[Thoracoabdominal aortic aneurysm].

Thoracoabdominal aortic aneurysms (TAAA) are rare events with an incidence of 5.9 cases per 100,000 persons per year. In Germany approximately 940 TAAA procedures are performed annually. The cause of TAAA is mostly degenerative but they can also occur on the basis of an aortic dissection or connective tissue disease (e. g. Marfan's syndrome). Patients often have severe comorbidities and suffer from hypertension, coronary heart disease or chronic obstructive pulmonary disease, mostly as a result of smoking. Operative treatment is indicated when the maximum aortic diameter has reached 6 cm (> 5 …

A false aneurysm.

OBJECTIVE: We report a case of a remaining hemi-thyroid following laryngectomy, which was misinterpreted as a pseudoaneurysm. METHODS: Case report and comment on this understandable error which is easily avoidable. RESULTS: A 59-year-old man had undergone salvage laryngectomy for recurrent squamous cell carcinoma of the larynx, which had previously been treated with radiotherapy. Three months after his laryngectomy, he presented with a sore neck and subcutaneous collections. Computed tomography revealed a unilateral mass with high signal contrast uptake anterior to the left common carotid ar…

Hepatic artery aneurysm.

OBJECTIVE: To present a successfully treated case of hepatic artery aneurysm. CLINICAL FEATURES: A 76-year-old, retired male Caucasian presented with acute abdominal pain which was diagnosed as acute cholecystitis. An aneurysm of the hepatic artery was found at operation for cholecystectomy, and an arteriogram confirmed the position of the aneurysm, distal to the origin of the gastroduodenal artery. INTERVENTION AND OUTCOME: The aneurysm was resected and replaced with an 8 mm diameter woven Dacron graft. The postoperative course was uneventful and the patient was discharged on the ninth post…

[Pulmonary arteriovenous aneurysm].

A case of large arteriovenous aneurysm in the right upper lobe of a 27-year old man is reported. The pulmonary angioma was responsible for a 30% shunt with hypoxia and polycythaemia but was only discovered while looking for the origin of a cerebral abscess. A few weeks after surgical treatment of the cerebral abscess, the right upper lobe was excised with immediate correction of the hypoxia. This case shows that pulmonary angiomas can sometimes be revealed by neurological complications, and that visceral angiomas should not remain undiagnosed. They should be considered as part of a Rendu-Osler…

Dissecting intracranial aneurysm.

A case of spontaneous dissecting aneurysm of the right internal carotid and middle cerebral arteries is presented in 13-year-old boy. The pathogenetic factors incriminated in previously reported cases are reviewed and the pathological findings are discussed. The abnormalities of the internal elastic lamina as seen in this patient have been observed in numerous cases of intracranial dissecting aneurysms. It is concluded that these defects play an important role in the development of dissection.

Coronary artery aneurysm.

The case is presented of a 28-year-old woman with systemic vasculitis, aortic insufficiency, and an aneurysm of the proximal right coronary artery. More than 100 patients with coronary artery aneurysm have been reported, and this represents the twenty-first case treated surgically. Coronary artery aneurysms occur more frequently than previously realized and are seen in approximately 1.5% of patients with coronary artery disease. In addition to arteriosclerotic aneurysm, other common types of coronary aneurysm include those occurring congenitally or from mycosis, dissection, trauma, vasculitis,…

Bifurcation dissecting aneurysm: tips and tricks to differentiate from saccular aneurysm.

Bifurcations are a common site for saccular aneurysms, but rarely can be a site for dissecting aneurysms. Identification of these aneurysms is extremely important because the management plan depends on it. We describe a rare case of a ruptured dissecting aneurysm at the right ICA bifurcation in a pre-teen child which posed a diagnostic dilemma but ultimately was successfully managed with flow diversion.

Midterm results of endovascular aneurysm sealing to treat abdominal aortic aneurysm.

BACKGROUND: Endovascular aneurysm sealing (EVAS) represents a novel approach to the treatment of abdominal aortic aneurysms. It uses polymer technology to achieve an anatomic seal within the sac of the aneurysm. This cohort study reports the early clinical outcomes, technical refinements, and learning curve during the initial EVAS experience at a single institution. METHODS: Results from 150 consecutive EVAS cases for intact, infrarenal abdominal aortic aneurysms are reported here. These cases were undertaken between March 2013 and July 2015. Preoperative, perioperative, and postoperative dat…

Late Sac Behavior after Endovascular Aneurysm Repair for Abdominal Aortic Aneurysm.

BACKGROUND: Sac behavior after endovascular aneurysm repair (EVAR) for abdominal aortic aneurysms (AAAs) is considered as a surrogate for the risk of late rupture. The purpose of the study is to assess the sac behavior of AAAs after EVAR. METHODS AND RESULTS: Late sac enlargement (LSE) (≥5 mm) and late sac shrinkage (LSS) (≥5 mm) were analyzed in 589 consecutive patients who were registered at 14 national centers in Japan. The proportions of patients who had LSE at 1, 3 and 5 years were 2.6% ± 0.7%, 10.0% ± 1.6% and 19.0% ± 2.9%. The proportions of patients who had LSS at 1, 3 and 5 years wer…

Glasgow aneurysm score in predicting outcome after ruptured abdominal aortic aneurysm.

OBJECTIVE: To assess the ability of Glasgow Aneurysm Score in predicting postoperative mortality for ruptured aortic aneurysm which may assist in decision making regarding the open surgical repair of an individual patient. METHODS: A total of 121 patients diagnosed of ruptured abdominal aortic aneurysm who underwent open surgery in our hospital between 1999 and 2013 were included. The Glasgow Aneurysm Score for each patient was graded according to the Glasgow Aneurysm Score (Glasgow Aneurysm Score = age in years + 17 for shock + 7 for myocardial disease + 10 for cerebrovascular disease + 14 f…

Chronic fusiform aneurysm evolving into giant aneurysm in the basilar artery.

Fusiform basilar aneurysm is a rare condition with elevated mortality within a few days if untreated. On the basis of clinical course, the fusiform aneurysm can be distinguished in an acute type, such as dissecting aneurysm, which usually causes subarachnoid hemorrhage or cerebral ischemia and in a chronic type with a relatively slow growth, which may evolve into a giant aneurysm leading to serious complications. We report a case of an 80-year-old man with a surgically untreated fusiform aneurysm that evolved into a giant aneurysm of the basilar artery within 4 years. The patient presented rec…
