PURPOSE: To examine the impact of upper airway stimulation therapy on the apnea index/hypopnea index ratio in patients with obstructive sleep apnea. METHODS: We retrospectively analyzed 118 patients who received an upper airway stimulation device between 2015 and 2022. RESULTS: The apnea–hypopnea index at baseline was 35.3 ± 8.9 events/hour, which significantly decreased to 14.9 ± 13.1 events/hour at three months and 14.9 ± 9.9 events/hour at twelve months (p < 0.001). The apnea index/hypopnea index ratio shifted from 1:1.3 at baseline to 1:2.3 and 1:2.4 at three and twelve months, respectivel

OBJECTIVE: To identify the patterns of airway collapse in patients with obstructive sleep apnea hypopnea syndrome(OSAHS) by dexmedetomidine induced sleep endoscopy.
METHODS: Forty-five obstructive sleep apnea patients diagnosed by polysomnography were given dexmedetomidine intravenously. Once the patient was sedated in dorsal position, the electronic nasopharyngoscope was inserted transnasally and positioned on five levels of the upper airway sequentially (velum, oropharyngeal lateral wall, tongue base, epiglottis and larynx) to observe and document the collapse. Each level should be observed

Arousal is an important protective mechanism that aids in the resolution of obstructive sleep apnea in adults and children, but its role in neonatal apnea has not been investigated. The primary aim of the present study was to determine the role of arousal in the termination of apnea in preterm infants. Videorecording was used to identify spontaneous behavioral arousal in a group of healthy full-term (n = 7) and preterm (n = 10) infants before and during polygraphic monitoring of cardiorespiratory variables and in a group of preterm infants with apnea (n = 10) during similar polygraphic monitor

Freeman-Sheldon syndrome, or distal arthrogryposis type IIA (DA 2A), is a rare and severe multiple congenital contracture syndrome that is associated with upper airway obstruction. This obstruction has been clinically significant enough to warrant tracheostomy and has been associated with mortality. We describe a patient who presented to us as a neonate and the novel management of her respiratory obstruction in the setting of DA 2A. Bilateral mandibular osteotomies were performed and bilateral internal mandibular distracters were placed. She was distracted a total of 3 cm over 15 days without

INTRODUCTION: Sleep-disordered breathing (SDB) in patients with motor neurone disease (MND) is normally attributed to hypoventilation due to muscle weakness. However, we have observed different patterns of SDB among MND patients referred for non-invasive ventilation, which do not appear to be explained by respiratory muscle weakness alone.
AIM: The aim of this study was to examine the characteristics of SDB in MND.
METHODS: This is a retrospective analysis of sleep studies (using polysomnography [PSG]), pulmonary function tests, and arterial blood gases in MND patients referred to a tertiary

Despite advances in medical and surgical therapy, heart failure (HF) remains a common and serious problem. An association between HF and sleep-related breathing disorders has been recognized since Cheyne's observations in 1818, but only recently have treatment options targeting sleep-related breathing disorders become available. This overview will consider the clinical features, pathophysiology, and treatment options of sleep-related breathing disorders in patients with HF.

In this study, we proposed an automatic detector for obstructive apnea episodes using only ECG-based time-series from a single-ECG channel. Several obstructive apnea episodes were provoked for different separated sequences of 15 minutes in anesthetized Sprague-Dawley rats. In this recurrent obstructive sleep apnea (OSA) model, each episode lasted 15s, while the number of total events per sequence was randomly selected. The beat-to-beat interval ( RR) and the R-wave amplitude ( Ra) time-series were extracted and processed for each sequence, and used to train Dynamic Bayesian Networks with diffe

Constant positive airway pressure (cPAP) is the predominant noninvasive therapy for the obstructive sleep apnea syndrome (OSAS). The average pressure thereby applied can be reduced by online-identification of respiratory events. Oscillatory impedance measurement as an indicator for airway patency can identify central and obstructive apneas and hypopneas. Our goal was to reduce the sensors needed, to auto-calibrate the signals to each individual patient and to rule out patient induced artifacts.

The prevalence of sleep-disordered breathing (SDB) is high in patients with cardiovascular diseases. Typical symptoms like daytime sleepiness can be absent and those patients may report unspecific, therapy-resistant symptoms related to their underlying disease. Particularly sleep-related symptoms like nocturia, nocturnal dyspnea and pectangina can be present. Based on the results of recently published studies, the treatment of central sleep apnea in patients with symptomatic, systolic heart failure by adaptive servo-ventilation is no longer recommended. Although the treatment of obstructive sl

Many types of positive airway pressure (PAP) devices are used to treat sleep-disordered breathing including obstructive sleep apnea, central sleep apnea, and sleep-related hypoventilation. These include continuous PAP, autoadjusting CPAP, bilevel PAP, adaptive servoventilation, and volume-assured pressure support. Noninvasive PAP has significant leak by design, which these devices adjust for in different manners. Algorithms to provide pressure, detect events, and respond to events vary greatly between the types of devices, and vary among the same category between companies and different models

Sleep disordered breathing (SDB), i.e., obstructive, central or mixed sleep apneas, has been recognized as a common occurrence in the elderly. Aging is per se associated with a decrease in the quality of sleep; SDB may further disrupt the sleep architecture in older subjects. The prevalence of obstructive sleep apnea (OSA) increases with aging; available studies report prevalence rates of 11-62%. Furthermore, OSA has been associated with increased mortality in older adults. Central apneas and periodic breathing occur with increased frequency either in subjects with neurological disorders such

We measured the frequency distribution and the ventilatory correlates of the various types of apneas 3 to 15 s long during sleep in eight term infants (birth weight 3.65 +/- 0.16 kg; gestational age 39.5 +/- 0.3 wk) and eight preterm infants (birth weight 2.07 +/- 0.18 kg; gestational age 34.3 +/- 0.4 wk). Each infant was studied on five to seven occasions from birth to 56 wk of postconceptual age using a modified flow-through system. Sixty-six paired epochs of quiet sleep (1163 min) and rapid eye movement sleep (829 min) were analyzed in term infants and 85 paired epochs of quiet sleep (1553

BACKGROUND: Sleep disorders and stroke share a bidirectional relationship. Sleep disturbances are independent stroke risk factors and frequent post-stroke complications. Obstructive sleep apnea (OSA), through intermittent hypoxia, systemic inflammation, endothelial dysfunction, and autonomic dysregulation, increases stroke risk by approximately 2- to 3-fold. Post-stroke sleep disorders affect up to 59.9% of patients, impeding neurological recovery.
OBJECTIVES: To examine the epidemiology, pathophysiology, and clinical impacts of sleep disorders on stroke recovery, and to summarize evidence-ba

Psoriasis is a systemic inflammatory disease caused by dysfunctional interactions between the innate and adaptive immune responses. The systemic inflammation in psoriasis may be associated with the development of comorbidities, including lung diseases. In this review, we aimed to provide a summary of the evidence regarding the prevalence of lung diseases in patients with psoriasis and the potential underlying mechanisms. Twenty-three articles published between March 2010 and June 2021 were selected from 195 initially identified records. The findings are discussed in terms of the prevalence of

The Pediatric Sleep Questionnaire described by Chervin et al. (Sleep Medicine, 2000, 1, 21-32) was originally validated for children with obstructive sleep apnoea syndrome but without other disorders. The aim of our study was to check the applicability of this questionnaire in children with underlying chronic medical conditions. Children aged 2-18 years who underwent a diagnostic sleep study at Great Ormond Street Hospital were recruited over a 10-month period. The Pediatric Sleep Questionnaire completed by their parents and cardiorespiratory polygraphy were scored. Sensitivities and specifici

BACKGROUND: Post-stroke sleep disorders (PSSD) are under-reported and under-treated, despite their impact on recovery, quality of life, and post-stroke depression. Although polysomnography (PSG) is the gold standard for diagnosis, its limited availability leads to underdiagnosis.
AIMS: To assess the prevalence of PSSD based on patient- and caregiver-reported data, and to evaluate their concordance with findings from overnight PSG in post-stroke patients.
METHODS: In this cross-sectional study, adult patients (aged ≥18 years) with ischemic or hemorrhagic stroke (1 month to 1 year post-onset)

PURPOSE OF REVIEW: Summarize the effects of opioids on sleep including sleep architecture, sleep disordered breathing (SDB) and restless legs syndrome.
RECENT FINDINGS: Opioids are associated with the development of central sleep apnea (CSA) and ataxic breathing. Recent reports suggest that adaptive servo-ventilation may be an effective treatment for CSA associated with opioids.
SUMMARY: Opioids have multiple effects on sleep, sleep architecture and SDB. Although originally described with methadone use, most commonly used opioids have also been shown to affect sleep. In patients on chronic m
