Parahippocampal tau pathology in healthy aging, mild cognitive impairment, and early Alzheimer's disease.

Abnormally phosphorylated tau accumulates as neurofibrillary tangles and neuropil threads in older persons with and without Alzheimer's disease. The relationship between neurofibrillary tangles and neuropil threads and how they relate to cognitive function is unknown. This study investigated the relationship between phosphorylated tau lesions and cognitive function in 31 persons participating in the Religious Orders Study, a prospective, longitudinal clinicopathological study of aging and Alzheimer's disease. All subjects underwent detailed neuropsychological performance testing within a year

Delayed hypersensitivity skin testing to mumps and Candida albicans antigens is normal in middle-aged HTLV-I- and-II-infected U.S. cohorts.

It has been reported that human T cell lymphotropic virus (HTLV)-I-infected persons in Japan have decreased delayed hypersensitivity skin test reactivity to tuberculin purified protein derivative (PPD), but HTLV-I- or -II-infected persons do not generally develop opportunistic infections. We administered standardized intradermal testing with PPD, mumps, and Candida albicans antigens to 31 HTLV-I, 48 HTLV-II, and 143 seronegative subjects in the United States. Reactivity at 48 hr was compared among the three groups. Response rates to PPD were very low in all subjects. Fifty-five percent of sero

Large-scale quarantine following biological terrorism in the United States: scientific examination, logistic and legal limits, and possible consequences.

Concern for potential bioterrorist attacks causing mass casualties has increased recently. Particular attention has been paid to scenarios in which a biological agent capable of person-to-person transmission, such as smallpox, is intentionally released among civilians. Multiple public health interventions are possible to effect disease containment in this context. One disease control measure that has been regularly proposed in various settings is the imposition of large-scale or geographic quarantine on the potentially exposed population. Although large-scale quarantine has not been implemente

[Consideration of mental symptoms and physical disabilities in deciding upon placement in a residential home versus a nursing home].

This study analysed 235 applications of elderly people for residential home and nursing home care. The applications were submitted to a Dutch municipal care allocation board. Based on the 1994 registration data from this board the impact of physical restrictions and mental problems on the care allocation for residential or nursing homes and connected care was studied. Physical complaints were measured with an adl and an hdl scale (Activities of Daily Living and Household Activities of Daily Living), mental problems were assessed by means of the Reality, Orientation and Restlessness Scales. Prevalence and conditions of urinary incontinence among the elderly.

In Japan, elderly disorders and diseases have markedly increased in recent years, because of rapid aging and an increasing number of older persons. The situation is creating serious social and community problems. These disorders, particularly dysuria and urinary incontinence (UI), disturb the quality of life (QOL) in latelife. Few reports on UI have been published, but precise investigation into the community level remains to be made. Our presentation is the development, implementation and evaluation of elderly UI in Kumamoto Prefecture. This study includes 2,304 people (male: 856, female: 1,4

Neurogenic bowel dysfunction after spinal cord injury: clinical evaluation and rehabilitative management.

Neurogenic bowel dysfunction (NBD) is one of many impairments that result from spinal cord injury (SCI). The experience of persons with SCI reveals that the risk and occurrence of fecal incontinence and difficulty with evacuation are particularly significant life-limiting problems. This review relates the anatomy and physiology of colon function to the specific pathophysiology that detracts from the quality of life of persons after SCI. There are two patterns of NBD after SCI: the upper motor neuron bowel, which results from a spinal cord lesion above the sacral level, and the lower motor neur

Onchocerciasis in an expatriate living in Cameroon.

Travelers to West, central and eastern Africa as well as to selected areas of Latin America are at risk for infection with Onchocerca volvulus. Infection with this tissue nematode may cause chorioretinitis and keratitis, and it is responsible in endemic areas for blindness in millions.1 In addition to ocular manifestations, it can produce a distressing pruriginous dermatitis or subcutaneous nodules. Clinical manifestations vary according to the parasitic load, previous immunity, and duration of infection.1 Infection is initiated by inoculation with larvae during the bite of the Simulium black

Preventing toxoplasmic encephalitis in persons infected with human immunodeficiency virus.

Toxoplasmic encephalitis (TE) is the second most common AIDS-related opportunistic infection of the CNS. It occurs in 10%-50% of patients with AIDS who are seropositive for antibodies to Toxoplasma gondii and have CD4+ T lymphocyte counts of < 100/mm3. Primary toxoplasmic infection usually is acquired by ingestion of T. gondii oocysts from soil contaminated by cat feces or by ingestion of tissue cysts present in undercooked red meats. In patients with AIDS, TE probably results from the reactivation of Toxoplasma tissue cysts that remained latent after the primary infection. Detection of IgG an

[Cryptosporidiosis in humans. Review].

Cryptosporidiosis basically is a gastrointestinal infection caused by the coccidian protozoa Cryptosporidium. The infection is associated with diarrhea worldwide but it is most prevalent among children below 5 years of age in the undeveloped countries. It is an important Public Health problem. Infection in humans is usually with C. parvum. The parasite appears to be transmitted by a variety of mechanisms but zoonotic- and person-to-person transmission, and contaminated water appear to be the most important. The mechanism by which the coccidium causes diarrhea is unknown. The extent of the dise

Fatal respiratory disease due to Corynebacterium diphtheriae: case report and review of guidelines for management, investigation, and control.

Dramatic reductions in the incidence of diphtheria and high levels of childhood vaccination in recent decades have led the United States to establish the goal of diphtheria elimination among persons < or = 25 years of age by the year 2000. In 1990, an unimmunized 25-month-old child died of respiratory diphtheria in Dade County, Florida, before treatment with diphtheria antitoxin could be instituted. Twenty-three asymptomatic household contacts and other close contacts of the child were identified, cultured for Corynebacterium diphtheriae, given antimicrobial prophylaxis, and vaccinated with di

[Poliomyelitis in The Netherlands, 1979-1991: immunity and exposure].

An overview is presented of serological and virological studies on poliovirus immunization and circulation in the Netherlands, performed between 1979 and 1991. In this period, only three patients with poliomyelitis were notified. All had acquired the infection abroad. The vaccinations in the national immunization programme, using inactivated poliovirus vaccine, build a strong immunity. This can also be seen in age-stratified serological profiles of the Dutch population. In these surveys, persons from the time at which vaccination was offered have neutralizing antibodies. Older persons, especia

A survey of the health of British missionaries.

The results of medical examinations carried out on 212 missionary personnel from one missionary society returning on leave to the UK are presented. The great majority of missionaries worked in developing countries. They served in 27 countries altogether and for a total of 488 person years. The commonest illnesses reported overseas were malaria (87.3 per 1000 person years at risk), diarrhoea (63.5), anxiety (63.5), depression (41.0) and giardiasis (38.9). More illnesses were reported from West Africa (698 per 1000 person years at risk) than from any other region. Ten people (4.7%) were repatria

Definition of two distinct types of AIDS-associated non-Hodgkin lymphoma.

Non-Hodgkin lymphoma (NHL) in a human immunodeficiency virus (HIV)-infected person is an AIDS-defining condition. There is both an increased incidence of NHL and a poorer prognosis for the disease in these HIV-infected persons, compared with patients who spontaneously develop NHL. A series of 41 consecutive patients with HIV-associated NHL treated at St Vincent's Hospital, Sydney, between 1984 and September 1989 was assessed. Of these, 36 had NHL proven on biopsy and NHL was diagnosed clinically in the other five. On histologic examination 11 had high-grade malignant lymphoma, small non-cleave

Artificial Intelligence in Lung Cancer: A Narrative Review of Recent Advances in Diagnosis, Biomarker Discovery, and Drug Development.

This review highlights the rapidly evolving role of artificial intelligence (AI) in transforming lung cancer care, with a specific focus on its integrated applications across diagnosis, biomarker discovery, and drug development. The novelty of this work lies in its holistic examination of how AI bridges these traditionally separate domains, from radiology and pathology to genomics and clinical trials, to create a more cohesive and personalized oncology pipeline. We detail how AI algorithms significantly enhance early detection by improving the accuracy and efficiency of pulmonary nodule charac

Frameworks to analyse accessibility of public recreational spaces for persons with disabilities: a rapid scoping review.

The global population currently struggles to meet the physical activity guidelines. This is exacerbated for persons with disabilities (PWD). There is increasing acknowledgement of the need for recreational public spaces to be accessible and provide opportunities for physical activity. Designing and evaluating facilities requires tools and frameworks that capture diversity and consider access beyond the physical environment. Through a rapid scoping review, we sought to describe peer-reviewed literature that developed or implemented frameworks (conceptual or analytical) to analyse access for PWD

Diabetes Technology in the Primary Care Setting: An American Diabetes Association Scientific Statement.

Most diabetes care is delivered in primary care, yet diabetes technologies remain inconsistently integrated. Workforce, training, time, data infrastructure, and insurance barriers contribute to inequitable access and adoption. This scientific statement provides evidence-informed recommendations for integrating diabetes technology into primary care practices. The American Diabetes Association (ADA) convened an interdisciplinary panel that met in person at the Diabetes Technology Meeting in October 2025. Panelists reviewed available evidence, published between 2020 and 2025, on diabetes technolo

Status of Alzheimer's and Parkinson's Disease Detections and Treatments.

Alzheimer's disease and related dementias affect more than 55 million people worldwide, a number projected to double by 2050 as populations age. Beyond the devastating personal toll on patients and families, the global economic burden is estimated at approximately $1.3 trillion annually. Despite more than a century of research since the initial descriptions of Parkinson's disease (PD) in 1817 and Alzheimer's disease (AD) in 1906, the fundamental drivers of these disorders and effective strategies to prevent their progression remain poorly understood, despite billions of dollars invested in res

Health literacy in adults with a spinal cord injury or disorder: An updated and expanded systematic review.

BACKGROUND: The importance of health literacy in personal health management is widely recognised in the general population, yet remains poorly understood in some patient groups, including people with spinal cord injury or disorder (SCI/D). OBJECTIVES: The systematic review had two objectives: (1) to determine the health literacy levels of individuals with SCI/D, and (2) to identify facilitators and barriers to developing health literacy in this group. METHODS: Seven databases (CINAHL, Embase, Emcare, ProQuest, PsycINFO, PubMed, and Web of Science) were searched from inception, with an update

Personalized intensity modulation radiation therapy auto-planning for esophageal cancer.

Esophageal cancer (EC) is among the most common malignancies worldwide, with radiotherapy as a key treatment. Intensity-modulated radiation therapy (IMRT) offers highly conformal dose distributions while sparing critical organs; however, planning is complex due to the esophagus' central location and proximity to vital structures. This study aimed to develop a personalized, automated IMRT planning system for EC. A retrospective analysis of 301 patients treated with 45 Gy in 25 fractions was conducted. Patient data, including CT images, structure sets, and clinical plan (CP) dose distributions,
