[Disorders of calcium metabolism during pregnancy and breast feeding].

Hypoparathyroidism following thyroidectomy or due to autoimmune process is the most common cause of hypocalcaemia. If untreated, maternal hypocalcaemia may stimulate fetal parathyroids which results in bone demineralization. Calcium supplementation may also reduce the risk of hypertension. Moreover, hypocalcaemia may inhibit the synthesis of calcium gene-related peptide (CGRP), which, in turn, decreases blood pressure and uterus contractions. In women of reproductive age hypercalcaemia is mostly due to hyperparathyroidism. The complications include toxemia, vomiting, and hypercalcaemic crisis.

The ins and outs of periodontal antimicrobial therapy.

A multifaceted antimicrobial approach is necessary for the successful management of destructive periodontal disease. Effective antimicrobial periodontal therapy aims to overwhelm periodontal pathogens with aggressive initial therapy and prevent previously suppressed pathogens from rising up anew through daily oral hygiene measures and frequent professional cleaning. Current antimicrobial periodontal therapy employs mechanical debridement performed with and without surgery, antibiotics, and antiseptics. Subgingival irrigation with povidone-iodine at the dentist's office and subgingival irrigati

Use of abdominal ultrasonography in the diagnosis of primary hyperaldosteronism in a cat.

A 13-year-old castrated male cat was examined because of a 2-week history of weakness, cervical ventroflexion, and dysphagia. Clinicopathologic abnormalities included hypokalemia and high serum creatine kinase activity. Abdominal ultrasonography revealed a 15-mm spherical mass in the area of the left adrenal gland. Plasma aldosterone concentration was high, and plasma renin activity was low. Findings were diagnostic of primary hyperaldosteronism. The cat responded well to intravenous and oral potassium supplementation while in the hospital. The owner declined surgery; therefore, repeated follo

Hepatocellular carcinoma in pregnancy. A case report.

BACKGROUND: Hepatocellular carcinoma presenting in pregnancy is very rare, and experience treating this condition is limited. Past reports have emphasized the need for pregnancy termination as part of treatment. CASE: A young, otherwise healthy woman was diagnosed with hepatocellular carcinoma in the second trimester. Her only risk factor was a six-year history of oral contraceptive use. She opted to maintain the pregnancy and was treated with surgical resection in the form of a right hepatectomy. Both she and her child were alive and well 24 months following surgery. CONCLUSION: Hepatocellu

Dumping syndrome in a young Jamaican child.

The dumping syndrome in childhood is an uncommon complication of gastro-oesophageal surgery, principally Nissen fundoplication. A Jamaican child developed the syndrome after fundoplication and pyloroplasty to relieve gastro-oesophageal reflux complicating the repair of a congenital tracheo-oesophageal fistula. He developed marasmus and failed to gain weight on the standard remedial milk-based high-energy diet. An oral glucose tolerance test confirmed the diagnosis of dumping syndrome. A low sugar low milk diet based on adult type meals with continuous nibbling of fried dumplings relieved his d

Persistent dysphagia and dysphonia following cervical spine surgery.

Persistent dysphagia and dysphonia following anterior cervical spine surgery have been underdiagnosed. Understanding the physiologic mechanism resulting in post-surgical dysphagia and dysphonia is essential to providing appropriate treatment. Two cases of protracted dysphagia and dysphonia following left anterior cervical corpectomy were reviewed. Videofluoroscopic evaluation, videostroboscopic assessment and laryngeal nerve-conduction testing were used to determine swallowing, vocal fold and cranial nerve integrity; to assess the ability to achieve oral nutrition; and to direct therapy and su

Contact laser vaporization of the prostate in patients on chronic anticoagulant therapy.

The patient with marked bladder outlet obstructive symptoms who is taking anticoagulant medication presents a challenge to the urologist. This report consists of six such patients requiring full anticoagulation with oral warfarin. All patients were treated with the Surgical Laser Technologies neodymium: YAG contact laser using the sapphire tip, while they were fully anticoagulated. All patients had their catheters removed the morning following surgery, and were discharged that same morning. On follow-up all are voiding with excellent streams and have marked reduction in their bladder outlet ob

Mycoses of the urogenital tract.

Mycotic infections of the urogenital tract are rarely caused by primary fungal pathogens, often however, by opportunistic fungi such as Candida species. Opportunistic fungi infect the urogenital tract either by haematogenous spread from a distant site, or a break of tissue integrity, or by retrograde ascension from the perianal region, or by wound contamination. After an initial polymorphonuclear reaction macrophages may invade giving rise to a granulomatous response. Risk factors for mycotic urogenital infections are extremes of age, long lasting and broad spectrum antimicrobial therapy, diab

Preoperative vascular assessment: an aid to radial forearm surgery.

Twenty-three patients undergoing oral reconstructive surgery with radial forearm free flaps had a vascular assessment of the forearm preoperatively. Segmental upper limb pressures were measured and colour flow duplex visualization of forearm vessels and blood flow done. In 18 the blood pressure and flow were within normal limits, but five (22%) showed either unilateral or bilateral arteriopathy or aberrant vascular anatomy. Identification of pre-existing vascular disease helps to rationalize the selection of donor vessels, reduces the risk of ischaemic damage to the hand after disruption of th

Aetiology of maxillofacial fracture.

The aetiological factors associated with maxillofacial fractures, and the trends in these factors over a 13 year period are reported. The First Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Tokyo Medical and Dental University, managed 695 patients with maxillofacial fractures between 1977 and 1989. The male to female ratio was 3.2:1 and the majority of patients were aged between 10 and 30 years old. Road traffic accidents and accidental falls were the main causes of fractures throughout the 13 year study period. Mandibular fractures occurred in 477 patients (68.6%). A hig

Wegener's granulomatosis, pituitary adenoma and BARN.

A 53-year-old man with Wegener's granulomatosis and a co-existing pituitary adenoma developed bilateral acute retinal necrosis (BARN), probably secondary to varicella-zoster virus (VZV) infection as IgM antibodies were detected in the serum. Intravenous acyclovir and ganciclovir limited the spread of necrosis, but to prevent recurrence he was maintained on oral acyclovir. A left cataract developed 17 months later which was extracted and replaced with a heparin surface modified intraocular lens. Intraocular specimens removed at the time of surgery were analysed by the polymerase chain reaction

Partial mandibulectomy for treatment of mandibular fractures in dogs: eight cases (1981-1984).

Partial mandibulectomy was performed in 8 dogs with open or infected mandibular fractures (3 second degree and 5 third degree) because of economic restriction, osteomyelitis, or severe bone and soft tissue injury. Six dogs ate voluntarily by the second day after surgery, and 2 dogs required pharyngostomy tube feeding for 2 to 6 days. Complications included oral wound dehiscence (3 dogs), shifting of the mandible toward the operated side (6 dogs), and drooping of the tongue (2 dogs). Slight malocclusion was of no consequence, and all dogs were eating normally at follow-up evaluations (10 days t

Persistent succinylacetone excretion after liver transplantation in a patient with hereditary tyrosinaemia type I.

A liver transplant was performed on a 4-year-old female in liver failure caused by hereditary tyrosinaemia, with hepatocellular carcinoma following a negative evaluation for metastases. However, serum alpha-fetoprotein levels never returned to normal after the surgery. Urinary succinylacetone (SA) was detected in her urine prior to transplantation despite strict adherence to a low-tyrosine diet. Other patients with severe liver disease awaiting liver transplantation do not excrete SA in the urine. She continued to excrete SA during the postoperative period despite normal liver functions. Vitamin A deficiency following small-bowel bypass surgery for obesity.

Side effects are common in individuals who have undergone small-bowel bypass surgery for morbid obesity. Most of these side effects subside after a few months. More serious complications may require reestablishment of the bypassed loop. Rarely have vitamin deficiencies been observed. Development of vitamin A deficiency in a patient was characterized by phrynoderma and night blindness. This was promptly reversed by oral treatment with vitamin A. There was no improvement in low to subnormal serum levels of fat-soluble vitamins after a course of tetracycline hydrochloride. Studies suggested prese

Conscious sedation with intravenous drugs: a study of amnesia.

A comparison of amnesia that is produced by two intravenous sedation techniques commonly used to reduce anxiety in ambulatory oral surgery patients showed that both techniques induce high levels of safe, predictable amnesia. More than 200 patients undergoing surgical extraction of third molars were asked to verbally identify visual and cutaneous-tactile stimuli intraoperatively. The patients responded via written questionnaires at the postoperative interview sessions. No retrograde amnesia was observed. Complete amnesia was attained for the local anesthetic injections. Amnesia for visual stimu

Development of relugolix combination therapy as a medical treatment option for women with uterine fibroids or endometriosis.

Treatment of uterine fibroids (UF) and endometriosis (EM) has relied on the surgical skills of gynecologists to improve symptoms and potentially alter the course of these debilitating diseases. Medical management of symptoms for both diseases leverages combined hormonal contraceptives used off label as a first-line treatment, with nonsteroid anti-inflammatory drugs and opioids to manage pain as needed. Gonadotropin-releasing hormone (GnRH) receptor agonists (peptide analogs) have been used as short-term therapy to manage severe symptoms of UF or EM, treat anemia, and reduce fibroid size before

Calibre-persistent artery: a vascular anomaly of the labial mucosa.

The aim of this study was to characterise the clinical and histologic features of calibre-persistent arteries (CPAs). A retrospective search of the archives of our diagnostic oral pathology service was performed for CPAs diagnosed between 2010 and 2025.There were 45 CPAs in 44 patients. The female-to-male ratio was 1.6:1, with a mean age of 50.8 years. The majority of CPAs occurred on the lower lip and most were described as blue or pink in colour. Lesions were usually asymptomatic; however, some patients reported fluctuations in size or drainage. A history of trauma or irritation was reported

Therapeutic Potential of Dimethyl Fumarate in Counteract Oral Squamous Cell Carcinoma Progression by Modulating Apoptosis, Oxidative Stress and Epithelial-Mesenchymal Transition.

Oral squamous cell carcinoma (OSCC) is a common human tumor, that originates from buccal mucosa and the tongue, associated with a high mortality rate. Currently, the treatment for OSCC involves surgery, chemotherapy and radiotherapy; however, survival outcomes for OSCC patients remain poor. For this reason, it is necessary to investigate new therapeutic strategies to counteract the progression of OSCC. In this study, we aimed to evaluate the role of dimethyl fumarate (DMF) in modulation of OSCC progression, both in vitro and in an in vivo orthotopic xenograft model. In vitro results revealed t

Stress-strain and fatigue life numerical evaluation of two different dental implants considering isotropic and anisotropic human jaw.

Dental prostheses are currently a valid solution for replacing potential missing tooth or edentulism clinical condition. Nevertheless, the oral cavity is a dynamic and complex system: occlusal loads, external agents, or other unpleasant events can impact on implants functionality and stability causing a future revision surgery. One of the failure origins is certainly the dynamic loading originated from daily oral activities like eating, chewing, and so on. The aim of this paper was to evaluate, by a numerical analysis based on Finite Elements Method (FEM), and to discuss in a comparative way,

A 20-year-old male presented to our facility with a worsening sensation of "the room spinning around" himself for the past three weeks. In the last week, he began to experience daily spells lasting for three hours each without losing consciousness. The patient had recently migrated from Central America six weeks prior to admission. On physical examination, his vital signs were within normal limits, with no focal neurological deficits. Magnetic resonance imaging (MRI) of the brain revealed a cystic-appearing lesion in the fourth ventricle with associated mass effect on the posterior aspect of t
