The prevalence of symptoms of sleep-disordered breathing in Brazilian schoolchildren.

OBJECTIVE: To identify the prevalence of symptoms of sleep-disordered breathing among children of low socioeconomic status in the South of Brazil. METHODS: This was a cross-sectional study, carried out in the city of Uruguaiana, RS, in which specific questionnaire about the symptoms of sleep-disordered breathing was completed by the parents of a sample of schoolchildren aged 9 to 14 years, enrolled on the International Study of Asthma and Allergies in Childhood (ISAAC). RESULTS: From the total of 1,011 eligible schoolchildren, 998 questionnaires were completed. The parents of 27.6% of the ch

Cardiovascular changes in children with snoring and obstructive sleep apnoea.

INTRODUCTION: Adults with obstructive sleep apnoea (OSA) are well documented to be at high risk for cardiovascular abnormalities. Growing evidence suggests that OSA is also associated with cardiovascular consequences in children. The purpose of this review is to examine the available data on this association in children. METHODS: Primary studies were extracted from a MEDLINE search limited to those published between 1970 and 2008. The keywords used included child, sleep disordered breathing, sleep apnoea, snoring, blood pressure and hearts. The relevant articles were selected by consensus bet

The effect of clomipramine on wake/sleep and orexinergic expression in rats.

We have previously found that neonatal treatment with clomipramine (CLI) induced a decrease in brain orexins during the juvenile period and that these changes were reversed at adulthood. This study investigated the effect of CLI on the orexinergic component and sleep/wake states. Two groups of adult male rats were conducted for 48-h polysomnographic recording. One group of rats was treated with CLI (20 mg/kg every 12 h), and a second group was treated with equivolume of saline (SAL) simultaneously after the first 24 h of polysomnographic recording. Rats were killed 2 h after the third dose of

Sleep apnea and mandibular advancement device. Revision of the literature.

Sleep apnea and hypopnea syndrome (SAHS) is a disorder characterized by intermittent and repetitive obstruction of the upper airway provoking pharyngeal collapse. It is characterized clinically by a triad of daytime hypersomnia, snoring and pauses in breathing during sleep that are normally reported by the partner. Polysomnography is the chosen method for diagnosing this pathology. Patients with this disorder tend to have the following dental and orofacial signs: a retrognathic jaw, a narrow palate, a wide neck, deviation of the nasal septum and relative macroglossia, among others. Dentists sh

Sleep disturbances in patients with schizophrenia : impact and effect of antipsychotics.

Difficulties initiating or maintaining sleep are frequently encountered in patients with schizophrenia. Disturbed sleep can be found in 30-80% of schizophrenic patients, depending on the degree of psychotic symptomatology. Measured by polysomnography, reduced sleep efficiency and total sleep time, as well as increased sleep latency, are found in most patients with schizophrenia and appear to be an important part of the pathophysiology of this disorder. Some studies also reported alterations of stage 2 sleep, slow-wave sleep (SWS) and rapid eye movement (REM) sleep variables, i.e. reduced REM l

The role of the nose in the pathogenesis of obstructive sleep apnoea and snoring.

Data from observational studies suggest that nasal obstruction contributes to the pathogenesis of snoring and obstructive sleep apnoea (OSA). To define more accurately the relationship between snoring, OSA and nasal obstruction, the current authors have summarised the literature on epidemiological and physiological studies, and performed a systematic review of randomised controlled trials in which the effects of treating nasal obstruction on snoring and OSA were investigated. Searches of bibliographical databases revealed nine trials with randomised controlled design. External nasal dilators w

The neural circuit of orexin (hypocretin): maintaining sleep and wakefulness.

Sleep and wakefulness are regulated to occur at appropriate times that are in accordance with our internal and external environments. Avoiding danger and finding food, which are life-essential activities that are regulated by emotion, reward and energy balance, require vigilance and therefore, by definition, wakefulness. The orexin (hypocretin) system regulates sleep and wakefulness through interactions with systems that regulate emotion, reward and energy homeostasis.

Orexin neuronal circuitry: role in the regulation of sleep and wakefulness.

Orexin A and orexin B were initially identified as endogenous ligands for two orphan G protein-coupled receptors [104]. They were initially recognized as regulators of feeding behavior in view of their exclusive production in the lateral hypothalamic area (LHA), a region known as the feeding center, and their pharmacological activity [104,30,49,107]. Subsequently, the finding that orexin deficiency causes narcolepsy in humans and animals suggested that these hypothalamic neuropeptides play a critical role in regulating sleep/wake cycle [22,46,71,95,117]. These peptides activate waking-active m

Atherosclerosis and obstructive sleep apnea in patients with ischemic stroke.

BACKGROUND: Of late, obstructive sleep apnea (OSA) has been suggested to be a risk factor for atherosclerotic artery disease. In the present study, we analyzed the prevalence of atherosclerosis in stroke patients with and without OSA. PATIENTS AND METHODS: Two hundred and fourteen consecutive patients with ischemic stroke were included in this study. Patients were screened for classical cerebrovascular risk factors. All patients received a cardiorespiratory polygraphy to determine the apnea-hypopnea index. Duplex ultrasonography was used to evaluate the carotid bulb and the internal carotid a

[The neurology of REM sleep. A synoptic tour de force].

REM or dreaming sleep is one of three states of consciousness. It is characterized by rapid eye movements, muscle atonia, desynchronized EEG, and autonomic dysregulation. A dysfunction of one of these four cardinal traits explains the primary disorders of REM sleep, including REM sleep behavior disorder and narcolepsy. Although seen in other stages, sleep apnea syndrome, coronary syndromes, and stroke are strongly triggered by the autonomic dysregulation of REM sleep. REM sleep has an antiepileptic effect, which has not yet been used in clinical practice. It favors procedural memory, but not d

Sleep disruptions and emotional insecurity are pathways of risk for children.

BACKGROUND: Sleep problems are prevalent in American children. A critical need is to identify sources and processes related to sleep disruptions and their sequelae. We examined a model linking parental marital conflict and children's emotional insecurity, sleep disruptions, and their adjustment and academic problems. METHOD: One hundred and sixty-six elementary school children reported on marital conflict and their emotional insecurity, the quantity and quality of children's sleep were examined through actigraphy, and parents and teachers reported on child functioning. RESULTS: In the contex

Children's sleep and adjustment: the moderating role of vagal regulation.

Children's vagal tone and vagal suppression were examined as moderators of associations between children's sleep disruptions and adjustment problems. A relatively large sample (n = 167) of boys and girls who ranged in age between 8 and 9 years participated with their parents. Sleep was examined via actigraphy in the child's home for seven consecutive days. Children's vagal tone was examined during baseline conditions, and their vagal suppression was assessed in response to an inter-adult argument. In comparison with children exhibiting higher levels of vagal tone, those with lower vagal tone w

Effect of tolterodine on sleep structure modulated by CYP2D6 genotype.

OBJECTIVE: Tolterodine, a drug for the treatment of overactive bladder symptoms, has a limited entry into the brain, which makes cognitive side effects seldom. However, some case reports have described central-nervous side effects such as sleepiness. The aim of this retrospective analysis was to investigate whether tolterodine-related effects on sleep stage parameters could be explained by different CYP2D6 metabolizer characteristics of subjects. METHODS: Data were taken from two randomized, double-blind, placebo-controlled studies conducted in a cross-over design. Forty-eight volunteers unde

Work factors as predictors of poor sleep in nurses' aides.

OBJECTIVES: The aim was to identify work factors that predict poor sleep in nurses' aides (assistant nurses). METHODS: The study was based on a randomly selected, nationwide sample of Norwegian nurses' aides. Of 5,513 nurses' aides, not on leave when they completed a mailed questionnaire in 1999, 4,771 (86.5 %) completed a second questionnaire 3 months later. A wide spectrum of work factors was assessed at baseline by questions from the General Nordic Questionnaire for Psychological and Social factors at Work. Subjective sleep quality during the previous 3 months was measured at baseline and

Daytime sleepiness in the obese: not as simple as obstructive sleep apnea.

OBJECTIVE: Excessive daytime sleepiness is a common symptom in obese patients, but what drives this condition is unclear. The objective was to look for clinical, anthropometric, biochemical, and polysomnographic predictors of excessive daytime sleepiness as measured by the Epworth Sleepiness Scale (ESS) in obese patients. RESEARCH METHODS AND PROCEDURES: The ESS questionnaire was completed by 1,055 consecutive patients presenting for obesity surgery. Those at high risk for obstructive sleep apnea (n = 331) had diagnostic overnight polysomnography preoperatively. All patients had preoperative

The predictors of central and obstructive sleep apnoea in haemodialysis patients.

BACKGROUND: Sleep apnoea (SA) is often observed in haemodialysis patients, but there have been few studies on types of SA and their predictors. We therefore investigated the prevalence and types of SA and the associations between types of SA and clinical factors in haemodialysis patients. METHODS: We initially examined nocturnal oxygen desaturation index (ODI) (desaturation of >4%/events per hour) in 119 haemodialysis patients (68 males, mean age of 61.4 years). Patients with ODI of more than five were diagnosed as having SA. Then, 30 patients underwent polysomnography and we measured Apnoea-

Placebo and modafinil effect on sleepiness in obstructive sleep apnea.

INTRODUCTION: Previous studies have evaluated the effect of modafinil on residual excessive daytime sleepiness (EDS) in patients with obstructive sleep apnea syndrome (OSAS) under effective CPAP treatment. Even though those trials also used placebo groups, we suppose that the placebo effect might influence the patients' response to modafinil. METHODS: Twenty sleepy patients with OSAS under CPAP treatment were selected. All of them had Epworth Sleepiness Scale (ESS) >10. Following baseline evaluation (T1), all subjects were instructed to take placebo for 7 days. After this single-blind placebo
