Hips With Protrusio Acetabuli Are at Increased Risk for Failure After Femoroacetabular Impingement Surgery: A 10-year Followup.

BACKGROUND: Protrusio acetabuli is a rare anatomic pattern of the hip in which the femoral head protrudes into the true pelvis. The increased depth of the hip and the excessive size of the lunate surface typically lead to severe pincer-type femoroacetabular impingement (FAI); however, to our knowledge, there are no published mid- or long-term studies on results of circumferential acetabular rim trimming through a surgical hip dislocation for patients with this condition. QUESTIONS/PURPOSES: (1) What is the 10-year survivorship of the hips treated with circumferential rim trimming through a su

Scaphoid waist nonunion with humpback deformity treated without structural bone graft.

PURPOSE: Conventional treatment of scaphoid nonunion with collapse deformity and bone loss involves the use of a structural corticocancellous bone graft. With internal screw fixation, we propose that length and alignment can be restored and maintained using only cancellous interposition graft. METHODS: We describe 12 patients with established scaphoid waist nonunions. Average patient age was 22 years, all were male, and 8 cases involved the dominant wrist. Time from injury to treatment averaged 11 months. We performed open reduction through an anterior approach with correction of the deformit

Analysis of two different surgical approaches for fractures of the mandibular condyle.

BACKGROUND: Fractures of the condyle account for one third of all the mandibular fractures. Different surgical approaches to the condyle described hitherto testify to the advantages and disadvantages of the different surgical techniques used for approaching the condyle in such cases of fractures. We have described and compared two of such surgical techniques in this study. AIM: The aim of this study is to compare the outcome of dealing with condylar fractures by two different surgical techniques: the mini retromandibular approach, and the preauricular approach. MATERIALS AND METHODS: A prosp

Direct comparison of polyethylene wear in cemented and uncemented acetabular cups.

BACKGROUND: It has been indicated that, in the long term, the rate of wear and the degree of osteolysis observed with uncemented acetabular components are greater than those associated with cemented cups, but most studies which compare the wear characteristics of cementless with cemented cups have used historical controls. We report a direct comparison of wear of a cemented and an uncemented cup with similar design, polyethylene, and sterilization method. MATERIALS AND METHODS: The study cohort includes 92 patients who were operated in 1997 with primary total hip replacement and have been fol

Iliac bone-block autograft for posterior shoulder instability.

INTRODUCTION: Posterior shoulder instability is a rare condition, representing only 4% of all shoulder-joint instabilities. Numerous surgical techniques are used to treat it when conservative functional treatment proves to be insufficient. This retrospective study relates to 8 patients, presenting recurrent posterior shoulder instability, all treated with a posterior iliac bone-block procedure. The results were assessed both clinically and with contemporary imaging techniques. MATERIALS AND METHODS: A unique identical surgical technique was used in all these cases including a posterior deltoi

Neer Award 2008: Arthropathy after primary anterior shoulder dislocation--223 shoulders prospectively followed up for twenty-five years.

BACKGROUND: Shoulder dislocation may cause arthropathy, but the natural history of this evolution is not well described. We therefore conducted a radiographic follow-up 25 years after the primary shoulder dislocation. METHODS: A prospective Swedish multicenter study (1978-1979) included 257 shoulders in 255 patients (age, 12-40 years) with a first-time anterior shoulder dislocation. After 25 years, 227 patients (229 shoulders) were alive and had follow-up. Radiographic imaging was performed in 223 shoulders (97%). RESULTS: Shoulders were normal in 44%. Arthropathy was mild in 29%, moderate i

Transtrochanteric posterior rotational osteotomy for osteonecrosis.

When osteonecrosis is located in the mid- to posterior region, we generally perform a transtrochanteric posterior rotational osteotomy. We retrospectively reviewed the clinical and radiographic results in 47 consecutive patients (51 hips) in whom we performed posterior rotational osteotomies. The average age was 37 years at the time of surgery. There were 30 male and 17 female patients. Thirty-six hips were ARCO Stage III, and 15 were Stage IV. Conversion to THA was defined as the failure end point. Three patients died and one was lost to followup. We were therefore able to follow 43 patients

Clinical outcome of a fibermetal taper stem: minimum 5-year followup.

Tapered femoral components were designed to provide a cementless alternative in patients with narrow femoral canals. Major concerns with this stem include perioperative fracture and thigh pain. To ascertain the frequency of these concerns, we retrospectively reviewed 100 consecutive hips in 97 patients who underwent total hip arthroplasty with a tapered fibermetal femoral component. Patients were evaluated postoperatively using Harris hip scores and sequential radiographs. The average age of the patients was 56.5 years (range, 33.7-73.5 years). Minimum followup was 5 years (average, 6.2 years;

Resurfacing for Perthes disease: an alternative to standard hip arthroplasty.

Metal-on-metal total hip resurfacing is an alternative to conventional total hip arthroplasty with several reports describing the benefits of this procedure in young patients. We retrospectively compared the clinical (including range-of-motion and leg length restoration) and radiographic outcome of resurfacing in young patients with Legg-Calvé-Perthes to those of patients of a similar age treated with a standard total hip arthroplasty. Eighteen patients (19 hip resurfacings) who had a mean age of 33 years (range, 18-34 years) were followed for a minimum of 26 months (mean, 51 months; range, 26

Extended Reality Biofeedback for Functional Upper Limb Weakness: Mixed Methods Usability Evaluation.

BACKGROUND: The perception-action cycle enables humans to adapt their behaviors by integrating sensory feedback into motor actions. Functional neurological disorder (FND) disrupts this cycle, leading to maladaptive motor responses and a diminished sense of agency. FND includes functional seizures, movement disorders, and cognitive impairments, significantly affecting quality of life. Recent advancements in extended reality (XR) neurotechnologies provide opportunities for novel rehabilitation approaches, leveraging visual and haptic feedback to retrain motor control and restore agency in indivi

Is Dorsal Vertical Double Plating an Effective Alternative to Volar Plating for Distal Radius Fractures With Dorsal Collapse?

OBJECTIVE: Dorsal articular collapse in distal radius fractures presents unique fixation challenges. While volar locking plating (VLP) dominates current practice, dorsal vertical double plating (DVDP) offers direct biomechanical support but carries perceived tendon risks. This study compares DVDP versus VLP for dorsally collapsed comminuted fractures. METHODS: A retrospective cohort of 106 patients (2022-2024) with AO type C2/C3 fractures received either VLP (n = 50) or DVDP (n = 56). General information encompassed gender, age, injured side, injury mechanism, AO classification, time from inj

A standardized rehabilitation protocol following two-stage surgery for nonunion of lateral humeral condyle fracture with cubitus valgus in children: functional and radiographic outcomes.

OBJECTIVE: Treating nonunion of the lateral humeral condyle with cubitus valgus deformity in children is challenging. While various surgical strategies exist, the role of a structured, standardized rehabilitation protocol in achieving optimal functional outcomes remains underexplored. This retrospective study aimed to evaluate the clinical and functional results of a two-stage surgical strategy (primary in situ fixation with bone grafting followed by secondary corrective osteotomy) coupled with a standardized, closely monitored rehabilitation protocol for pediatric lateral humeral condyle nonu

Humanoid Robot Walking and Grasping Method Using Similarity Reward-Augmented Generative Adversarial Imitation Learning.

This study aims to enhance the precision of humanoid robots in imitating complex human "walking-grasping" coordinated movements. Addressing limitations in sample efficiency and reward function design in Generative Adversarial Imitation Learning (GAIL), we propose the Similarity Reward-Augmented Generative Adversarial Imitation Learning (SRA-GAIL) framework. The method integrates plantar thin-film resistive pressure sensors to measure the real-time pressure distribution at four key points on both feet, combined with roll/pitch angle data acquired from JY901S inertial measurement units (IMUs).
