Medicines and the drug control treaties: is buprenorphine for opioid addiction at risk of being lost?

Over the past century, a worldwide system for the control of drugs with abuse potential has developed through the adoption of a series of international treaties. The important multilateral conventions currently in force are the United Nations Single Convention on Narcotic Drugs, 1961 (Single Convention), the United Nations Convention on Psychotropic Substances, 1971 (Psychotropic Convention) and the United Nations Convention Against Illicit Traffic in Narcotic Drugs and Psychotropic Substances, 1988. From the beginning, the aim of these drug control treaties has been to control the abuse and t

Opioid Addiction Medicine in Nigeria: Bridging Clinical Gaps in a Silent Epidemic-A Narrative Review.

Opioid use disorder (OUD) is an emerging public-health challenge in Nigeria, driven largely by non-medical use of tramadol and codeine and compounded by limited access to evidence-based care. This narrative review synthesises evidence on the epidemiology, patterns, harms and treatment gaps for OUD in Nigeria and identifies priorities for policy and research. We searched PubMed/MEDLINE, Embase, PsycINFO, the Cochrane Library, African Journals Online, Google Scholar and grey literature from 2005 to 2024 for Nigerian data on non-medical opioid use, clinical harms, opioid agonist therapy (OAT) ser

Integrated internist - addiction medicine - hepatology model for hepatitis C management for individuals on methadone maintenance.

Despite a high prevalence of hepatitis C virus (HCV) among drug users, HCV evaluation and treatment acceptance are extremely low among these patients when referred from drug treatment facilities for HCV management. We sought to increase HCV treatment effectiveness among patients from a methadone maintenance treatment program (MMTP) by maintaining continuity of care. We developed, instituted and retrospectively assessed the effectiveness of an integrated, co-localized care model in which an internist-addiction medicine specialist from MMTP was embedded in the hepatitis clinic. Methadone mainten…

Specifying and Pilot Testing Quality Measures for the American Society of Addiction Medicine's Standards of Care.

OBJECTIVES: In 2013, the American Society of Addiction Medicine (ASAM) approved its Standards of Care for the Addiction Specialist Physician. Subsequently, an ASAM Performance Measures Panel identified and prioritized the standards to be operationalized into performance measures. The goal of this study is to describe the process of operationalizing 3 of these standards into quality measures, and to present the initial measure specifications and results of pilot testing these measures in a large health care system. By presenting the process rather than just the end results, we hope to shed ligh…

Meconium drug testing reveals maternal misuse of medicinal opioids among addicted mothers.

Meconium drug testing is a non-invasive method to detect prenatal drug exposure, which can cause severe problems for the infant, indicating the need for follow-up measures to ensure the welfare of the child. Meconium samples for drug testing were collected from 143 infants as part of routine clinical work among addicted mothers. The drug testing findings were combined with medical records including clinical background and follow-up data. The substances screened for included medicinal opioids, 6-monoacetylmorphine (a metabolite of heroin), amphetamines and tetrahydrocannabinolic acid. At least

Impact of an addiction medicine consult service on medications for opioid use disorder initiation in hospitalized patients.

OBJECTIVE: The number of overdose deaths rose by 60 percent between 2019 and 2021. Medications for opioid use disorder (MOUD) have been associated with reduced overdoses and opioid-related hospitalizations. Hospital addiction medicine consult services (AMCSs) have been shown to reduce all-cause 90-day mortality post-discharge. This study aimed to expand on the impact of AMCS on the initiation of MOUD in inpatients. Design, setting, patients/participants, interventions, main outcome measure: This study is a retrospective, single site, multicenter, cohort study conducted at two sites. Utilizing …

Improving access to direct acting antivirals via a multimodal integrated care program in an addiction medicine clinic.

BACKGROUND AND OBJECTIVES: Injection drug use is a driver of hepatitis C virus (HCV) transmission, thus integrating HCV care into addiction care is likely necessary for HCV elimination. Here we describe how we integrated HCV care into our addiction medicine (AM) clinic and evaluate its effect on HCV treatment. METHODS: Integrated care involves HCV screening and treatment with either DAA prescription directly in AM clinic or via telemedicine with an infectious diseases (ID) specialist. Using retrospective chart review, we assessed if the program affected rates of direct-acting antivirals (DAA)…

A rapid access to addiction medicine clinic facilitates treatment of substance use disorder and reduces substance use.

BACKGROUND: Substance use is prevalent in Canada, yet treatment is inaccessible. The Rapid Access to Addiction Medicine (RAAM) clinic opened at the University Health Network (UHN) in January 2018 as part of a larger network of addictions clinics in Toronto, Ontario, to enable timely, low barrier access to medical treatment for substance use disorder (SUD). Patients attend on a walk-in basis without requiring an appointment or referral. We describe the RAAM clinic model, including referral patterns, patient demographics and substance use patterns. Secondary outcomes include retention in treatme…

Evaluation of an injectable monthly extended-release buprenorphine program in a low-barrier specialty addiction medicine clinic.

INTRODUCTION: Monthly injectable extended-release buprenorphine (XR-BUP) can address several systemic and individual barriers to consistent sublingual buprenorphine treatment for patients with opioid use disorder (OUD). Real-world evaluations of XR-BUP in the outpatient addiction treatment setting are limited. The purpose of this study was to compare 6-month treatment retention and urine drug tests between patients who initiated XR-BUP compared to those who were prescribed but did not initiate XR-BUP in a low-barrier addiction medicine specialty clinic. METHODS: We conducted a retrospective c…
