the American native populations. Devastati…

Herd immunity and the HIV epidemic.

Background. Herd immunity describes the collective immunocompetence of a population and its ability to resist disease. The diseases of mycobacteria, salmonella, hepatitis A, cryptosporidia, syphilis, measles, influenza, and numerous others recently have been seen in epidemic proportions in the United States. An association between these superimposed secondary infections and the human immunodeficiency virus (HIV) epidemic can be made since the HIV's imposition on individual immunity has ramifications on a population level through a decline in herd immunity. Conclusion. Exploring these epidemic …

Depopulation of the Northern Plains Natives.

Nine major epidemics of acute infectious diseases swept the Northern Plains of the Western Interior of Canada between 1774 and 1839. The Blackfeet, Plains Cree and Assiniboin, Atsina and Saulteaux who exploited the Plains were differentially exposed to these epidemics of smallpox, measles, whooping cough and influenza. Mortality estimates from these epidemics were used in assessing the degree to which a series of epidemics contributed to depopulation of the Plains Natives. A criteria was established to determine an epidemic from a depopulation epidemic, which involved among other factors, the …

Subacute sclerosing panencephalitis.

A retrospective study of 36 confirmed cases of subacute sclerosing panencephalitis (SSPE) was carried out to detect any variation in the clinical and EEG patterns previously described. There were 31 males and 5 females, aged 4 to 24 years. Onset of SSPE at or after 15 years of age was observed in 22.2% of cases. Rapid progression of the disease was observed in 52%. The first symptom was myoclonus in 61.6%, mental regression in 22.2% and generalised seizures in 11.1% of cases. Low positive measles antibody titres in the CSF were found in 36.1% of cases. EEG analysis revealed slow background in …

[Rubella monitoring in Norway].

WHO's "Health for All by the Year 2000" gives as subsidiary objective number 5 "The elimination of measles, poliomyelitis, neonatal tetanus, congenital sequelae of rubella, diphtheria, congenital syphilis and malaria from the European region by the year 2000". This would be attained by a well organized primary care which guarantees effective epidemiological supervision, a vaccination programme with full support, instruction on the risks associated with syphilis, and screening and eventual treatment of pregnant women. It was earlier declared in Norway that congenital rubella should not occur af…

[Persistence of viruses in rheumatoid arthritis].

Humoral and cell-mediated immune response to antigens of hepatitis B (HBsAg and HBeAg), delta-virus, rubella, and measles viruses was studied. Detection in a number of patients with rheumatoid arthritis of HBsAg and HBeAg in blood sera permits a conclusion that productive virus infection is formed in such patients. At the same time in patients with rheumatoid arthritis areactivity of immunocytes to HBsAg and HBeAg was found while marked sensitization of lymphocytes to rubella virus antigens was demonstrated. The patients with rheumatoid arthritis, however, do not develop mere long-term carrier…

Threshold and stability results for an epidemic model with an age-structured meeting rate.

This paper examines threshold and stability results for simple mathematical models for the transmission of infectious diseases with permanent natural immunity. Examples of such diseases are measles, chicken-pox, hepatitis, and mumps. An important feature of this work is the introduction of an age structure into the population amongst whom the disease is spreading and, in particular, the realization of the fact that the contact rate itself may depend on age. Equilibrium and stability analyses are performed on these models. These results are in part directed towards establishing conditions suffi…

A simple surveillance system for xerophthalmia and childhood corneal ulceration.

A simple cost-effective surveillance system is described for reporting cases of xerophthalmia and childhood corneal ulceration. The system is suitable for use by ophthalmic auxiliaries in developing countries.Presented are the results of a 2-year surveillance involving 15 hospitals in 11 areas of the United Republic of Tanzania. Cases of xerophthalmia in its early stages (characterized by night blindness or Bitot's spots) were found in all areas surveyed. Of almost 300 cases of unilateral and bilateral corneal ulceration, 46% and 79%, respectively, followed a recent infection with measles. The…

Ten years of serological surveillance in England and Wales: methods, results, implications and action.

BACKGROUND: The first age-stratified serological survey of antibody to measles, mumps and rubella in the UK was conducted in 1986/87 prior to the introduction of MMR vaccine into the immunization programme. Serum collection and testing have continued annually, allowing trends over time to be monitored. These sera have also been available for ad hoc surveys of other infections. METHODS: Residual sera are collected in participating laboratories and sent to a central store where they are irrevocably unlinked from identifying data. A unique identity number is assigned to each serum and details of…
