Knowledge, Attitude, and Practice of Anesthetists in Managing Severe Craniocerebral Trauma in China: A Cross-Sectional Study.

OBJECTIVE: The study explored the knowledge, attitude, and practice (KAP) toward severe craniocerebral trauma perioperative management among anesthetists in China. METHODS: This cross-sectional study recruited anesthetists from 15 provinces in China between October 17, 2022, and November 09, 2022. Pearson's correlation was used to assess the relationships between KAP dimension scores, and multivariable linear regression was performed to identify factors associated with higher KAP scores. RESULTS: The study included 200 anesthetists (122 females). The mean knowledge, attitude, and practice sc…

AANA Journal Course: update for nurse anesthetists--femoral nerve block and 3-in-1 block in anesthesia.

Femoral nerve blocks and 3-in-1 blocks are simple and useful alternatives to other regional or general anesthetic techniques for selected surgeries. These blocks also may provide postoperative analgesia that may be a useful alternative to epidural or parenteral analgesia. Understanding the techniques of blockade, its appropriate applications, and the relevant anatomy may provide the anesthetist with a valuable alternative.

Time-to-intubation in obese patients. A randomized study comparing direct laryngoscopy and videolaryngoscopy in experienced anesthetists.

BACKGROUND: Airway management may be difficult in obese patients. Moreover, during prolonged intubation, oxygen desaturation develops rapidly. Videolaryngoscopy improves the view of the larynx, and the Storz® C-MAC™ has been shown to be superior to other videolaryngoscopes in terms of intubation time in obese patients. However, no effort has been made to compare the Storz® C-MAC™ with direct laryngoscopy. The aim of the study was to evaluate if the use of Storz® C-MAC™ may reduce intubation time when compared to direct laryngoscopy (classic Macintosh® blade). METHODS: Eighty patients with Bod

Carpal tunnel syndrome in female nurse anesthetists versus operating room nurses: prevalence, laterality, and impact of handedness.

UNLABELLED: Nurse anesthesia may be a high-risk occupation for carpal tunnel syndrome (CTS) in the workplace. We designed a cross-sectional investigation to study the prevalence of CTS in nurse anesthetists (NAs) as compared with operating room nurses (ORNs). Two-hundred forty-four female operating room workers were classified by job title as NAs (n = 63) and ORNs (n = 181). The case definition of CTS was established by a history of surgical correction or a combination of four positive historical and physical findings. There were 10 cases of CTS in NAs and 10 cases of CTS in ORNs. The crude od…

Impact of anesthetist licensing examination on quality of education in Ethiopia: a qualitative study of faculty and student perceptions.

BACKGROUND: Ethiopia drastically increased the anesthesia workforce density by training 'associate clinician anesthetists' as a task-shifting and sharing strategy. However, there were growing concerns about educational quality and patient safety. Accordingly, the Ministry of Health introduced the anesthetist national licensing examination (NLE) to assure the quality of education. However, empirical evidence is scarce to support or refute the overall impact of NLEs, which are relatively costly for low- and middle-income settings. Therefore, this study aimed to explore the impact of introducing …

Comparison of the Effects of Automated and Manual Record Keeping on Anesthetists' Monitoring Performance: Randomized Controlled Simulation Study.

BACKGROUND: Anesthesia information management systems (AIMSs) automatically import real-time vital signs from physiological monitors to anesthetic records, replacing part of anesthetists' traditional manual record keeping. However, only a handful of studies have examined the effects of AIMSs on anesthetists' monitoring performance. OBJECTIVE: This study aimed to compare the effects of AIMS use and manual record keeping on anesthetists' monitoring performance, using a full-scale high-fidelity simulation. METHODS: This simulation study was a randomized controlled trial with a parallel group de…

Narrative review of mathematical and psychological studies of staff scheduling for holidays as applicable to anesthesiologists and nurse anesthetists.

We performed a narrative review of articles applicable to anesthesiologists' and nurse anesthetists' choices of who works each statutory holiday for operating room and non-operating room anesthesia. We include search protocols and detailed supplementary annotated comments. Studies showed that holiday staff scheduling is emotional. Working on holidays often is more stressful and undesirable than comparable workdays. Intrinsic motivation may overall, among practitioners, be greater by preferentially scheduling practitioners who choose to work on holidays, for compensation, before mandating that …

Complex regional pain syndrome: a review of diagnostics, pathophysiologic mechanisms, and treatment implications for certified registered nurse anesthetists.

The pathophysiologic mechanisms for complex regional pain syndrome (CRPS) are complex and elusive. The proposed etiologic mechanisms for CRPS include inflammatory responses, peripheral or central sensitization, and sympathetic dysfunction. Anesthesia care of patients with CRPS is challenging. Treatments including physiotherapy, peripheral vasodilators, sympathetic blockade, analgesics, and other systemic medications can help optimize mobility, perfusion, and pain relief for affected patients.

AANA Journal course: update for nurse anesthetists--anesthetic techniques for patients with valvular heart disease presenting for noncardiac procedures.

Providing anesthesia for patients with valvular heart disease undergoing noncardiac surgery can be extraordinarily challenging. Not only must the principles of normal cardiac physiology be mastered, but also those of the pathophysiology associated with the various valvular lesions. This course will provide a review of the pathophysiology and specific compensatory mechanisms for aortic stenosis, aortic insufficiency, mitral stenosis, and mitral regurgitation. Characteristic findings in the patient interview, physical examination, and diagnostic studies will be discussed. Anesthetic and monitori

AANA journal course. 13. New technologies in anesthesia: update for nurse anesthetists--clonidine: an established drug with futuristic indications.

Clonidine, an alpha-2 agonist, has traditionally been used to treat hypertension. Recently, it has been found to be a potent analgesic when administered either orally, transdermally or parenterally. This antinociceptive action is mediated at the spinal cord level through presynaptic inhibition of norepinephrine. Clonidine is also a valuable adjunct to anesthesia in a variety of clinical scenarios. In this lesson, the potential uses of clonidine will be elucidated, along with an explanation of its pharmacological and physiological mechanisms of action. Clonidine will be explored as an alternati

Does higher performance in a national licensing examination predict better quality of care? A longitudinal observational study of Ethiopian anesthetists.

BACKGROUND: Ethiopia made a national licensing examination (NLE) for associate clinician anesthetists a requirement for entry into the practice workforce. However, there is limited empirical evidence on whether the NLE scores of associate clinicians predict the quality of health care they provide in low-income countries. This study aimed to assess the association between anesthetists' NLE scores and three selected quality of patient care indicators. METHODS: A multicenter longitudinal observational study was conducted between January 8 and February 7, 2023, to collect quality of care (QoC) da…
