Early-onset or rapidly progressive scoliosis in children: check the eyes!

Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare autosomal recessive disorder characterized by the absence of conjugate horizontal eye movements, and progressive scoliosis developing in childhood and adolescence, caused by mutations in the ROBO3 gene which has an important role in axonal guidance and neuronal migration. We describe two female children aged 12 years and 18 months, with progressive scoliosis, in whom the neurological examination showed absent conjugate horizontal eye movements, but preserved vertical gaze and convergence. Cerebral Magnetic resonance imaging fin

Unilateral tremor induced by risperidone in a patient with acute mania: vitamin B12 deficiency as possible mediating factor.

Identification and management of drug-induced movement disorders is a clinical challenge, more so when the clinical presentation is atypical. A young male with acute mania was under treatment with sodium valproate and risperidone. He developed tremors of right hand and neck. These were present at rest and exacerbated by mental activity, when under observation and during voluntarily initiated activity. There were no associated extra pyramidal symptoms or cerebellar signs. Investigations for other common causes of tremors did not reveal any evidence except for low value of serum vitamin B12 leve

TorsinA and DYT1 dystonia: a synaptopathy?

DYT1 dystonia is an autosomal dominant movement disorder, characterized by early onset of involuntary sustained muscle contractions. It is caused by a 3-bp deletion in the DYT1 gene, which results in the deletion of a single glutamate residue in the C-terminus of the protein TA (torsinA). TA is a member of the AAA+ (ATPase associated with various cellular activities) family of chaperones with multiple functions in the cell. There is no evidence of neurodegeneration in DYT1 dystonia, which suggests that mutant TA leads to functional neuronal abnormalities, leading to dystonic movements. In rece

Myoclonus in fraternal twin toddlers: a French family with a novel mutation in the SGCE gene.

Myoclonus dystonia is a rare movement disorder caused by mutations in the SGCE gene on chromosome 7q21 (DYT11) encoding the epsilon-sarcoglycan. Myoclonus is present in almost all patients and affects most often neck, trunk and upper limbs. Dystonia is present in about half of the patients. The mode of inheritance is autosomal dominant with variable clinical expression and maternal imprinting. Onset is usually in childhood or adolescence. Alcohol might relieve symptoms. We present a 33 month old girl and her twin brother with disabling involuntary jerky movements during intentional tasks. Fami

Chorea-ballism associated with familial amyotrophic lateral sclerosis. A clinical, genetic, and neuropathological study.

Hyperkinetic movements in amyotrophic lateral sclerosis (ALS) are extremely rare. We present clinical, neuropathological, and genetic data for a 53-year-old woman with spinal onset ALS presenting chorea affecting the face, mouth, neck, and hands, and ballism in both arms 31 months after leg weakness onset. Her father and older sister had ALS, but had no movement disorders. As well as the typical neuropathological findings of ALS (marked upper and lower motor neuron loss), post-mortem examination showed prominent neuronal loss and gliosis in the subthalamus, and in the internal globus pallidus,

Apraxia, metaphor and mirror neurons.

Ideomotor apraxia is a cognitive disorder in which the patient loses the ability to accurately perform learned, skilled actions. This is despite normal limb power and coordination. It has long been known that left supramarginal gyrus lesions cause bilateral upper limb apraxia and it was proposed that this area stored a visual-kinaesthetic image of the skilled action, which was translated elsewhere in the brain into the pre-requisite movement formula. We hypothesise that, rather than these two functions occurring separately, both are complementary functions of chains of "mirror neurons" within

Restless legs syndrome and motor activity during sleep in spinocerebellar ataxia type 6.

BACKGROUND AND PURPOSE: We investigated the frequency of restless legs syndrome (RLS) and sleep disturbance in spinocerebellar ataxia type 6 (SCA6). PATIENTS AND METHODS: Five patients out of three multigenerational SCA6 families underwent a standardized investigation protocol including clinical interview for RLS, neurophysiological evaluation as well as the clinical assessment of ataxia. Polysomnography (PSG) was performed during two consecutive nights. RESULTS: Two out of five patients fulfilled the clinical criteria for RLS. A periodic leg movements in sleep (PLMS) index>15/h was present

Endodontic treatment of a Patient with Huntington's disease.

Huntington's disease is a lethal autosomal dominant disorder that affects men and women equally. The prevalence of Huntington's disease in North America is 5 to 10 per 100000 people. It is characterized by regular personality changes, dementia, and choreiform (dance-like) movements. Literature concerning dental treatment of patients who have been diagnosed with this type of disease is scarce. Continuous ingestion of medications to control the symptoms of this disease leads to xerostomia, which, maximized by unrestrained movements, often leads to progressive tooth decay, irreversible pulpitis,

Generalised myoclonus evolving into epilepsia partialis continua due to a cingulate gyrus lesion: case report and review of the literature.

A young Chinese male was admitted for a generalised tonic-clonic seizure preceded by a week-long history of fever. Subsequently, he developed continuous myoclonic jerks in all four limbs, with clear left sided predominance, and no accompanying clouding of consciousness. Contrast MRI of the brain demonstrated a venous angioma in the right cingulate gyrus. Over the next few days, the clinical picture evolved, with focal motor status involving primarily the left lower limb and the abdomen. These movements resolved with anticonvulsant therapy. This case illustrates generalised myoclonus arising fr

Tremor.

Tremor, a rhythmic, involuntary, oscillatory movement of body parts, is the most common movement disorder. Tremors are classified as rest or action tremors. Rest tremor occurs when the affected body part is completely supported against gravity. Action tremors are produced by voluntary muscle contraction and are further divided into postural, isometric, or kinetic tremors. This article describes clinical signs and symptoms of six tremor syndromes, including physiologic tremor, essential tremor, Parkinson's disease, toxic and drug-induced tremor, cerebellar tremor, and psychogenic tremor, and pr

Phonosurgery of the vocal folds: a classification proposal.

The Phonosurgery Committee of the European Laryngological Society (ELS) has examined the definition and technical description of phonosurgical procedures. Based on this review, the committee has proposed a working classification. The current presentation is restricted to vocal fold surgery (VFS) with phonosurgical intent. Both the pathology and the therapeutic aim define VFS: (1). vocal fold lesions that impair vibratory movements require excision (e.g., vocal fold nodule), incision and suction (e.g., Reinke's edema), dissection and/or augmentation (e.g., sulcus-vergeture), coagulation or vapo

Disc related and non-disc related disorders of the thoracic spine.

Different anatomical structures and pathophysiological functions can be responsible for lumbar pain, each producing a distinctive clinical profile. Pain can arise from the intervertebral disc, either acutely as a primary disc related disorder, or as result of the degradation associated with chronic internal disc disruption. In either case, greatest pain provocation will be associated with movements and functions in the sagittal plane. Lumbar pain can also arise from afflictions within the zygapophyseal joint mechanism, as a result of synovitis or chondropathy. Either of these conditions will p

Motor persistence of orbicularis oculi muscle in eyelid-opening disorders.

We describe clinical and EMG findings in three patients with an inability to reopen the eyes after voluntary closure of the eyelids. Synchronous EMG recording from the levator palpebrae (LP) and orbicularis oculi (OrbOc) muscles revealed that after voluntary closure of the eyelids and upon the command to open the eyes, all three patients were unable to inhibit the "voluntary" contraction of the OrbOc muscles, while on clinical examination there was no evidence of ongoing OrbOc muscle contraction. This "motor persistence" was restricted predominantly to the pretarsal portion of the OrbOc. In on

Vestibular function and sensory interaction in space flight.

The vestibular system and vestibulo-visual interaction were examined in 11 astronauts by the electrooculographic (EOG) method during short- and long-term flights on days 2, 3, 5, 9, 22, 50, 164, and 169 (experiments OPTOKINES and LABYRINTH). In space (flight days 2 and 3), they showed enhanced spontaneous vertical nystagmus, and disorders of tracking of vertical and diagonal movements of the stimulus which improved after active head movements. Early increasing of the reactivity of the cupulo-endolymphatic system (flight days 2-3) was replaced after 5 days of flight with a reduction of the vest

[Internuclear ophthalmoplegia and associated oculomotor syndrome (author's transl)].

Fifteen patients with internuclear ophthalmoplegia (INO) were examined by simultaneous electro-oculography of each eye. In 11 patients the INO was bilateral and in 4 patients unilateral. In 6 of the bilateral cases, the INO was asymmetric. A classic presentation of the INO was shown by only 2 patients. The rest had additional disorders of eye mobility (INO plus). The high percentage of additional affections, which were not often clinically apparent, consisted of slowing of abduction (45%), impairment of vertical movements (80%) and convergence (30%), and dysmetric overshoot of the abducing eye

MR Imaging of the Elbow.

Almost all movements of the upper extremities during routine daily life and many athletic activities require a painless and stable elbow joint. Studying the elbow is a diagnostic challenge because of its complex anatomy. MR imaging is an extremely important tool in the evaluation of common elbow disorders, the spectrum of which ranges from obvious acute lesions to chronic overuse injuries whose imaging manifestations can be subtle.

Progress in research on Tourette syndrome.

Tourette syndrome (TS) is a heritable neuropsychiatric disorder commonly complicated by obsessions and compulsions, but defined by frequent unwanted movements (motor tics) and vocalizations (phonic tics) that develop in childhood or adolescence. In recent years, research on TS has progressed rapidly on several fronts. Inspired by the Fifth International Scientific Symposium on Tourette Syndrome, the articles in this special issue review advances in the phenomenology, epidemiology, genetics, pathophysiology, and treatment of TS.

Sleep, stroke and poststroke.

There is abundant and still evolving scientific clinical evidence linking sleep apnea with stroke risk factors, cardiovascular disease, and stroke. Sleep apnea is a modifiable risk factor and efforts to control this condition should be pursued vigorously, particularly in patients at risk of vascular disease. Emerging evidence has also linked other sleep disorders, such as periodic limb movements of sleep and restless legs syndrome, with vascular risk.

Tardive dystonia of the larynx in a quadriplegic patient: an unusual cause of stridor.

We describe a case of a quadriplegic brain-damaged man with severe upper airway obstruction. A diagnosis of tardive dystonia affecting the vocal folds was made and confirmed by flexible nasopharyngoscopy. Emergency tracheotomy was required. The tardive movement disorder resolved with discontinuation of thioridazine. This case serves to heighten awareness of potentially serious airway complications associated with the use of anti-psychotics and anti-emetics particularly in those with organic brain disease.

Acquired cervical spine impairment in young adults with cerebral palsy.

Three patients with spastic cerebral palsy and no associated movement disorder--each of whom presented with loss of functional skills and delay in the definitive diagnosis of cervical myelopathy--are reported, in order to increase awareness of the possibility of cervical spine pathology in these adults. The possibility of myelopathy should be investigated when considering the etiology of functional deterioration. A functional neurological examination for all multiply disabled individuals is proposed as a reference for future comparison.
