Anterior capsulotomy of the lens capsule is one of the important steps before extracapsular lens extraction. A neodymium:yttrium aluminum garnet laser anterior capsulotomy was done in 144 eyes with immature, mature, and hypermature senile cataracts. This noninvasive technique is a safe and useful way to treat immature cataracts. To achieve the best result, the time interval between laser capsulotomy and surgery should be within six hours. Flap types of capsulotomy give the best results.

In chronic renal failure, aluminum overload may influence parathyroid function. In a study of possible aluminum-induced parathyroid abnormalities, parathyroid glands from nine parathyroidectomized patients on hemodialysis were examined by light and electron microscopy and by X-ray microanalysis. An obstructing aluminum hydroxide gel bezoar was first recognized on a retrograde gastrografin study through an ileostomy. In retrospect, a computed tomographic (CT) scan had demonstrated the bezoar as a high density intraluminal mass surrounded by somewhat less dense contrast material distending the bowel. A CT image of a phantom made of aluminum hydroxide gel surrounded by gastrografin was remarkably similar.

To determine whether perikaryal neurofilamentous accumulation in cholinergic neurons is associated with a deficit in cholinergic function, we developed a new model of aluminum-induced neurofibrillary degeneration, referred to as focal lumbar aluminum myelopathy. The model is produced by direct intramedullary microinjection of AlCl3, which results in a characteristic neurological syndrome. Interaction of trace metal metabolism was studied in a patient with Wilson's dease. Atomic absorption analysis showed markedly increased urinary excretion of copper and aluminum and an increased aluminum content was found in the biopsied liver by neutron activation analysis. These findings suggest a complicated pathogenetic mechanism involving other metals besides copper in the Wilson's disease.

METHODS: A total of 1278 employees from four aluminum smelters determined to have significant beryllium exposure based on 5 years of sampling were invited to participate in medical surveillance that included a respiratory symptoms questionnaire, spirometry, and blood beryllium lymphocyte proliferation test.

Aluminum is a neurotoxin and in susceptible species induces a neurofibrillary pathology characterized by argentophilic masses in neuronal perikarya and in axonal spheroids. Reductions in the formation of new bone matrix are a consistent finding in both clinical and experimental studies of aluminum-associated bone disease. This adverse effect of aluminum on collagen synthesis may be mediated through reductions in either the number or the cellular activity of osteoblasts.
