Not gone and not forgotten: a spouse's experience of late-stage Alzheimer's disease.

1. Late-stage caregiving is different from earlier stages, is difficult, and requires an empathic response from staff. 2. Ethical decision-making, supportive care, and the inevitability of death are important late-stage caregiver topics. Staff should understand the concept of palliative care. 3. Current educational programs and family support groups should be assessed for attention to late-stage caregiver issues. Further research is needed on helping families prepare for and cope with late stage caregiving.

The challenge of performing three heart transplantations in one day.

Heart transplantation procedures, which present a number of challenges not generally seen in perioperative nursing practice, provide important knowledge for perioperative nurses because they demonstrate the importance of organization, communication, collaboration, and resourcefulness among surgical team members. On Friday, Aug 13, 1993, the Virginia Heart Center at Fairfax Hospital, Falls Church, implanted donor hearts in three adult recipients in a single day. The day was even more challenging because of elective coronary artery bypass graft procedures performed on three other patients. Succe

Closed head injuries in children: is hospital admission always necessary?

The objective of this study was to identify a group of patients with mild closed head injury, lack of other significant trauma, and normal head computerized tomograph (CT), who could be safely observed at home by a reliable caretaker. Data were from a retrospective chart review of pediatric emergency department (PED) and hospital course of an urban university children's hospital. The pediatric trauma registry was used to identify patients one to 17 years old seen in the PED with closed-head injury and normal head CT between June 1991 and August 1992. A total of 746 patients with heads injury w

Risk factors and consequences of bacteriuria in non-catheterized nursing home residents.

BACKGROUND: There has been disagreement over the significance of bacteriuria in nursing home residents. METHODS: During an 18-month period, the risks and consequences of bacteriuria (BU) in 195 residents of a skilled nursing facility without indwelling catheters were examined. Clinical and epidemiologic data and urine for culture were collected every 2 weeks to identify risk factors, symptoms, and occurrences of BU. A mean of 23 cultures per resident was collected. RESULTS: Forty-three percent of the study population (35% of men; 47% of women) had "persistent BU" defined as > 10(5) cfu/ml of

Tourette's syndrome, Part II: Contemporary approaches to assessment and treatment.

Clinical assessment of a child with Tourette's syndrome (TS) includes a careful review of motor and phonic tics. In addition, commonly associated problems of such as obsessive-compulsive symptoms, or symptoms of attention deficit hyperactivity disorder (inattention, impulsiveness, and overactivity) should also be evaluated. Treatment almost always includes education of the child, family, and school personnel concerning the natural history and behavioral boundaries of the disorder. Other treatment interventions depend to a great extent on the primary source of impairment. This article, the seco

Pressure ulcer prevalence, incidence and associated risk factors in the community.

This study examined the prevalence and incidence of pressure ulcers as well as associated risk factors in the community. By using the Web of Causation and the Braden conceptual schema, barriers to effective interventions were identified and analyzed. The Web of Causation for pressure ulcer development includes socioeconomic factors and personal belief systems. It also includes the following risk factors: mobility, activity, moisture, nutrition, friction, shear, and altered sensory perception. There were 103 participants in this study. Data gathered by public health nurses included occurrence,

[Multi-infarct dementia in nursing home patients; more comorbidity and shorter life expectancy than in Alzheimer's disease].

A retrospective analysis of the medical charts of 117 patients (50 men and 67 women) with multi-infarct dementia took place. All patients admitted to the psychogeriatric nursing home 'Joachim en Anna' in Nijmegen between 1980 and 1989 were studied. The aim of the study was to obtain epidemiological information and to investigate the prevalence of comorbid conditions, prognosis and mortality. The results were compared with patients with Alzheimer's disease. The patients remained in the institute for 1.4 years and the mean total duration of the disease was 5.3 years. About twenty-five percent di

Pressure ulcer incidence and severity in a community hospital.

In a study of 1320 adult patients admitted to a community hospital, there was a pressure ulcer incidence rate of 8.4%, with 3.2% of pressure ulcers present on admission. Of the 190 ulcers found, 63% (n = 120) were hospital acquired and were less severe than those present on admission. There was an average of 1.6 ulcers per patient; 56.3% (n = 107) were Stage I ulcers and 36.3% (n = 69) were Stage II ulcers. The most frequent sites were coccyx-sacral area, heels, and elbows. Preventive measures used most frequently were turning, special mattresses, and special skin care. After a pressure ulcer

Superior vena caval obstruction in patients with gut failure receiving home parenteral nutrition.

Clinical suspicion and venographic conformation were used to diagnose 15 cases of superior vena caval obstruction (SVCO) in 107 home parenteral nutrition (HPN) patients over 379 cumulative years of HPN (3.9 cases/100 patient-years). Patients with SVCO had been on HPN a mean of 51.5 months and had used 6.2 (range 1-50) central catheters, including short- and long-term, before SVCO was diagnosed. The frequency of inflammatory bowel disease (IBD) with SVCO was approximately the same as that in our general HPN population. Positive blood cultures were present immediately preceding the diagnosis of

Teaching home care for ventilator-dependent patients: the patients' perception.

An increasing number of ventilator-dependent patients are being transferred from hospitals to their homes. Discharge teaching has been based on health professionals' perception of what patients need to know and how they can learn best. In this study we asked ventilator-dependent patients who were living at home (N = 13) to complete a questionnaire concerning their perceptions of ideal discharge teaching. Most respondents believed that teaching should occur (1) at the bedside, (2) with one or two family members, (3) with only the team member doing the teaching, (4) for a 30-minute session, (5)

Susceptibility to poliomyelitis, measles, mumps and rubella in university students.

A serological study of 433 university students in various health care professions revealed levels of susceptibility of 2-7% for measles, 0-4% for mumps, 6-17% for poliomyelitis and a particularly disquieting 13-23% for rubella. Representing a developed population with a supposedly high level of immunisation cover these levels of susceptibility indicate students' vulnerability to outbreaks caused by viruses coming from reservoirs in under-immunised developing populations in the country. Nevertheless, no evidence of occupational exposure between the first- and final-year students was revealed. I

Instruments for screening for depression and dementia in a long-term care facility.

The high prevalence of mental disorders such as depression and dementia in institutionalized elderly patients warrants screening for psychiatric diagnosis in patients newly admitted to long-term care facilities. The diagnostic accuracy of the Mini-Mental State Examination (MMSE) and the Geriatric Depression Scale (GDS) was evaluated against a standardized psychiatric interview. The MMSE was found to be 81% sensitive and 83% specific in screening for dementia using a previously established cutpoint of less than 24/30 points. Adjusting MMSE scores for physical disabilities precluding completion

Congenital dislocation of the hip in Western Australia. A comparison of neonatally and postneonatally diagnosed cases.

Speculation that neonatally diagnosed congenital dislocation of the hip (CDH) may have a different etiology from cases diagnosed in the postneonatal period has not been examined in Australia because insufficient data have been available. A population-based study of CDH in children up to the age of two years who were born in Western Australia (WA) in 1981, 1982, or 1983 is the subject of this report. Study material comprised cases of CDH from the WA Congenital Malformations Register and denominator data (all births in WA for 1981-1983) from the Health Department of WA. From this material, rates

Factors explaining the referral of the elderly to home nursing or home help.

Factors explaining the referral of the elderly to home nursing or home help or both were studied in a population (n = 1358) aged 60 years or over and living in the commune of Ahtäri, a semi-industrialized community in middle-western Finland, using social and health care registers and questionnaires in collecting data. The clients of home nursing and of combined home nursing and home help were predominantly women, whereas men were in the majority among those referred to home help. A stepwise regression analysis showed old age, living alone, low self-perceived health, low education and few visit
