Diabetes Associated With Maternally Inherited Diabetes and Deafness (MIDD): From Pathogenic Variant to Phenotype.

Maternally inherited diabetes and deafness (MIDD) is a mitochondrial disorder characterized primarily by hearing impairment and diabetes. m.3243A>G, the most common phenotypic variant, causes a complex rewiring of the cell with discontinuous remodeling of both mitochondrial and nuclear genome expressions. We propose that MIDD depends on a combination of insulin resistance and impaired β-cell function that occurs in the presence of high skeletal muscle heteroplasmy (approximately ≥60%) and more moderate cell heteroplasmy (∼25%-72%) for m.3243A>G. Understanding the complex mechanisms of MIDD is

Examining the cognitive phenotype in Neurofibromatosis 1: a systematic review of functional imaging studies

Neurofibromatosis 1 (NF1) is a rare genetic condition, frequently associated with learning difficulties. Functional neuroimaging techniques have been used to investigate brain mechanisms underlying learning difficulties in NF1, but these remain poorly understood. We conducted a systematic review of functional neuroimaging studies in NF1 from 2000-2024 aiming to develop a neurobiological framework for understanding the NF1 cognitive phenotype. A total of 44 studies were identified as relevant across functional magnetic resonance imaging (fMRI), electroencephalography (EEG), magnetic resonance s…

The Role of Protein Kinases in the Suppressive Phenotype of Myeloid-Derived Suppressor Cells.

Inflammation is a self-defense mechanism that controls the homeostasis of an organism, and its alteration by persistent noxious stimuli could lead to an imbalance in the regulation of inflammatory responses mediated by innate and adaptive immunity. During chronic inflammation, sustained exposure of myeloid cells to the various inflammatory signals derived from inflamed tissue could lead to the generation of myeloid cells with an immunosuppressive state, called myeloid-derived suppressor cells (MDSCs), which can exert protective or deleterious functions depending on the nature of signals and th

D-- phenotype in a South Indian family: A multicentric approach to workup and management.

BACKGROUND: The Rh system is an extremely important blood group system with over 50 antigens, 5 of which (D, C, E, c and e) are considered most clinically significant. Rare Rh deficient phenotypes include D--, which is a blood group characterised by the lack of expression of C, c, E and e and exalted expression of the D antigen on the red cells due to mutations in both alleles of the RHCE gene. This is a multicentre approach to a case of the rare D-- phenotype. CASE REPORT: A 56-year-old lady with bad obstetric history presenting with severe anaemia had to be evaluated for a panreactive antib…

Baked Milk Modulates Cow's Milk Allergy in Children: Impact of Phenotype, Age, and Intake.

BACKGROUND: Limited evidence indicates that baked milk (BM) intake accelerates cow's milk allergy (CMA) resolution or modulates immunity in BM tolerant (BMT) and BM-allergic (BMA) phenotypes. OBJECTIVE: This study evaluates the efficacy, safety, and immunological effects of BM introduction, focusing on regular intake in BMT and low-dose intake in BMA, and their impact on tolerance development. METHODS: A prospective study including children 12 to 72 months of age with an oral food challenge (OFC) confirmed CMA. Children with CMA were categorized into BMT or BMA phenotypes based on BM-OFC res…

Expanding the Phenotype of CYFIP2-Related Developmental Epileptic Encephalopathy: Case Report and Literature Review.

BACKGROUND: Pathogenic CYFIP2 variants cause developmental and epileptic encephalopathy (DEE), characterized by early-onset intractable epilepsy and developmental delay. The disease course has not been delineated. Codon Arg87 is a de novo mutational hotspot associated with a severe DEE phenotype via gain-of-function mechanisms. Currently, there are no targeted therapies for CYFIP2-related DEE. Genetic therapies initiated at early symptomatic stages have demonstrated benefit in other disorders and may be an important consideration for this patient population. METHODS: We describe a child with …

Differences in the Phenotype of Bacterial and Viral Sepsis-A Prospective, Multicenter, Observational Study.

Sepsis is defined as a dysregulated host response to an infection, leading to life-threatening organ dysfunction. While sepsis is most commo … SOFA components, while other organ dysfunctions were less frequent. Significant differences in the phenotype of bacterial and viral sepsis were observed, characterized by distinct inflammatory profiles and differing patterns of organ dysfunction. These findings may support the improved differentiation of bacterial and viral etiologies in sepsis.

Blown-out myotomy: emerging phenotype in post-myotomy failure after POEM and Heller myotomy.

Blown-out myotomy (BOM) is an underrecognized but increasingly acknowledged long-term structural complication following both laparoscopic Heller myotomy (LHM) and peroral endoscopic myotomy (POEM) for achalasia. Characterized by a localized pseudo-diverticulum or eccentric dilation at the prior myotomy site, BOM often manifests with recurrent dysphagia, regurgitation, and food stasis, and may mimic treatment failure or other anatomic abnormalities. Radiologically, BOM is defined as a > 50% increase in distal esophageal diameter, best visualized on barium esophagogram, with corresponding endosc

Dysfunction of core clock genes regulates malignant phenotype and gemcitabine sensitivity of cholangiocarcinoma cells.

The circadian clock governs daily rhythms in numerous physiological processes through precise regulation of gene expression and biochemical functions. Dysregulation of the circadian rhythm has been implicated in carcinogenesis and cancer progression. However, the mechanisms by which the circadian clock influences cancer phenotype and chemotherapy resistance, particularly in cholangiocarcinoma (CCA), remain poorly understood. Using cell lines established from primary CCA and metastatic ascites of two male patients, we manipulated core clock genes ( BMAL1 , PER2 , and NR1D1 ) to evaluate their e…

A FABulous Couple: Adding PHENotype to GENotype Improves HMA/VEN Response Prediction in AML.

In a large retrospective multicenter study of patients with acute myeloid leukemia treated with hypomethylating agents and venetoclax, Lachowiez and colleagues examine the interplay among genetic mutations, disease phenotype, and clinical outcomes. They show that within genetic subgroups-particularly NPM1 wild-type cases-monocytic differentiation is linked to poor treatment response and inferior long-term prognosis, adding a phenotypic layer to genetics-based response prediction. See related article by Lachowiez et al., p. 437.

The Impact of Mechanical Circulatory Support Devices on White Blood Cell Phenotype and Function.

BACKGROUND: Mechanical circulatory support devices (MCSDs) have gradually become an effective treatment of end-stage heart failure (HF). However, the introduction of foreign surfaces and non-physiological shear stress (NPSS) can cause severe damage to various blood cells, leading to impaired function of immune system and increased risk of complications such as inflammation and thrombosis. The effect of mechanical injury on white blood cell (WBC) has been largely neglected compared to that on red blood cell (RBC) and platelet (PLT). METHOD: To better understand the impact of MCSDs on WBCs and

Incidence of Leading Causes of Pediatric CKD Using Electronic Health Record-Driven Computable Phenotype.

KEY POINTS: We created computable phenotypes to accurately identify cases of pediatric CKD by underlying diagnosis. Combined annual incidence of five leading causes of pediatric CKD was high, 47.07 (95% confidence interval, 45.96 to 48.20) per 100,000 children. Our computable phenotypes have the potential to be broadly implemented to advance epidemiologic research in pediatric CKD. BACKGROUND: Incidence data on pediatric CKD are incomplete. We developed electronic health record–based algorithms (e-phenotypes) to identify cases and provide incidence estimates of five leading causes of pediatri…
