The prediction of clinical outcome during antifungal therapy is an issue of paramount importance in clinical research, because no consistently reliable parameters are available. Ninety-seven environmental and four clinical isolates of Cryptococcus neoformans from several localities in extreme southern Italy were serotyped. All proved to be Cr. neoformans var. neoformans serotype A. The homogeneity of the serotypes suggests that geographical climatological conditions may play a role in Cr. neoformans serotype diffusion. It was furthermore revealed that, unlike the data reported for the serotypes of Cr. neoformans in mainland Italy, Cr. neoformans serotype D was not present in the examined sites in extreme southern Italy.

Forty Cryptococcus neoformans strains isolated from cerebral spinal fluid specimens collected from 39 patients were included in the study. The MICs for fluconacole were determined by YNB macrodilution test, microdilution tests using both RPMI1640 and YNB medium and E-tests on solidified RPMI1640 medium, Casitone and YNB agar. In comparison with the reference macrodilution method NCCLS M27-P both the microdilution as well as the E-test techniques can be used for fluconacole susceptibility testing of Cr. neoformans.

A fatal case of craniofacial zygomycosis caused by Apophysomyces elegans in a 52-year-old man was diagnosed by the presence of broad aseptate, branched hyaline hyphae in tissue from paranasal sinuses and surrounding areas, and isolation of the fungus from the same tissue. The patient suffered from idiopathic myelofibrosis as underlying disease, he was thrombocytopenic and was mildly hyperglycaemic. The infection represents the second case of craniofacial zygomycosis due to A. elegans.

Cryptococcus neoformans strains isolated from 207 HIV positive and HIV negative patients hospitalized in Northern Italy were serotyped by slide agglutination. Cladistic analysis of partial 26S rRNA sequences was used to estimate evolutionary distances among species and varieties of the dimorphic onygenalean genera Blastomyces, Coccidioides, Emmonsia, Histoplasma and Paracoccidioides. Despite its common adverse effects intravenous (i.v.) amphotericin B is an indispensable antifungal drug in childhood oncology. We report here on three cases of painful cyanotic Raynaud's phenomenon after i.v. administration or inhalation of amphotericin B. A liposomal i.v. preparation of amphotericin B was well tolerated by the infants. Spasms of peripheral vessels mediated by thromboxane A2 could be responsible for the Raynaud's phenomenon. Hence, inhibitors of prostaglandin synthesis are suggested for therapy.

We successfully treated a feline case of cryptococcosis with itraconazole (ITZ) at a lower dosage. The patient was a 2-year-old castrated male Abyssinian cat weighing 4.1 kg and with two masses on the head. Clinical signs were sneezing and nasal discharge. The plasma cryptococcal antigen titer measured by the latex agglutination test was proved to be high (512). Trichophyton tonsurans is an anthropophilic dermatophyte that is predominantly responsible in man for tinea capitis. Although this fungus has a world-wide distribution, most reports of the occurrence of Trichophyton tonsurans infections originate from the USA, Latin America and some European countries. A 31-year-old African woman, formerly a resident of Zaire, with tinea corporis caused by Trichophyton tonsurans infection is described here.

Paracoccidioidomycosis is a systemic fungal infection caused by the thermally dimorphic fungus Paracoccidioides brasiliensis. The authors present a case of a 49-year-old man who developed verrucous lesions on nasal mucosa and on genital localization (inguinal and scrotal), without signs of systemic disease. Direct mycological examination, culture on Sabouraud glucose medium, and biopsy were positive for P. brasiliensis. The interest of this case is the genital localization which is uncommon in this infection. Moreover, there are few cases described in the literature.

We describe an HIV-positive patient suffering from cutaneous cryptococcosis who was a former intravenous drug abuser and who presented a nodulo-ulcerated lesion on the back of his left hand. This was caused 4 months previously when the patient was injured by a pigeon nesting in an abandoned house. After fluconazole treatment for 4 months, no other visceral infections were recorded 18 months after complete healing of the lesion.

A forty-three-year old patient suspected of having a bone tumor in his left femur, was submitted to the hospital. However, osteomyelitis caused by Cryptococcus neoformans has been demonstrated by culture. The history of the patient revealed a long-term therapy with corticosteroids due to sarcoidosis. The osteomyelitis was treated with fluconazole (200 mg daily p.o.) for three months. Under this therapy the infection resolved.

A commercially available antigen assay (Pastorex Aspergillus) was used to detect Aspergillus antigen in serial bronchoalveolar lavage (BAL) fluids and sera of patients undergoing bone marrow transplantation (six patients with autopsyproven aspergillosis and 10 control patients without evidence of fungal infection).
