Spondylolisthesis and Scoliosis Progression and Associated Revision Rates Following Bilateral Lumbar Spine Microscopic Decompression.

Lumbar spine microscopic decompression (LSMD) is a common surgical procedure for decompressing neural elements. Although the optimal extent of decompression remains a critical consideration, limited evidence-based guidelines define the threshold for instrumented fusion to maintain biomechanical stability. Existing studies suggest that unilateral LSMD generally does not result in iatrogenic instability. However, the potential instability associated with bilateral segmental decompression (BLSMD) is less well-defined, particularly in patients with pre-existing degenerative lumbar scoliosis (SC) o…

Full-endoscopic spine surgery in oldest old patients aged over 90 years:A case report.

BACKGROUND: Transforaminal full-endoscopic spine surgery (FESS) is the least invasive spinal surgery and can be performed under local anesthesia. In Japan, the population is rapidly aging and the number of spinal surgeries performed in the elderly is also increasing. OBJECT: In this report, we describe 3 patients aged 90 years or older in whom we performed FESS under local anesthesia. CASE: The first case was a 90-year-old man who presented with severe leg pain. He had multiple medical comorbidities and was unsuitable for general anesthesia. We performed FESS. After surgery, the leg pain res…

Full-endoscopic lumbar spine surgery using working-channel endoscopes: technical tips for practical effectiveness.

INTRODUCTION: Full-endoscopic spine surgery (FESS) has emerged as an effective and minimally invasive option for the surgical treatment of degenerative lumbar spine disease. FESS can be characterized as endoscopic spine surgery conducted via the percutaneous uniportal approach using working-channel endoscopes under continuous saline irrigation. Despite available evidence, the clinical application of this endoscopic procedure may require more work for standard spine surgeons. Therefore, this review aims to provide comprehensive technical tips to ensure the practical effectiveness of FESS. AREA…

Nonlaminotomy bilateral decompression: a novel approach in biportal endoscopic spine surgery for spinal stenosis.

Biportal endoscopic spine surgery (BESS) is an emerging technique for lumbar spinal stenosis. Previous BESS techniques involve partial osteotomy for access to spinal canal such as partial laminotomy, partial facetectomy, and other forms to access the spinal canal for decompression. However, approaches that include osteotomy can cause bone bleeding intraoperatively, leading to obscured vision, and may be at risk of postoperative facet arthritis and segmental instability due to damage to the posterior stability structure. This study aimed to introduce a BESS technique, i.e., nonlaminotomy bilate…

Biomechanical Characteristics of Kissing Spine During Extension Using a Human Cadaveric Lumbar Spinal Model.

INTRODUCTION: Although kissing spine syndrome in the lumbar spinal region is a relatively common condition in older adults, no study examining its biomechanical characteristics has been reported. We hypothesized that kissing of the spinous processes during extension causes an increase in the flexural rigidity of the spine and significantly limits the deformation behavior of extension, which in turn might cause lower back pain. METHODS: Three test models (human cadavers A, B, and C) were prepared by removing supraspinal/interspinous ligaments between L4 and L5. The dental resin was attached to…

Severe left kidney laceration after minor fall in a patient with lumbar spine scoliosis.

BACKGROUND: The kidneys are one of the organs most commonly affected by blunt trauma; however, the anatomical variations that predispose kidney injury are poorly understood. CASE PRESENTATION: An 80-year-old Japanese woman with lumber scoliosis was brought to our hospital because of severe flank pain after a minor fall at home. Contrast-enhanced computed tomography showed a severe left kidney laceration at the level of an acute angle in the vertebral column caused by scoliosis. Extravasation of contrast medium from the left renal artery branches was also noted. Selective arterial embolization

Intracranial solitary fibrous tumor with delayed symptomatic metastasis to the lumbar spine: illustrative case.

BACKGROUND: Intracranial solitary fibrous tumors (SFTs), formerly known as hemangiopericytomas, are rare, aggressive mesenchymal extra-axial tumors typically treated via resection, often with preoperative embolization and postoperative radiation and/or antiangiogenic therapy. Although surgery confers a significant survival benefit, local recurrence and distant metastasis are not uncommon and may occur in a delayed fashion. OBSERVATIONS: The authors describe the case of a 29-year-old male who initially presented with headache, visual disturbance, and ataxia, and was found to have a large right

Cervical Spine Instability Screening Tool Thai Version: Assessment of Convergent Validity and Rater Reliability.

Neck pain, dizziness, difficulty supporting the head for an extended period, and impaired movement are all symptoms of cervical spine instability, which may produce cervical spondylolisthesis in patients who have more severe symptoms. To avoid problems and consequences, early detection of cervical spine instability is required. A previous study created a Thai-language version of a cervical spine instability screening tool, named the CSI-TH, and evaluated its content validity. However, other characteristics of the CSI-TH still needed to be evaluated. The objective of the current study was to as…

Association Between Industry Sponsorship of Spine-Related Clinical Trials, Publication Status, and Research Outcomes.

STUDY DESIGN: Observational Database Study. OBJECTIVES: Prospective clinical trials in spinal surgery are expensive to conduct, especially when randomized, appropriately powered, and/or multicentered. Industry collaborations generate symbiotic relationships promoting technological advancement; however, they also allow for bias. To the authors' knowledge, there is no known analysis of correlations between industry sponsorship and publication rates of spine-related clinical trials. This observational work evaluates such potential associations. METHODS: The ClinicalTrials.gov database was queri…

[Our experience with minimally invasive decompression spine surgeries performed with microscopic and endoscopic visualization].

Introduction: The rapid development of minimally invasive techniques did not leave neurosurgery untouched either. New surgical interventions, both skull and spine surgery, are gaining more and more popularity. Due to the aging society, previously inoperable degenerative abnormalities are also receiving more and more attention. In the treatment of spinal canal stenosis caused by degenerative changes, the initially used laminectomy was replaced by unilateral “over-the-top” surgery, and in recent years, by techniques that ensure the least possible collateral tissue damage, such as tubular retract…

Vertebral Compression Fracture After Spine Stereotactic Body Radiotherapy: The Role of Vertebral Endplate Disruption.

BACKGROUND AND OBJECTIVES: Vertebral compression fracture (VCF) is a common, but serious toxicity of spinal stereotactic body radiotherapy (SBRT). Several variables that place patients at high risk of VCF have previously been identified, including advanced Spinal Instability Neoplastic Score (SINS), a widely adopted clinical decision criterion to assess spinal instability. We examine the role of tumoral endplate (EP) disruption in the risk of VCF and attempt to incorporate it into a simple risk stratification system. METHODS: This study was a retrospective cohort study from a single instituti
