Current Innovations in Peripheral Nerve Stimulation.

Peripheral nerve stimulation has been used in the treatment of several chronic pain conditions including pain due to peripheral nerve dysfunctions, complex regional pain syndrome, and cranial neuralgias. It has been shown to be effective for chronic, intractable pain that is refractory to conventional therapies such as physical therapy, medications, transcutaneous electrical stimulations, and nerve blocks. Recently, a new generation of peripheral nerve stimulation devices has been developed; these allow external pulse generators to transmit impulses wirelessly to the implanted electrode, and t…

Facial palsy and atrial fibrillation: a special case of Ramsay Hunt syndrome.

Ramsay Hunt syndrome (herpes zoster oticus) is a rare complication of latent Varicella Zoster virus infection. It can be complicated by permanent hearing loss, loss of taste and postherpetic neuralgia. Although Ramsay Hunt syndrome most prominently involves the facial nerve, a number of other cranial nerves can be involved such as the vestibulocochlear, glossopharyngeal and the vagus nerve. We report on a case of Ramsay Hunt syndrome with cranial polyneuritis complicated by atrial fibrillation. Vagal involvement as evidenced by physical examination and MRI findings was present in our patient. …

Black hairy tongue with a fixed dose combination of olanzapine and fluoxetine.

Black hairy tongue (BHT) is a benign disease characterized by elongated filiform lingual papillae, with a carpet-like appearance of the dorsum of the tongue. It is has been reported to occur with a prevalence ranging from 0.6% to 11.3%. Although its etiology is not fully understood, BHT may be triggered by smoking, excessive coffee or black tea drinking, poor oral hygiene, trigeminal neuralgia, general debilitation, dry mouth as well as certain drugs. We present here a case of a patient with psychosis, depression, and benign prostatic hyperplasia, who developed BHT following treatment with a f…

Clostridium difficile infection induced by pregabalin-associated agranulocytosis.

A 33-year-old man who had recently undergone surgery for cervical spondylotic myelopathy was prescribed pregabalin for neuralgia, and the dose was increased to 600 mg/day during hospitalization. However, the patient was diagnosed with a Clostridium difficile infection on day 34 after admission. A complete blood count showed agranulocytosis (neutrophil count: 105/μL). We did not observe any changes in vital signs, a relative increase in band cells, or intestinal edema. The patient's agranulocytosis resolved after withdrawing pregabalin. This is the first reported case of agranulocytosis associa…

The association of hypno-anesthesia and conventional anesthesia in a patient with multiple allergies at risk of anaphylactic shock.

A male patient needed surgery for the ablation of 4 impacted maxillary molars that prevented chewing and had contributed to progressively worsening trigeminal neuralgia. Two previous anesthetic procedures led to episodes of severe anaphylactic shock with the need for a prolonged stay in the ICU. Hypnotic anesthesia was therefore selected as a safer option for this patient. After 4 preparative sessions, on the day of surgery, the hypnotist provided an induction followed by suggestions for mouth and face anesthesia. Intubation occurred following the introduction of remifentanil and sevoflurane. …

Femoral nerve compression after migration of bone cement to the groin after hip arthroplasty.

Although often a benign complication of total hip arthroplasty, cement extrusion can cause nerve, vessel, and organ compression. We report the case of a 70-year-old male patient in whom an extruded cement mass migrated anteriorly and compressed the femoral nerve and impinged on the femoral artery producing acute, severe groin pain with neuralgia 9 years postoperatively. Paresthesia of the anterior and medial thigh was found on examination. Radiographic, ultrasound, and computed tomographic studies confirmed a 6 × 1.5-cm mass of bone cement in the right groin compressing the femoral nerve that …

Trigeminal trophic syndrome with features of oral CMV disease.

Trigeminal trophic syndrome (TTS) is an exceedingly rare complication following injury to the trigeminal ganglion, characterized by painless ulcerations, which has only rarely been reported with intraoral features. We present a patient with multiple intraoral ulcerations of the right buccal and alveolar mucosa that had previously been treated with nerve ablation therapy for trigeminal neuralgia. Positive immunohistochemistry staining of a biopsy specimen for cytomegalovirus suggested a viral etiology; however, lesions persisted despite antiviral therapy, and immunohistochemistry was negative o…

Microvascular decompression as a treatment for cranial nerve hyperactive dysfunction--a critical view.

Neurovascular compression has been postulated as a probable mechanism for a large number of cranial nerve syndromes, with trigeminal neuralgia (TGN) as the prime example. Microvascular decompression (MVD) is often cited as the procedure of choice for treatment of medically refractory TGN. Arguments against these assumptions are: MRA studies indicate that vascular contact with the trigeminal nerve is present in most healthy individuals. Treatment results of MVD in multiple sclerosis patients with TGN are almost as good (at least in the short term) as in idiopathic cases. MVD is reported to prov…

[The suffering of Heinrich Heine].

The analysis of clinical neurological symptoms on the basis of available letters and other documents confirms beyond a doubt the long standing diagnosis of neurosyphilis in the form of chronic meningitis with cranial polyneuritis and wide spread polyradiculitis, formerly called Lues cerebro-spinalis. The symptoms which recurred over 25 years include multiple severe paralysis of cranial nerves, dramatic radicular neuralgias and radicular paralysis in addition to a partial transverse lesion of the spinal cord and terminal coughing and regurgation fits in the absence of symptoms in the cerebrum a…

[Glomus tumor of the thumb].

The article presents a case of 42-year-old patient with Recklinghausen disease treated in the outpatient clinic for 5 years for hyperaesthesia of a thumb. On physical examination the thumb was painful but smooth, soft with no features of infection. Neuralgia and Raynaud syndrome were suspected. Additional investigations did not show significant abnormalities, however, successive forms of conservative treatment (including denervation of the thumb) were not successful. Instant, persistent pain caused by even a slight touch or thermal changes suggested glomus tumor of the thumb. Physical examinat…

Nitric oxide involvement in the trigeminal hyperalgesia in diabetic rats.

Trigeminal hyperalgesia frequently appears in diabetic neuralgia altering the transmission of orofacial sensory information. This study was designed to explore the effects of trigeminal hyperalgesia in streptozotocin-induced diabetes monitoring the expression of nitric oxide synthase in the trigeminal ganglion cells. The threshold to heat noxious stimuli decreased in diabetic animals. The number of NADPH-diaphorase (NADPH-d)-positive neurons significantly decreased in the diabetic rats compared with controls. Insulin treatment prevented the decreased nociceptive threshold and reduction of the …

Olfactory groove meningiomas.

Olfactory groove meningioma is a rare clinical entity. The principal symptoms associated with olfactory groove meningiomas are anosmia and headache which lead the patient to the ENT specialist. Frontal sinusitis, migraine and neuralgia are most frequent incorrect diagnoses. This retrospective study of 13 consecutive patients who underwent excision of Olfactory Groove meningiomas in the North Staffordshire Royal Infirmary, Department of Neurosurgery over a 20-year period was undertaken to study the presenting features, duration of symptoms, results of surgery, and long-term follow-up. Symptoms …

Choristoma of the intracranial maxillary nerve in a child. Case report.

This report describes what the authors believe to be the first reported case of choristoma of the intracranial maxillary nerve. This 12-year-old girl presented with a 5-year history of severe isolated left-sided trigeminal neuralgia. Computerized tomography and magnetic resonance imaging revealed a mass below the anterior portion of the left cavernous sinus, enlarging the foramen rotundum. Total resection was achieved via a pterional extradural approach. Histological examination revealed a choristoma composed of smooth-muscle fibers. The histogenesis of these tumors when they develop in a nerv…
