Communicating the Geriatrician's Brand in Age-Friendly Care.

The implementation of CMS age-friendly care initiatives has created new opportunities for geriatricians to shape clinical care, quality improvement, education, and health system strategy. A geriatrician's unique contribution extends beyond managing medical complexity; it lies in applying expertise in aging physiology, functional change, psychosocial context, heterogeneity, prognosis, patient priorities, and ageism to clinical and organizational decision-making. Geriatrics expertise must be visible, clearly communicated, and connected to the people and structures that guide organizational chang

COVID-19 worry, mental health indicators, and preparedness for future care needs across the adult lifespan.

OBJECTIVE: The COVID-19 pandemic has been a source of worry for many, but older adults have been identified as more vulnerable to serious cases and may therefore feel more concerned about the virus. We assessed whether COVID-19 worry was related to indicators of mental health and preparedness for future care, in an adult lifespan sample. METHOD: An online study (n = 485; age 18-82, M = 49.31, SD = 15.39) included measures of COVID-19 worry, depression, general anxiety, health anxiety, hostile and benevolent ageism, preparedness for future care, and demographic information. RESULTS: Age and l

From Clinical Perception to Implicit Bias: Understanding Personality Traits in Lymphoma Patients.

Background/Objectives: Some haematologists share the perception that patients with Hodgkin's lymphoma (HL) behave and manifest emotional expressions in a characteristic way. Previous research suggested a unique personality profile in HL patients compared to the general population. This study aimed to analyse and compare the personality traits of HL and NHL patients to identify potential differences. Methods: In this cross-sectional, descriptive, multicentre and replicative study, we included patients with HL and NHL from the Spanish Group of Lymphoma (GELTAMO). Personality traits and other psy

Aging women's sexual agency: Subverting the male gaze in Japanese pink film Lily Festival (2001).

This article critically examines Sachi Hamano's Lily Festival (2001) as a feminist and queer cinematic intervention within Japanese pinku eiga (softcore pornography), analyzing how the film challenges ageism and sexism through three interconnected strategies: subverting the male gaze, reconfiguring aging sexuality, and articulating an aging female/queer gaze. Methodologically, the article employs an integrated textual-contextual analysis, combining close reading of cinematography-such as obscured phallic imagery and extreme close-ups of aging erogenous zones-with critique of Japan's socio-cult

From Aging to Thriving: Redefining Psychiatric-Mental Health Professional Value in a Longevity Society.

The aging population is transforming societies worldwide, necessitating a shift from deficit-based models of care to approaches that promote thriving, purpose, and well-being across the lifespan. The current article reframes aging as an opportunity for innovation and value creation, highlighting the critical role of psychiatric-mental health professionals in leading this transformation. Four key pillars are emphasized-reframing aging as opportunity, enhancing communication as a catalyst for change, transitioning from reactive to proactive care, and leveraging technology while aligning innovati

Fitness for work and the aging workforce: medical assessment as a de facto mechanism of retirement in the United Kingdom after the abolition of the default retirement age.

BACKGROUND: Fitness for work is the judgement that a person is medically able to perform a role safely. This review considers the concept to have been shaped for a younger workforce expected to leave employment at a fixed retirement age. As populations have aged and mandatory retirement ages have been abolished, it is the instrument that remains available for questions it was not designed to answer. METHODS: This review traces the origins of the concept and examines what fitness for work means when applied to an aging workforce and the risks of relying on it at scale. RESULTS: Aging is gradu

Screening for cancer beyond recommended upper age limits: views and experiences of older people.

BACKGROUND: Internationally, screening programmes and clinical practice guidelines recommend when older adults should stop cancer screening using upper age limits, but it is unknown how older adults view these recommendations. OBJECTIVE: To examine older adults' views and experiences about continuing or stopping cancer screening beyond the recommended upper age limit for breast, cervical, prostate and bowel cancer. DESIGN: Qualitative, semi-structured interviews. SETTING: Australia, telephone. SUBJECTS: A total of 29 community-dwelling older adults (≥70-years); recruited from organisation

Clinical Blindness to Eating Disorders in Older Adults: A Qualitative Study of Primary Care Nurses' Perspectives.

Background/Objectives: Eating disorders (EDs) have historically been associated with young women, and their presence in older adults remains poorly recognized despite evidence that they can persist, recur, or emerge in later life. Primary care nurses are strategically positioned for community-based detection, yet their perspective has been scarcely explored. This study aimed to examine the experiences, perceptions, and described practices of primary care nurses regarding EDs and disordered eating in older adults, identifying patterns, barriers to detection, and training needs. Methods: A quali

Network Structure of Depression and Anxiety Symptoms in Older Asian Patients With Depressive Disorders: Findings From REAP-AD3.

OBJECTIVE: The clinical presentation of depressive disorders might be influenced by age, and its diagnosis and treatment can be affected by ageism-related bias. A network analysis can reveal symptom patterns unrecognized by the reductionistic approach. Therefore, this study explores the network structure of depression and anxiety symptoms in older Asian patients with depressive disorders and examines age-related differences in the context of ageism. METHODS: We used data from the Research on Asian Psychotropic Prescription Patterns for Antidepressants, Phase 3 study and included 2,785 psychia

Attitudes toward Organ Donation among Japanese American Older Adults: Implications from the Open-Ended Responses.

BACKGROUND: Despite the removal of upper age limits for organ donation and registration, little research has addressed the underrepresentation of donors among Asian American older adults. Japanese Americans, the fastest aging minority group in the U.S. and considered the most acculturated and assimilated Asian subgroup, offer a unique perspective. Understanding their attitudes toward organ donation may provide valuable insights into strategies for increasing donor registration among older adults in Asian American communities. METHODS: Using a community survey dataset of Japanese American olde

Clinical complexity and Occam's razor: navigating between Scylla and Charibdy of the geriatric practice. A case of secondary hypertension in a very old patient.

The clinical approach toward elderly patients is often very complex and associated with an increased risk of medical errors. This case report is an example of how various objective (related to patient) and subjective (related to physicians) factors may influence the optimal diagnostic approach in elderly frail patients. We also discuss geriatric practice, which must be characterized by the intellectual honesty to refuse any sort of prejudices (such as ageism) and by the skill to navigate between the Scylla (ie, viewing clinical problems as unrelated to each other) and the Charibdy (ie, applyin

Endometriosis in later life: an intersectional analysis from the perspective of epistemic injustice.

Endometriosis, a chronic inflammatory condition affecting 10% of biological women, is widely understudied and particularly overlooked in later life. Discussions surrounding endometriosis predominantly centre on medical gender bias during reproductive years, with limited attention to intersecting factors of discrimination and the impact of ageism on affected individuals. As endometriosis is framed as a disease of reproductive age, research is lacking when it comes to the effects of the illness on the older population. Symptoms in (post)menopausal individuals are frequently misattributed to othe

Factors affecting the will to live among elderly jews living in israel.

Objectives: The aim of this qualitative study was to explore factors that influence older persons' will to live (WTL).Methods: Twenty-five Israeli elders were interviewed about what weakens and/or strengthens their WTL. Elements of the grounded theory method guided the data collection and analysis. Results: Our analysis revealed that the elements that pertain to WTL could potentially both increase and decrease the WTL in elders. These factors included social networks, financial security, religious faith, working and volunteering, and honor versus ageism. Conclusions: Five factors represent diffe

Ethical issues in caring for older persons in Sub-Saharan Africa: a scoping review.

BACKGROUND: Rapid population ageing in sub-Saharan Africa (SSA) is unfolding within fragile health systems, weak social protection, and limited geriatric and palliative services, creating ethical challenges in older adult care. This study critically reviews current literature and policy reports, synthesizes key themes, and proposes evidence-based recommendations for policy, practice, and research. METHODS: This scoping review was conducted in accordance with the Joanna Briggs Institute (JBI) methodology and reported following PRISMA-ScR guidelines. A comprehensive search of biomedical, ethics

Psychiatry and the elderly: an overview.

The author notes that the elderly in our society have not been provided with treatment, research, and services commensurate with their needs. These needs are reflected in the increasing incidence of psychopathology, suicide, and poverty with increasing age. The psychiatric profession's therapeutic nihilism toward the elderly may reflect unresolved countertransference issues that result in a form of prejudice called "ageism." Many of the conditions labeled "senility" are actually manifestations of socioeconomic or medical problems that could be resolved with prompt, appropriate treatment. The a

The challenge of inclusion for older people with impairments: Insights from a stigma-based analysis.

The intersection of disability and aging often presents obstacles and discomfort for older people with disabilities keen to access mainstream opportunities for social participation. Besides individual situations and preferences - disability-based or not - environmental and social factors may limit full access to participation for older people with disabilities. Although ageist and ableist trends of contemporary ideas of aging have been documented, few studies have examined how those discourses are enacted in the field. In the context of participatory action research carried out since 2014 in a

Prognostic Value of Malnutrition, Frailty, and Physical Performance in Transthyretin Cardiac Amyloidosis: Insights From a Prospective Multicenter Cohort Study.

BACKGROUND: The prevalence of transthyretin cardiac amyloidosis among older adults (often octogenarians) is increasing. We aimed to determine whether age and geriatric syndromes bear any impact on the management and outcomes in transthyretin cardiac amyloidosis and assess the risk of ageism. METHODS: In a prospective, multicenter cohort study, 256 patients diagnosed with transthyretin cardiac amyloidosis from March 2021 to March 2024 underwent comprehensive geriatric assessment (CGA). The study evaluated the prevalence and clinical associations of CGAs across different disease stages (Nationa
