[Burkitt's leukemia presenting as atypical acute mastoiditis in a 7-month-old child].

A 7-month-old girl presented with left acute mastoiditis and a white blood cell count of 79,000/mm(3). A surgically obtained bacteriological sample showed Streptococcus pneumoniae. Direct blood examination revealed leukemic blast cells. Histological samples showed leukemic infiltration of the left temporal bone. The final diagnosis was Burkitt's leukemia with left temporal bone leukemic infiltration. The patient received chemotherapy according to the LMB 2001 protocol from the Société française d'oncologie pédiatrique. Clinical, biological, and imaging follow-up 30 months after the end of trea…

Serotype and antimicrobial profile distribution of invasive pneumococcal isolates in the pre-vaccine introduction era in Pretoria, South Africa, 2005 through 2009.

A description of invasive Streptococcus pneumoniae isolates over a 5-year period from blood culture and cerebrospinal fluid culture follows, in Pretoria South Africa January 2005 through December 2009. Isolates were identified using standard microbiological techniques, serotyped, and a MIC determined for penicillin and cefotaxime. A total of 177 isolates were included in the analysis. Eighty percent of patients in the 18- to 45-year age group tested positive for HIV. In children <5 years of age, 66% (n = 49) of serotypes were those present in the heptavalent pneumococcal conjugate vaccine (PCV…

Potentiation of TLR9 responses for human naïve B-cell growth through RP105 signaling.

Toll-like receptor 9 (TLR9) signals induce important pathways in the early defense against microbial pathogens. Although TLR9 signaling can … B cells showed enhancement of proliferation/survival by RP105 and produced specific IgM antibody to Streptococcus pneumoniae polysaccharides in response to interleukin-21 stimulation. Thus, RP105 strongly affects expansion of the naïve B-cell pool, and suggests that the putative RP105 ligand (s) upon cytokine stimulation facilitates antibody-mediated acute pathogen cleara …

Human immunodeficiency virus and respiratory disorders: clinical and diagnostic considerations.

Respiratory infections are a major health burden for the people of Papua New Guinea (PNG) who are positive for human immunodeficiency virus (HIV). In the face of an ongoing HIV epidemic, little is known about the epidemiology and aetiology of respiratory infections in people living with HIV in PNG. In this article we provide an overview of the most important respiratory pathogens in HIV-positive people globally, focusing primarily on adults. Particular attention is given to respiratory viruses, bacterial pathogens such as Streptococcus pneumoniae and Mycobacterium tuberculosis, and Pneumocysti…

Pneumococcal conjugate vaccination and otitis media: differences among high-risk and low-risk populations.

The impact of the heptavalent pneumococcal conjugate vaccine (PCV7) on the natural history of otitis media (OM) has been studied recently. In a high-risk population, PCV7 significantly reduced the risk of recurrent, bilateral or multiple tympanic membrane perforation, as well as the risk of chronic suppurative OM at 9 months of age. PCV7 did not reduce the time to the development of OM nor the burden of OM. Major challenges are the polymicrobial etiology of OM (Streptococcus pneumoniae and Haemophilus influenzae) in high-risk populations and the serotype replacement among pneumococcal strains.…

Dalbavancin, a long-acting lipoglycopeptide for the treatment of multidrug-resistant Gram-positive bacteria.

Dalbavancin is a new lipoglycopeptide antibiotic in late-stage clinical development as a once-weekly treatment for serious infections including skin and skin structure infections. Its in vitro potency is greater than that of vancomycin, with a MIC(90) of 0.06 mg/l for Staphylococcus aureus and coagulase-negative staphylococci (irrespective of oxacillin susceptibility), 0.06-0.12 mg/l for vancomycin-susceptible Enterococcus spp. and 0.003 mg/l or less for Streptococcus pneumoniae or beta-hemolytic streptococci. Dalbavancin has dual routes of elimination. The results of Phase II/III studies show…

[Hemolytic and uremic syndrome in the child].

Hemolytic and uremic syndrome (HUS) is characterized by the association of hemolytic anemia with sckizocytes, thrombopenia and renal involvement secondary to thrombotic microangiopathy. The typical form is the most frequent in children occurring after an episode of diarrhea caused by Escherichia coli. Other microorganisms may be responsible for HUS such as Shigella dysenteriae or streptococcus pneumoniae. Acute renal failure is reversible in most cases, but long-term renal sequelae are seen in one third of patients when the anuria at onset of disease has lasted more than one week. Treatment of…

Antibody response to 7-valent conjugated pneumococcal vaccine in patients with chronic lymphocytic leukaemia.

Chronic lymphocytic leukaemia (CLL) is a common adulthood mature B-cell neoplasm. Infections are the most important cause of mortality in this condition, and Streptococcus pneumoniae has been considered the most important single pathogen. We investigated the immunogenicity of 7-valent pneumococcal conjugate vaccine in patients with CLL. The study material comprised 52 patients with CLL and 25 age- and sex-matched controls. The subjects were vaccinated with Prevenar pneumococcal conjugate vaccine. Serum samples were taken for antibody determinations before and four weeks after vaccination. Anti…

Antimicrobial activity of Substance P and Neuropeptide Y against laboratory strains of bacteria and oral microorganisms.

Infection and inflammation of mucosal tissue may induce the production of neuropeptides, specifically Substance P and Neuropeptide Y. Since these neuropeptides are similar to antimicrobial peptides in their amino acid composition, amphipathic design, cationic charge, and size, we wanted to determine if they had antimicrobial activity against a panel of common bacteria and oral microorganisms using the radial diffusion assay. Neuropeptide Y and Substance P had antimicrobial activity against E. coli (MIC 20.6+/-5.5 microg/ml SEM and 71.5+/-15 SEM microg/ml, respectively), but did not have activi
