Progressive agraphia, acalculia, and anomia: a single case report.

A case of a 50-year-old, right-handed female, mono-lingual native Spanish-speaker with a university-level education and cognitive changes is reported. Over approximately 2 years, she presented with a progressive deterioration of writing abilities associated with acalculia and anomia. An MRI disclosed a left parietal temporal atrophy. Two years later, further significant cognitive decline consistent with a dementia of the Alzheimer's type was observed. Amnesia, executive dysfunction, and ideomotor apraxia were found. Writing was severely impaired, and some difficulties in reading were observed.

Phonological agraphia following a focal anterior insulo-opercular infarction.

Following a unique infarction, restricted to the left anterior insula and the adjacent part of the intrasylvian frontal opercular cortex, an 83-year-old right-handed patient acutely developed a severe speech disorder that evolved into mere mutism within a few hours. After rapid recovery from mutism, oral language was characterized by severe apraxia of speech. In-depth language investigations further disclosed an isolated, highly selective disturbance of the spelling system (phonological agraphia) which resolved rapidly. One year after onset of neurological symptoms, the apraxia of speech had a…

[Two cases of pure agraphia developed after thalamic hemorrhage].

We have observed two cases suffered from left thalamic hemorrhage, that showed pure agraphia. The first case was a 49 year-old, right handed male, who was educated through 11th grade. The CT scan revealed left thalamic hemorrhage with ventricular casting. The second case was a 48 year- old, right handed male, who was educated through 9th grade. The CT scan revealed left thalamic hemorrhage which is smaller than that of the first case, without intraventricular rupture. With a standard neuropsychological evaluation performed at chronic stage, neither cases showed oral language disorder, apraxia …

[A case of left hand agraphia without callosal apraxia].

A 65-year-old male who had agraphia confined to the left hand was reported. The patient was admitted to the neurological department of Kasugai city Hospital because of suddenly-developed mild right-sided hemiparesis with central facial palsy. Computerized tomography of the brain was performed 2 and 14 days after admission. As a result, low-density regions were found in the left cingulate and medial frontal gyri and the trunk of the corpus callosum. Magnetic resonance imaging of the saggital plane more clearly visualized a localized infarction affecting both the trunk of the corpus callosum and…

Recovery from alexia without agraphia: report of an autopsy.

The patient is a 58-year-old Japanese teacher of German literature who suffered twice from cerebrovascular accidents, showing alexia without agraphia. Pathological examination showed an old infarct in the posterior two-thirds of the fusiform and almost the whole lingual gyrus, involving the posterior border of the parahippocampal gyrus in the left hemisphere. The left cuneus and the calcarine cortex were preserved. There was degeneration of the lower third of the splenium of the corpus callosum, extending to its occipital radiation and tapetum on both sides. Comparing clinico-pathological find…

Revival of Historical Kana Orthography in a Patient with Allographic Agraphia.

Japanese people born before World War II learned Japanese kana (Japanese syllabograms) writing in a style that is not currently used. These … trophy of the left fronto-parietal lobe and hypoperfusion of the left frontal lobe. His allographic agraphia might have resulted from the disturbance of the current style of kana orthography.

Temporal lobe epilepsy manifesting as alexia with agraphia for kanji.

Alexia with agraphia results from lesions of the left angular gyrus or the left posteroinferior temporal lobe. In Japanese or Korean persons, lesions of the latter cause alexia with agraphia for ideograms. We describe a case of alexia with agraphia for kanji (Japanese ideograms) caused by temporal lobe epilepsy. After generalized convulsions, a 32-year-old man noticed that he had difficulty in reading and writing kanji, although he could read and write kana (Japanese syllabograms). His EEG showed frequent sharp waves on the left occipitotemporal region. MRI of the brain revealed a hyperintense…

Pure agraphia of kanji due to thrombosis of the Labbé vein.

A case is described of a 56 year old Japanese male with pure agraphia of kanji (the Japanese morphograms) due to haemorrhagic infarction of the left temporal lobe caused by the rare condition of cortical vein thrombosis of Labbé. Writing kanji was severely impaired without disturbed consciousness, aphasia or apraxia. On the other hand, writing kana (the Japanese syllabograms), and reading kanji and kana were almost intact. This suggests that the process of writing kanji involves a different pathway from that of reading kanji in the left temporal lobe. Pure agraphia of kanji is considered to be…

[Agraphia and preservation of music writing in a bilingual piano teacher].

A bilingual virtuoso piano teacher developed aphasia and amusia, probably due to cerebral embolism. The perfectly demarcated and unique lesi … no remaining elements of auditory agnosia. Reading was possible, but with some difficulty and total agraphia and acalculia persisted. Musical ability was better conserved, particularly with respect to repetition and above all to writing, the sparing of the latter constituting a fairly uncommon dissociation in relation to agraphia. Findings are discussed in …

[Case of Moyamoya disease with agraphia caused by a frontal lesion].

A 45-year-old woman showed agraphia and decreasing spontaneity. She was diagnosed as Moyamoya disease by cerebral angiography. CT scans revealed the abnormal low density area in the left frontal lobe, and the regional cerebral blood flow study showed low perfusion in the bilateral frontal lobe and the left high convexity area. She could copy specimens of the author's writing, but her spontaneous writing and dictation were impaired with either hand. The form of each letter she wrote was not so distorted, but wrong letters were substituted for the correct ones. Errors in Kana-writing were more c…

Alexia without agraphia in a left-handed patient with prosopagnosia.

A left-handed patient was studied who had the acute onset of alexia without agraphia, a left homonymous hemianopia, and prosopagnosia. Neurodiagnostic tests including computerized axial tomography and angiography disclosed bilateral lesions in the posterior cerebral hemispheres involving the splenium of the corpus callosum. Neuropsychologic examination indicated marked impairment of facial discrimination, deficient naming and memory of presented visual material, and mild visual agnosia with relative preservation of other cognitive functions. These findings are consistent with a double disconne…

Cerebellar-induced apraxic agraphia: a review and three new cases.

Apraxic agraphia is a writing disorder due to a loss or lack of access to motor engrams that program the movements necessary to produce letters. Clinical and functional neuroimaging studies have demonstrated that the neural network responsible for writing includes the superior parietal region and the dorsolateral and medial premotor cortex. Recent studies of two cases with atypical lesion localisations in the left thalamus and the right cerebellum support the hypothesis that the written language network is larger than previously assumed. The aim of this study is twofold: (1) to provide a surve…

[Pure agraphia with topographical disorientation caused by right hemisphere lesion].

A 69-year-old, right-handed, Japanese male patient presented with pure agraphia with topographical disorientation after hemorrhage in the right parietal lobe. Upon developing cerebral hemorrhage, he was referred to our hospital for close examination of agraphia. There was no paresis or clumsiness in his extremities. His speech was fluent and well-articulated. Neither aphasia nor reading impairments was found, although there was a clear writing impairment with effort and hesitation. His writing of both kanji and kana letters contained additional, absent or deformed strokes or parts. No hemianop…

Nonconvulsive status epilepticus manifesting as pure alexia (alexia without agraphia).

Pure alexia is a rare disorder usually caused by an ischemic or hemorrhagic stroke. We describe a case of pure alexia due to nonconvulsive status epilepticus (NCSE). This 57-year-old man presented in a hyperosmolar, nonketotic state with fluctuations in mental status and an inability to read. His EEG was diagnostic of NCSE originating from the left temporo-occipital region. MRI of the brain revealed increased FLAIR signal over the left occipitotemporal region. Following initiation of antiepileptic treatment, his neurological examination normalized concomitantly with resolution of the NCSE. A f

Agraphia with reversible splenial corpus callosum lesion caused by hypoglycemia.

BACKGROUND: Neurological manifestations caused by hypoglycemia range from reversible focal deficits and transient encephalopathy to irreversible coma or death. Recently, high signal intensity lesions in the splenium of the corpus callosum on diffusion-weighted magnetic resonance imaging were reported in adults experiencing hypoglycemia. However, patients presenting with agraphia are rare. SUBJECT AND METHODS: We examined a 17-year-old left-handed female patient with type 1 diabetes who exhibited transient left agraphia with a reversible splenium lesion of the corpus callosum on diffusion-weig…

Alexia without agraphia following biopsy of a left thalamic tumor.

Alexia without agraphia is a rare disconnection syndrome characterized by the loss of reading ability with retention of writing and verbal comprehension. We report a patient who developed alexia without agraphia after undergoing a biopsy for a malignant glioma involving the left thalamus. A 15-year-old right-handed male presented with 3 days of severe headache, and vomiting, and 1 month of blurry vision in his right visual field. Magnetic resonance imaging of the brain disclosed a large exophytic mass originating in the left thalamus, with mass effect and hydrocephalus. The patient underwent b…

Transitory alexia without agraphia: a disconnection syndrome due to neurocysticercosis.

We describe a 65-year-old male who presented with acute onset inability to read, without any difficulty in writing. A clinical diagnosis of alexia without agraphia was made and the patient was subjected to routine investigations including contrast MRI. MRI showed a ring-enhancing lesion in left occipital area, suggestive of neurocysticercosis supported by quantitative enzyme-linked immunosorbant assay from purified cell fraction of taenia solium cysticerci (PCF-ELISA). Patient was treated with albendazole and prednisolone for one week. The clinical manifestation as well as the radiological fin…
