The gastric secretion of 99mTc-pertechnetate as a means of assessing parietal cell function was investigated in 12 pernicious anemia (PA) patients with histamine-fast achlorhydria. Twelve control subjects were either normal volunteers or patients proven not to have either PA or any other gastrointestinal disease. The results indicated that, while the stomachs of PA patients secreted no hydrochloric acid, they continued to secrete 99mTc-pertechnetate, indicating that 99mTcO4 - is secreted not by parietal cells alone or else not by parietal cells at all. HISTORY AND FINDINGS: A now 54-year-old woman was 32 years ago found to have immune thrombocytopenia and 3 years ago ANA-positive and HBsAg-negative hepatitis with cirrhotic metaplasia. Numerous small asymptomatic carcinoids with marked hypergastrinaemia (1626 ng/l) were also first found 3 years ago. No gastrinoma could be found. Severe arthralgia was the main symptom on admission.
INVESTIGATIONS: Gastroscopy revealed a polypoid carcinoid, 1 cm in diameter. There was total achlorhydria. No pernicious anaemia or carcinoid syndrome was found.

It is suggested that there are two hormonal syndromes associated with noninsulin-secreting islet cell tumours and this case is an example of the non-gastrin-secreting type with watery diarrhoea and hypokalaemia. The patient had histamine-fast achlorhydria and a normal gastric biopsy and no gastrin was recovered from the tumour tissue. The watery diarrhoea was isosmotic with plasma and was increased by an intravenous saline load. There was a dramatic response to steroids.

A mother and her identical twin daughters were found to have pernicious anaemia and achlorhydria. The twins lacked detectable serum IgA and had low to absent serum IgG. Both girls also demonstrated a marked impairment of antibody response to a variety of antigens. Evidence for lymphocyte sensitization to intrinsic factor was demonstrated in these patients by the increased incorporation of tritiated thymidine into DNA and by the production of migration inhibition factor.

A case of non-beta islet cell tumor of the pancreas with elevated vasoactive intestinal polypeptide (VIP) levels and WDHA syndrome (watery diarrhea, hypokalemia, and achlorhydria) was studied. Angiography and radio isotope scanning showed profuse abnormal vascularity. Ultrasonography showed a solid mass with small- to medium-sized cystic spaces. Histologic examination with special staining (Sevier-Munger) showed carcinoid tumor.

A 10-year-old girl with genetically confirmed Jervell-Lange-Nielsen syndrome treated with beta-blocker and developed electrical storm after changing propranolol syrup to tablets. Jervell-Lange-Nielsen is characterised by long QT and congenital sensorineural deafness, with high risk of malignant arrhythmias at early ages. Gastric involvement and achlorhydria may be present, with subsequent alteration of medication bioavailability which can trigger severe arrhythmic complications.

Vitamin B12 deficiency develops over a slowly progressive continuum. Early manifestations may be generalized weakness or fatigue, indigestion, diarrhea, or depression. Pernicious anemia is considered the classic cause, but others include malabsorption because of achlorhydria or other gastric dysfunction, fish tapeworm infection, and strict vegetarianism. Iron deficiency often coexists. Because presentation is often atypical, vitamin B12 deficiency is a diagnostic consideration whenever neuropsychiatric signs or symptoms are unexplained.

Pneumonia, in infants suffering from esophageal atresia with lower esophageal fistula, is usually caused by gastric reflux through the fistula, In order to abolish the acidity of the gastric content, Cimetidine i.v. infusion was used during the first 12--48 hr after diagnosis, while treating the pneumonia. The amount of Cimetidine required to induce achlorhydria was established by serial aspiration of gastric content, in infants and children with gastrostomies performed for various causes.

Idiopathic late-onset immunoglobulin deficiency in a young man was associated with achlorhydria and a severe intestinal malabsorption syndrome that did not respond to conventional therapy. Combined therapy with high doses of prednisone and tetracycline hydrochloride resulted in weight gain, cessation of diarrhea, improved absorption of water, fat, and vitamin B12, and production of gastric acid after stimulation with histamine. Serum immunoglobulin levels, however, did not increase.

The authors report a potentiating effect of sodium glutamate on gastric secretion in subjects free of gastrointestinal diseases. Similar effect has been discovered in dogs. In subjects with gastric hyposecretion (chronic gastritis, functional regulatory disturbances) sodium glutamate combined with pentagastrin is a helpful tool in overall evaluation of gastric secretion. In achlorhydria is can be used for determination of a residual capacity of the stomach to secrete the hydrochloric acid in failure of humoral stimulators.
