Is population structure sufficient to generate area-level inequalities in influenza rates? An examination using agent-based models.

BACKGROUND: In New Haven County, CT (NHC), influenza hospitalization rates have been shown to increase with census tract poverty in multiple influenza seasons. Though multiple factors have been hypothesized to cause these inequalities, including population structure, differential vaccine uptake, and differential access to healthcare, the impact of each in generating observed inequalities remains unknown. We can design interventions targeting factors with the greatest explanatory power if we quantify the proportion of observed inequalities that hypothesized factors are able to generate. Here, w

Comparing the effectiveness, cost-effectiveness and implementation of low-dose oral modified release morphine in people with chronic breathlessness: a synopsis of a RCT and process evaluation.

BACKGROUND: Chronic breathlessness is common and disabling in chronic medical conditions. There is good rationale to use opioids from neuroscience and exercise laboratory studies. OBJECTIVES: We assessed the 56-day effectiveness, cost-consequences and safety of long-acting oral morphine for breathlessness in people with cardio-respiratory diseases or cancer. METHODS: We conducted a parallel-group, randomised, placebo-controlled trial in 11 UK outpatient services, comparing 10-20 mg daily oral morphine with placebo (and blinded laxative) in people with moderate to severe breathlessness, with

One-year all-cause mortality and comorbidity predictors in 14,975 adults with PCR-confirmed COVID-19: a retrospective Turkish cohort study.

BACKGROUND: Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), produces outcomes ranging from mild illness to death. Although in-hospital prognostic factors are well described, long-term mortality data are scarce, particularly from middle-income countries with relatively young populations. We therefore quantified one-year all-cause mortality and identified independent predictors in a large Turkish cohort support follow-up planning and prevention for high-risk groups. METHODS: We retrospectively included all adults (≥18 years) with poly

Cross-system patterns of physical, psychological and cognitive multimorbidity and 9-year all-cause mortality among middle-aged and older Chinese adults: a prospective cohort study from the China Health and Retirement Longitudinal Study.

BACKGROUND: Cross-system multimorbidity-the co-occurrence of physical, psychological and cognitive disorders-is increasingly recognised as a defining feature of population ageing, yet its relationship with long-term mortality has rarely been examined at the level of specific combinations. Most studies have used chronic-disease counts or binary indicators that obscure cross-system patterning. We assessed the association of cross-system multimorbidity with 9-year all-cause mortality and identified high-risk patterns in a national sample of middle-aged and older Chinese adults. METHODS: We analy

Chronic low-grade inflammation drives skeletal aging and neurocognitive decline: inflammaging as a central hub coupling bone-brain aging.

BACKGROUND: Global population aging has driven a marked rise in the co-prevalence of osteoporosis and cognitive impairment, and a bidirectional epidemiological association between the two conditions is now supported by multiple meta-analyses. The shared biological mechanisms underlying this comorbidity, however, remain incompletely defined, and the two disorders continue to be managed within largely separate clinical disciplines. AIM AND SCOPE: This review consolidates existing evidence into an integrative immunopathological framework in which inflammaging-the chronic, low-grade, sterile syst

Detecting and Predicting Cognitive Decline in Individuals with Traumatic Brain Injury: A Longitudinal Telephone-Based Study.

Traumatic brain injuries (TBIs) can lead to long-lasting cognitive impairments, and some survivors experience cognitive decline post-recovery. Early detection of decline is important for care planning, and understanding risk factors for decline can elucidate targets for prevention. While neuropsychological testing is the gold standard approach to characterizing cognitive function, there is a need for brief, scalable tools that are capable of detecting clinically significant changes in post-TBI cognition. This study examines whether a clinically significant change can be detected using the Brie

Is There Any Correlation between Baseline Serum Cortisol Levels and Disease Severity in PCR-Positive COVID-19 Patients with and without Diabetes Mellitus?

Background: COVID-19 has caused a pandemic and is associated with significant mortality. The pathophysiology of COVID-19, affecting many organs and systems, is still being investigated. The hypothalamus, pituitary gland, and possibly adrenal glands are the targets of SARS-CoV-2 because of its angiotensin-converting enzyme 2 (ACE2) and transmembrane serine protease 2 (TMPRSS2) receptors expression. Hypocortisolemia can be seen in the postinfection period. COVID-19 infection tends to be severe in diabetic patients due to immune dysfunction. In this study, our aim was to investigate the relations

Zinc Salts Block Hepatitis E Virus Replication by Inhibiting the Activity of Viral RNA-Dependent RNA Polymerase.

Hepatitis E virus (HEV) causes an acute, self-limiting hepatitis in healthy individuals and leads to chronic disease in immunocompromised individuals. HEV infection in pregnant women results in a more severe outcome, with the mortality rate going up to 30%. Though the virus usually causes sporadic infection, epidemics have been reported in developing and resource-starved countries. No specific antiviral exists against HEV. A combination of interferon and ribavirin therapy has been used to control the disease with some success. Zinc is an essential micronutrient that plays crucial roles in mult

Prevalence of Pain Diagnoses and Burden of Pain Among Active Duty Soldiers, FY2012.

INTRODUCTION: Soldiers are at risk for acute and chronic pain due to the mental and physical challenges of military duties and ongoing training for force readiness. With the burden of pain on any individual attributable across pain sources, a broad perspective that goes beyond prior characterizations of pain is important. We aim to further the understanding of pain's effects among non-deployed active duty soldiers and the Military Health System (MHS), by describing prevalence of 10 painful conditions, reported pain levels, duration of pain and impact of pain on military duty limitations. [What do we know about compassion focused therapy for anxiety and depressive symptoms: A scoping review].

OBJECTIVE: Anxiety and depressive disorders are among the most common psychiatric conditions and have a high comorbidity rate. While cognitive behavioral therapy (CBT) remains the gold standard psychotherapeutic treatment, its protocols which focus on a specific disorder have limitations when faced with clinical complexity. Transdiagnostic approaches aim to overcome these constraints by targeting processes common to different mental disorders. Compassion-Focused Therapy (CFT) is part of this approach. It is particularly aimed at patients with overwhelming shame and marked self-criticism, often

AI in Psoriatic Disease: Scoping Review.

BACKGROUND: Artificial intelligence (AI) has many applications in numerous medical fields, including dermatology. Although the majority of AI studies in dermatology focus on skin cancer, there is growing interest in the applicability of AI models in inflammatory diseases, such as psoriasis. Psoriatic disease is a chronic, inflammatory, immune-mediated systemic condition with multiple comorbidities and a significant impact on patients' quality of life. Advanced treatments, including biologics and small molecules, have transformed the management of psoriatic disease. Nevertheless, there are stil

Heparanome-Mediated Rescue of Oligodendrocyte Progenitor Quiescence following Inflammatory Demyelination.

The proinflammatory cytokine IFN-γ, which is chronically elevated in multiple sclerosis, induces pathologic quiescence in human oligodendrocyte progenitor cells (OPCs) via upregulation of the transcription factor PRRX1. In this study using animals of both sexes, we investigated the role of heparan sulfate proteoglycans in the modulation of IFN-γ signaling following demyelination. We found that IFN-γ profoundly impaired OPC proliferation and recruitment following adult spinal cord demyelination. IFN-γ-induced quiescence was mediated by direct signaling in OPCs as conditional genetic ablation of

Prevalence and risk factors for chronic kidney disease in Indonesia: An analysis of the National Basic Health Survey 2018.

BACKGROUND: The prevalence of chronic kidney disease (CKD) in Indonesia is rising, but the exact extent of the burden of CKD in Indonesia is unknown. To design a screening program for individuals at high-risk, more knowledge is required regarding the prevalence and risk factors of CKD in Indonesia. The latter could have a big impact on the prevention and management of patients with CKD in Indonesia. METHODS: For this purpose, we analysed data from The National Basic Health Survey 2018 (Riset Kesehatan Dasar, Riskesdas 2018), a descriptive cross-sectional study in 34 provinces, 416 districts a

Prefrontal-centered cortical alterations and network dysconnectivity in cocaine use disorder.

BACKGROUND: Cocaine use disorder (CUD) is a chronic, relapsing condition with substantial clinical burden, yet its neurobiological basis remains incompletely understood, particularly in the socially vulnerable populations most affected in real-world clinical settings. Existing neuroimaging evidence is fragmented across imaging modalities and derived largely from samples that underrepresent sustained adversity and polysubstance use. We used multimodal MRI to characterize structural and functional brain alterations in a clinically representative CUD sample. METHODS: Seventy-two individuals with

Incidence of spontaneous fungal peritonitis in patients with liver cirrhosis in a Mexico City population.

BACKGROUND: Individuals with liver cirrhosis (LC) are likely to experience multiple infectious processes due to the immune dysfunction caused by the disease. Our hypothesis is that this group of patients is predisposed to fungal infections. To date, the incidence of spontaneous fungal peritonitis (SFP) has not been determined in Mexico; this endeavor is of great importance because many patients may be suffering from this condition without receiving targeted treatment, which may increase mortality. AIM: To report the incidence of SFP in patients presenting with decompensated LC with ascites.

Physical activity phenotypes in endometriosis using unsupervised learning via functional mixture models.

BACKGROUND: Endometriosis is a chronic condition associated with severe pelvic pain, dysmenorrhea, infertility, and worsening quality of life. Regular physical activity (PA) is effective for pain management and reducing chronic disease symptoms, yet individuals with endometriosis are more likely to be insufficiently active. This study investigated latent profiles of daily PA trajectories in this population via clustering. METHODS: We analyzed 171 adults (4,795 person-level days) with a confirmed diagnosis of endometriosis enrolled in the All of Us Research Program. PA data were collected from
