Two cases with the heart murmurs considered to be caused by aortocoronary (A-C) bypass surgery were reported. Case 1: A systolic and an early and late diastolic murmurs were noted following A-C bypass surgery. The murmurs were located at the second left intercostal space. Hundred years ago in Great Britain, Sir Thomas Lauder Brunton was acclaimed as an authority on the action of drugs on the heart. His famous work, "Pharmacology and Therapeutics" (1867), was read and appreciated all over Europe and America. He stated that nitrite of amyl has a beneficial effect on the heart. This led to the discovery of many other nitrite compounds that are now used for the treatment of angina pectoris. Despite the wide application of aids for cardiac auscultation, their diagnostic efficacy has not been comprehensively evaluated. The most commonly used auscultatory aids, including respiration (Carvallo's maneuver), Müller and Valsalva maneuvers, squatting, standing, handgrip, intrinsic cycle length changes, and amyl nitrite, are discussed. A review of the literature suggests that enthusiasm for these aids must be tempered because of limitations in sensitivity and specificity as well as weaknesses in the methods used in the validation studies.

A woman suffering from TCGV associated with steno-insufficiency with a prolapse of left atrioventricular valve (tricuspid) and an insufficiency with a prolapse of right atrioventricular valve (mitral) presented a systolic honk which was to be registered in all the precordial area. The increase of this noise at the end of inspiration and after amyl nitrite, in the absence of any relevant change as to the prolapse of both atrioventricular valves, demonstrated that the honk originated from the right atrioventricular valve more than from the left one.
