Has There Been a Shift in Use of Subacute Rehabilitation Instead of Hospice Referral Since Immunotherapy Has Become Available?

PURPOSE: Immunotherapy has rapidly become the mainstream treatment of multiple cancer types. Since the first drug approval in 2011, we have noted a decline in referrals from inpatient oncology to hospice and an increase in referrals to subacute rehabilitation (SAR) facilities, possibly with the aim of getting strong enough for immunotherapy and other promising drugs. This study explores outcomes after discharge to SAR, including rates of cancer-directed therapy after SAR, overall survival, and hospice use. METHODS: We performed an electronic chart review of patients discharged from our inpati

Open Vs Minimally Invasive Adult Ureteral Reimplantation: Analysis of 30-day Outcomes in the National Surgical Quality Improvement Program (NSQIP) Database.

OBJECTIVE: To examine 30-day outcomes of robotic-assisted and pure laparoscopic ureteral reimplantation (LUR) vs open ureteral reimplantation (OUR) in adult patients for benign disease. METHODS: We identified adult patients undergoing LUR or OUR by urologists between 2006 and 2013 using the American College of Surgeons National Surgical Quality Improvement Program database, excluding those with concomitant partial cystectomy or ureterectomy. Multivariable regression modeling was used to assess for the independent association of minimally invasive surgery (MIS) with 30-day complications, reope

Improving Long-Term Outcomes After Sepsis.

Although acute survival from sepsis has improved in recent years, a large fraction of sepsis survivors experience poor long-term outcomes. In particular, sepsis survivors have high rates of weakness, cognitive impairment, hospital readmission, and late death. To improve long-term outcomes, in-hospital care should focus on early, effective treatment of sepsis; minimization of delirium, distress, and immobility; and preparing patients for hospital discharge. In the posthospital setting, medical care should focus on addressing new disability and preventing medical deterioration, providing a susta

Update in hospital medicine: studies likely to affect inpatient practice in 2011.

Dabigatran (Pradaxa) will likely start to replace warfarin (Coumadin) both to prevent stroke in patients with atrial fibrillation and to prevent recurrent venous thromboembolism. Using a checklist during insertion of central venous catheters can decrease the rate of catheter-related bloodstream infections in the intensive care unit. The overall survival rate of patients who undergo cardiopulmonary resuscitation in the intensive care unit is approximately 16%; the rate is lower in patients who are receiving pressor drugs and higher in those with ventricular tachycardia or ventricular fibrillati

Hepatitis at Home: Ambulatory management of severe immune checkpoint inhibitor related hepatitis.

BACKGROUND: Liver toxicity is common in patients treated with immune checkpoint inhibitor (ICI) therapy. Most guidelines advocate for the admission of patients with grade 3/4 ICI mediated hepatitis. Ambulatory emergency care forms a fundamental part of the strategy of trying to ensure safe and sustainable acute care services. METHODS: An ambulatory pathway for ICI mediated hepatitis (CTCAE grade 3/4) was commenced in March 2023. Patients with ICI mediated hepatitis with normal synthetic liver function (bilirubin <30µmol/L, INR <1.5, albumin >30 g/L and grade ≤2 encephalopathy) were considered

Short-term outcomes following posterior cervical fusion among octogenarians with cervical spondylotic myelopathy: a NSQIP database analysis.

BACKGROUND CONTEXT: Degenerative changes in the cervical spine occur in an age-dependent manner. As the US population continues to age, the incidence of age-dependent, multilevel, degenerative cervical pathologies is expected to increase. Similarly, the average age of patients with cervical spondylotic myelopathy (CSM) will likely trend upward. Posterior cervical fusion (PCF) is often the treatment modality of choice in the management of multilevel cervical spine disease. Although outcomes following anterior cervical fusion for degenerative disease have been studied among older patients (aged

Inpatient zoledronic acid in older hip fracture patients is well tolerated and safe.

UNLABELLED: Administering zoledronic acid (ZA) to older hip fracture patients during the hospital stay has faced safety concerns. However, in this study of 161 patients, no ZA-related side effects or readmissions were observed, demonstrating that ZA administration during hospitalization is safe and effective for secondary fracture prevention. PURPOSE: According to the 2022 Dutch 'Osteoporosis and fracture prevention' guideline, zoledronic acid (ZA) is the preferred osteoporosis treatment for hip fracture patients. Less than 25% of hip fracture patients visit the outpatient fracture liaison se

Unrecognized pulmonary arterial hypertension in hospitalized patients.

Pulmonary hypertension (PH) is an underdiagnosed and potentially fatal condition. The utility of screening for PH in hospitalized patients undergoing echocardiography is unknown. The goal of this study was to determine the prevalence of undiagnosed pulmonary hypertension (PH) and probable pulmonary arterial hypertension (PAH) in hospitalized patients undergoing echocardiography for any indication. All hospitalized patients undergoing echocardiography were identified and echocardiographs reviewed prospectively for the presence of a tricuspid regurgitant (TR) jet. Electronic medical records (EMR

Patient-Directed Discharge Among Hospitalized Persons With Opioid Use Disorder in the Fentanyl Era: A Scoping Review.

BACKGROUND: Patient-directed discharge (PDD), also known as discharge against medical advice, is associated with poor outcomes, such as increased hospital readmission and mortality. PDD is 10-20 times more common among persons with opioid use disorder (OUD). This scoping review sought to describe the reported incidence in the fentanyl era and characterize relevant patient level, clinical care level and systems level risk factors that may contribute to this phenomenon. METHODS: A comprehensive assessment of the literature was conducted across 4 electronic databases: PubMed, CINAHL, Cochrane Li

A risk nomogram for assessing complications in patients undergoing surgical procedures for cervical cancer. Does baseline quality of life play a role?

OBJECTIVE: Surgery for cervical cancer is a known risk factor for hospital readmission, often due to urological, lymphovascular, or neurological complications. While comorbidities are commonly used to assess surgical risk, patient-reported measures like baseline quality of life (QoL) may also help predict complications. This study aimed to estimate how often surgical complications occur and what factors predict them, with a focus on baseline QoL. METHODS: We conducted a prospective observational study of 100 cervical cancer patients who had surgery at a tertiary hospital between January 2010

Use of Negative Pressure Wound Therapy in Selected Cases to Facilitate Abdominal Wound Healing: A Multiple Case Series.

BACKGROUND: Surgical wound dehiscence resulting in exposed bowel increases the risk for development of an enterocutaneous fistula. Consequently, management of exposed bowel in an open wound presents challenges in terms of wound care management. In addition, a surgical wound dehiscence within the peristomal plane of an ileal conduit also creates a complex challenge of maintaining an ostomy pouching system seal. CASES: Case 1 was a patient with peristomal skin complications requiring stoma re-siting and subsequent surgical wound dehiscence at the prior ostomy site. As a result, we addressed wou

Using a Novel Smart-Device Application for Follow-up After Percutaneous Coronary Intervention.

Smart-device applications offer a potential novel and accessible opportunity to follow-up patients undergoing percutaneous coronary intervention (PCI) and identify opportunities to improve care. To date, no studies have described patient follow-up through surveys administered with the use of smart-device applications. Through a large online personal health management record solution system (MyChart) and a complementary smart-device application, an electronic 26-item survey was administered to patients on their mobile device at 4 days, 1 month, 3 months, and 6 months after PCI at a single terti

COPD Home Oxygen Therapy and Home Mechanical Ventilation: Improving Admission-Free Survival in Persistent Hypercapnic COPD.

As seen in this CME online activity (available at http://journal.cme.chestnet.org/copd-hot-hmv), acute exacerbations of COPD are associated with significant levels of morbidity and mortality. Acute noninvasive ventilation has been demonstrated its clinical efficacy and cost-effectiveness in reducing intubation rate and mortality and in patients with acute decompensated hypercapnic exacerbations of COPD. However, those patients with evidence of chronic hypercapnic respiratory failure have worse long-term outcomes compared with patients who have only transient hypercapnia during the acute phase

Hyperbaric oxygen therapy for the management of hemorrhagic radio-induced cystitis.

OBJECTIVES: Radio-induced cystitis (RADC) is an inflammatory bladder disease that presents as anemic-hematuria in its most serious form. Classic treatments can not control the disease in the mid-to-long term because they don't treat the pathogenesis of the disease. Thus, we evaluated the effectiveness of hyperbaric oxygen (HBO) therapy as a potential treatment for patients with RADC. METHODS: This prospective study included 38 patients, 21 men and 17 women, mean age of 66.5 years(46-75), who had been subjected to pelvic radiotherapy (RT), with the diagnosis of RADC with or without radio-induc
