Aggressive surgical treatment of periprosthetic femur fractures can reduce mortality: comparison of open reduction and internal fixation versus a modular prosthesis nail.

OBJECTIVES: The purpose of this study was to determine if immediate full weightbearing after surgical treatment for periprosthetic femur fractures can decrease perioperative and total mortality. DESIGN: Retrospective review. SETTING: Level II trauma center. PATIENTS: Fifty-two consecutive patients with a periprosthetic femur fracture during a 16-year time period. INTERVENTION: Comparison of open reduction and internal fixation with a plate (non- or partial postoperative weightbearing) versus stem exchange to a modular prosthesis nail (immediate full postoperative weightbearing). MAIN OUTC

Trapped in keratin; a comparison of dermatophyte detection in nail, skin and hair samples directly from clinical samples using culture and real-time PCR.

Traditionally, laboratory detection and identification of dermatophytes consists of culture and microscopy which yields results within approximately 2-6 weeks. In 2007 our medical microbiological diagnostic laboratory implemented a molecular method for the detection of dermatophytes. A real-time PCR assay was developed which simultaneously detects and identifies the most prevalent dermatophytes directly in nail, skin and hair samples and has a turnaround time of less than two days. For 1437 clinical samples, received by our diagnostic laboratory, we compared the results obtained from both cult

A comparative evaluation of combination therapy of fluconazole 1% and urea 40% compared with fluconazole 1% alone in a nail lacquer for treatment of onychomycosis: therapeutic trial.

This is a randomized, double-blind study enrolling 70 patients with onychomycosis of the finger and toenails. Clinical and mycological efficacies as well as measures of safety were assessed monthly for a maximum of 6 months of treatment. The treatment regimens were: fluconazole 1% and fluconazole 1% with urea 40%. These results indicated topical treatment of onychomycosis with a combination of fluconazole 1% and urea 40% was more effective (82.8%) than fluconazole 1% (62.8%) nail lacquer alone in treatment of dermatophytic onychomycosis. Fluconazole was well tolerated and side effects were neg…

Early results of reverse less invasive stabilization system plating in treating elderly intertrochanteric fractures: a prospective study compared to proximal femoral nail.

BACKGROUND: Intertrochanteric femur fracture is common in elderly population. Though multiple treatment options are available, the choice of implant remains controversial. The reverse less invasive stabilization system (LISS) plating was introduced for treatment of a patient with ipsilateral intertrochanteric and midshaft femoral fractures. The aim of this research was to compare such technique to intramedullary nailing (proximal femoral nail, PFN) for intertrochanteric fractures. METHODS: Fifty-six patients with an age of at least sixty-five years and an AO/OTA type-A1 or A2 fractures were i

Anatomic study of the axillary nerve in a Chinese cadaveric population: correlation of the course of the nerve with proximal humeral fixation with intramedullary nail or external skeletal fixation.

INTRODUCTION: The axillary nerve can be injured during external skeletal fixation with Schanz screws or proximal locking screws of intramedullary nails. Being aware of the axillary nerve's anatomic relationship to the proximal humerus is vital for avoiding complications. METHODS: We investigated the relationship of the axillary nerve to surrounding bony landmarks by studying 88 axillary nerves in 44 embalmed cadaveric adult Chinese males. These measurements were then compared with the results from a similar study among Caucasians using the same reference points. RESULTS: We identified three …

Finite element analysis of a subtrochanteric fractured femur with dynamic hip screw, dynamic condylar screw, and proximal femur nail implants--a comparative study.

Selection of the correct type of implant for fracture fixation has become a very interesting problem in the orthopaedic community. The present work studies the biomechanical behaviour of the femur with three different implant configurations for simple transverse subtrochanteric fracture and the intact femur using finite element analysis. The implants considered in this study are as follows: dynamic hip screw (DHS), dynamic condylar screw (DCS), and proximal femur nail (PFN). The modelling software Unigraphics and finite element simulation software ANSYS are used for the present analysis. The t

[How many severely injured multiple-trauma patients can benefit from the biomechanical advantage of early mobilization following femoral intramedullary nailing?].

The benefit of early operative stabilization of femoral fractures is established in patients with multiple injuries. In the last few years the unreamed femoral nail is favoured for internal fixation of femoral fractures despite pathophysiological concerns. The foremost advantage of femoral nails compared with plate fixation is the possibility of early full weight bearing. The aim of this retrospective study was to investigate, under consideration of the severity of injury, the extent of injury, and the clinical course, if multiple injured patients with concomitant femoral fractures benefit fro…

Autosomal dominant postaxial polydactyly, nail dystrophy, and dental abnormalities map to chromosome 4p16, in the region containing the Ellis-van Creveld syndrome locus.

We have studied a four-generation family with features of Weyers acrofacial dysostosis, in which the proband has a more severe phenotype, resembling Ellis-van Creveld syndrome. Weyers acrofacial dysostosis is an autosomal dominant condition with dental anomalies, nail dystrophy, postaxial polydactyly, and mild short stature. Ellis-van Creveld syndrome is a similar condition, with autosomal recessive inheritance and the additional features of disproportionate dwarfism, thoracic dysplasia, and congenital heart disease. Linkage and haplotype analysis determined that the disease locus in this pedi

Healing smarter: A systematic review and meta-analysis of bioresorbable implants for paediatric forearm fractures.

PURPOSE: Paediatric forearm fractures are among the most common childhood injuries, with diaphyseal fractures often requiring surgical management due to their instability and poorer remodelling potential. Traditional methods, such as elastic stable intramedullary nailing, are effective but require secondary procedures for implant removal, increasing healthcare burdens. Bioabsorbable intramedullary nails offer an alternative, eliminating the need for implant removal. This systematic review and meta-analysis evaluates the efficacy and safety of bioabsorbable intramedullary nails compared to conv…

Leg lengthening using intramedullay skeletal kinetic distractor: results of 57 consecutive applications.

BACKGROUND: Mechanically activated intramedullary lengthening nails are advantageous over external fixator. However, difficulties with the control of the distraction rate are the main drawbacks, which may in turn cause insufficient bone regenerate. PATIENTS AND METHODS: A total of 57 lengthening procedures were performed using intramedullary skeletal kinetic distractor (ISKD) nail in 53 patients (femoral = 45 and tibial = 12). Average length gain was 4.3 ± 1.6 cm. The cause of shortening was post-traumatic (n = 33), congenital (n = 20), post-tumour resection (n = 1), cosmetic femoral lengthen…

Whole exome sequencing identifies a novel dominant missense mutation underlying leukonychia in a Pakistani family.

Hereditary leukonychia (also known as porcelain nails or white nails) is a genetic disorder. It may exist as an isolated feature or associated with other cutaneous or systemic disorders. Although a number of genes have been described to cause leukonychia, still the underlying genetic etiologies of many cases remain unknown. Here, we report a Pakistani family presenting leukonychia and koilonychia nails in mother and five of her kids. All the affected individuals had white to pale nails in appearance exhibiting complete and partial leukonychia, respectively. Similarly, nails of finger and toe a…

A 35-year-old female Central Africa Republican refugee with an uneventful medical and family past histories presented to a hospital with a 3-year history of a progressive painful outgrowth of the five nails of the left hand.

Onychogryphosis is a dermatological disease characterized by hypertrophy, opaque or yellow-brown thickening, gross hyperkeratosis, elongation, and excessive curving of the nail plate, especially that of the big toe of the foot. The resultant nail deformity is often termed "ram's horn nail" or "claw-hand" deformity. Affected patients suffer mainly from pain, and cosmetic concerns. This disorder can be acquired or congenital. Herein, we present the case of an usual form of acquired onychogryphosis affecting all the five nail plates of the left hand in a bit to raise awareness of demaytologist an

Treatment of Pediatric Femoral Shaft Fractures.

Diaphyseal femur fractures are common in pediatric orthopaedic settings. A patient-specific treatment plan incorporates several factors, including age, weight, fracture pattern, associated injuries, and social considerations. Nonaccidental trauma should be considered in children younger than 3 years. In general, young children are treated with noninvasive immobilization (Pavlik harness or early hip spica casting) while school-aged children are treated with internal fixation. Internal fixation options include flexible intramedullary nails, rigid locked intramedullary nails, and plate osteosynth…

Changes in PFNA nail position do not influence bone healing and femoral head varus in patients with stable intertrochanteric fractures: a comprehensive clinical and biomechanical study.

BACKGROUND: Nail positioning is highly adjustable during the proximal femoral nail anti-rotation (PFNA) procedure for treating intertrochanteric fractures. Our published study demonstrates that changes in nail position can significantly affect local stress distribution, and ventral side nail insertion may increase the risk of varus deformity in the femoral head, particularly in unstable fracture types. However, adjusting the guidewire position tends to prolong operation time and increase blood loss. The necessity of this adjustment in stable fracture cases remains to be clarified. METHODS: Th

Minimally invasive plate osteosynthesis, open reduction and plate fixation, and intramedullary nailing for humeral shaft fractures: A network meta-analysis of randomized controlled trials.

BACKGROUND: The optimal surgical technique for humeral shaft fractures remains debated, and earlier syntheses often pooled all plate constructs, disregarding the distinction between open reduction and plate fixation (ORPF) and minimally invasive plate osteosynthesis (MIPO). This network meta-analysis compared ORPF, MIPO, and intramedullary nailing (IMN), focusing on iatrogenic radial nerve injury. METHODS: We searched PubMed/MEDLINE, Embase, Cochrane CENTRAL, and Scopus from inception through December 2024, with the PubMed search updated through January 2026, following the PRISMA network meta
