The "Wall of Fear": the bridge between the traumatic event and trauma resolution therapy for childhood sexual abuse survivors.

A multitude of published books and papers on child sexual abuse (CSA) describe symptoms, long-term effects, and therapy for survivors of abuse. However, the parallels between the nature of the sexual trauma event(s) as originally experienced by the victim and the therapeutic process into which the survivor later becomes engaged have not been reported. This paper attempts to fill that gap and proposes that the concept of a Wall of Fear is the bridge connecting the two. In the first part of the paper, a model of the CSA experience based upon Furniss (1991) will be explained in order to point out

Irritable-hostile depression: further validation as a bipolar depressive mixed state.

BACKGROUND: "Hostile depression" has unofficially long been described as a depressive subtype, but since DSM-III, the affect has been made a defining characteristic of borderline personality disorder. The related affect of irritability in DSM-IV-TR subsumes various hostile nuances and is included in the stem question for mood disorders--especially for hypomanic episodes; in children, it is nonetheless a sign of depression. Then, there is the unofficial more general concept of depression with anger attacks, until recently ostensibly a "unipolar" (UP) disorder. A veritable tower of Babel indeed.

["Operationalised psychodynamic diagnostics in childhood and adolescence" (OPD-CA) in practice].

"Operationalised psychodynamic diagnostics in childhood and adolescence" (OPD-CA) has been completed in its first version after years of development. In a first interview with a 13-year-old boy, the diagnostic instruments of OPD-CA are presented by means of examples. The text passages of the transcribed first interview are evaluated in four sections by representatives of the respective working groups regarding the axes--relationships, conflict, structure and preconditions for treatment--and the diagnostic conclusions resulting from this are presented, thus providing a direct insight into the s

Agoraphobic behavior and panic attack: a study of male twins.

We used data from 3372 pairs of male twins from the Vietnam Era Twin Registry to examine comorbidity and familial influences on the frequently observed association between agoraphobic behavior and panic. Due to low prevalence of DSM-III-R-defined panic disorder, we also examined subjects who had experienced at least one panic attack during their lives. Agoraphobic behavior among individuals with a history of panic attacks showed familial aggregation (odds ratio = 5.7; P = .018 ). Probands with panic attacks and agoraphobic behavior and their co-twins had higher risk of major depression than pr

Identifying the concepts contained in outcome measures of clinical trials on stroke using the International Classification of Functioning, Disability and Health as a reference.

OBJECTIVES: To systematically identify and quantify the concepts contained in outcome measures in stroke trials using the International Classification of Functioning, Disability and Health (ICF) as a reference. METHODS: Randomized controlled trials between 1992 and 2001 were located in MEDLINE and selected according to predefined criteria. Outcome measures were extracted and concepts contained in the outcome measures were linked to the ICF. RESULTS: A random sample of 160 (50%) of 320 eligible studies was included. A total of 148 standardized health status measures were identified. Of 11,283

The self-fulfilling prophecy as an intrafamily dynamic.

This research examined whether parents' and children's perceptions of one another have reciprocal self-fulfilling effects on each other's behavior. Parents and their adolescent children completed self-report surveys and engaged in dyadic videotaped interaction tasks. The surveys assessed mothers', fathers', and children's perceptions of their own and the other's hostility and warmth. Observers coded the videotaped interactions to assess the actual hostility and warmth exhibited by parents and children. Data from 658 mother-child dyads were consistent with the conclusion that children had a sel

Expanding our understanding of the relationship between negative life events and depressive symptoms in black and white adolescent girls.

BACKGROUND: Little is known about the extent to which negative life events predict depressive symptoms in ethnically diverse groups or whether this relationship is proximal or enduring. METHOD: The relationship between negative life events in adolescence and depressive symptoms in young adulthood was studied in a sample of over 1300 black and white female adolescents. Five domains of life events were assessed at age 16 years and depressive symptoms were measured at age 18 and again at age 21 years. Questions of interest included whether the association continued over time and whether there we

Anticipatory processing in social anxiety: two pilot studies.

Two studies investigating the cognitive processes associated with anticipatory social anxiety are reported. Study 1 used a semi-structured interview to compare high and low socially anxious individuals ( [Formula: see text] per group) in terms of their reported mental processes during periods of anticipatory social anxiety. Study 2 investigated the anxiety inducing effects of the mental processes that were shown to be characteristic of high socially anxious individuals in Study 1. Prior to giving a speech, high and low socially anxious individuals ( [Formula: see text] per group) either engage

Household willingness to pay for azithromycin treatment for trachoma control in the United Republic of Tanzania.

OBJECTIVE: Household willingness to pay for treatment provides important information for programme planning. We tested for relationships between socioeconomic status, risk of trachoma, perceptions of the effects of azithromycin, and the household willingness to pay for future mass treatment with azithromycin. METHODS: We surveyed 394 households in 6 villages located in central United Republic of Tanzania regarding their willingness to pay for future azithromycin treatment. A random sample of households with children under 8 years of age was selected and interviewed following an initial treatm

Effectiveness of pharmacotherapy for pathological gambling: a chart review.

Although pathological gambling is a relatively common disorder, there exists only limited information regarding the effectiveness of pharmacotherapy for this illness. This study examines which medications may be effective, dose and duration of medication trials needed to achieve response, and possible predictors of response. Using a chart review, 50 adult outpatients with DSM-IV pathological gambling treated in clinical practice were assessed regarding response to a variety of medications, including augmentation strategies, and response to concomitant psychotherapy. All subjects received pharm

Social-cognitive processes in preschool boys with and without oppositional defiant disorder.

The social-cognitive characteristics of 88 preschool boys with Oppositional Defiant Disorder (ODD) and 80 nondisruptive boys were assessed 3 times over a 2-year period. Three questions were addressed: (1) Do social-cognitive processes (encoding, attribution, problem solving, and response evaluation) distinguish clinic-referred preschool boys from peers without behavior problems? (2) What is the relation between preschoolers' social-cognitive processes and observed problem behavior? (3) Are the social-cognitive processes of clinic boys with ODD influenced by comorbidity with Attention Deficit H

An open naltrexone treatment study in pathological gambling disorder.

The present study was designed to test the short-term efficacy and safety of naltrexone in the treatment of pathological gambling disorder. Seventeen subjects (seven men, 10 women) who fulfilled DSM-IV criteria for pathological gambling disorder, and were free from other Axis I diagnoses by Structured Clinical Interview for DSM-III-R screening, participated in a 6-week open naltrexone flexible dose trial. Gambling symptom change was assessed with the patient-rated Clinical Global Impression (CGI) Scale, the clinician-rated CGI and the Gambling Symptom Assessment Scale. Side-effects were monito

Longitudinal findings from the Normative Aging Study: III. Personality, individual health trajectories, and mortality.

Individual differences in physical and psychological health trajectories were examined in 1,515 Normative Aging Study men. Mean age at baseline was 47.15 years (range = 28-80), and average follow up was 18.55 years (range = 8-25). Both linear and nonlinear growth curves were estimated with random-effects models and then clustered to identify patterns of change. Men whose physical health trajectories were characterized by high, increasing symptoms were higher in hostility and anxiety, were overweight, and smoked. Those whose trajectories were characterized by low symptoms were emotionally stabl

Associations among maternal depressive symptomatology, state of mind and parent and child behaviors: implications for attachment-based interventions.

Associations among maternal depressive symptomatology, maternal state of mind with regard to attachment, and mother and child behaviors were examined in the treatment groups of an attachment-oriented intervention for primiparous women at high risk for parenting difficulties. The Adult Attachment Interview (AAI) was completed by 48 intervention and 62 control subjects when their children were 19 months old and the Center for Epidemiologic Studies-Depression Inventory (CES-D) and parent-child laboratory tasks were completed by the same subjects when their children were 24 months old. Evaluations, attributions, affect, and disciplinary choices in mothers at high and low risk for child physical abuse.

OBJECTIVE: The present study investigated several components of a social information-processing model of child physical abuse. The main objective was to examine the extent to which high-risk, relative to low-risk, mothers differed in their evaluations, attributions, negative affect, and disciplinary choices for children's behavior, and to explore whether these differences may be expressed in interactions between risk status and mitigating information. METHOD: Nineteen high- and 19 matched low-risk mothers' evaluations of children transgressions, attributions, affect, and choices of disciplina

Psychiatric diagnoses in inner city outpatients with moderate to severe asthma.

OBJECTIVE: Psychiatric symptoms may be associated with increased asthma morbidity and mortality. However, no investigations have identified syndromal psychiatric diagnoses in asthma patients using current diagnostic criteria or examined treatment received for mental illness. METHOD: We conducted structured clinical interviews on 32 patients with moderate to severe asthma to identify current and past psychiatric illness. RESULTS: Twenty-five percent of subjects had current major depressive disorder, but only 25 percent of these received antidepressants. Anxiety disorders, including panic diso

Return to work after lung transplantation.

The social rehabilitation of lung transplant recipients becomes increasingly important as the results of lung transplantation improve. Although return-to-work (RTW) rates have been published for recipients of other organ transplants, no such data are available after lung transplantation. The purpose of this study was to determine what factors influence RTW after lung transplantation. Of 99 lung transplant recipients (43 single, 56 bilateral) surveyed from Denver, Colorado, (n = 49) and Toronto, Ontario, Canada (n = 50), 22% (n = 22) were employed, 38% (n = 38) were unemployed but medically abl

Measuring treatment outcome by the Beck Depression Inventory.

The construct validity of the Beck Depression Inventory (BDI) in measuring treatment outcome is assessed in 103 psychiatric inpatients. In this context, construct validity means that the BDI measures the same construct in repeated measurement and that the change scores can be explained by treatment effects. In confirmatory factor analyses, only the first factor proved to be stable. In accordance with other studies, the sensitivity to therapeutic change in long-term intervals of several weeks could be confirmed. Significant changes in a short-term interval of 1 day in the non-endogenously depre
