OBJECTIVE: To explore the method of diagnosis for uterine septum and the clinical effect of hysteroscopic transcervical resection of the septum.
METHODS: One-hundred ninety cases of patients with uterine septum who were diagnosed and treated at our hospital during 2007-2011 were selected, and their general information, perioperative status, postoperative recovery treatment, and postoperative pregnancy rates were statistically analyzed.

The authors shows detailed classification of Buttram and Gibbons for the Muller uterine anomalies. According this classification the anomalies are classified in 6 different groups based on the stage of insufficiency in the normal development. Many theoretical arguments have been made on the different malformations in groups. The stress has been put on the clinical view and the choice of a surgery behavior. Some special cases are described which caused difficulties with the diagnosis and the treatment. The anomalies in the different groups are illustrated in different figures.

In 12 human embryos and 76 fetuses the uterine ligaments were investigated. The round ligament appears at 8th week and the broad ligament is observed in 19th week. Up to 17th week the round ligament is connected with the uterine tube. It arises separately from the border of the uterus beginning from the 18th week. In fetuses of 11 and 12 weeks the broad ligament forms a thick plate enclosing vessels and nerves. The cardinal ligament appears at 10th week.

This paper reports the improved technics in 578 abdominal total hysterectomies. After dividing the infundibulo-pelvic ligaments and both layers of the broad ligaments the ovarian vessels were dissected out, cut and ligated, the utero-sacral ligaments revered but not ligated, and if necessary, the bleeding points were tied. After cutting the intervening tissues on both sides of the uterus, the uterine vessels could be identified, clamped and ligated at level below the cervical internal os. An endoscopic alternative to the surgical treatment of uterine septum is presented. The hysteroscopic technique of septal division is described on the basis of 7 preliminary cases, with emphasis on the equipment used, the choice of the irrigation system, the problems encountered near the fundus uteri, where it is important to spare the myometrium, and the adjuvant treatments, notably in the post-operative period. Interventional hysteroscopy has a very low trans--and post-operative morbidity and seems to be a suitable procedure for the treatment of this type of uterine malformation.

Twelve additional cases are added to our previous report on nonoperative construction of the congenitally absent vagina, bringing the total reported cases to 20, of which 19 have been treated successfully, We have not found it necessary to construct a vagina surgically with skin grafting since adopting this method.
