Ascites and Hepatorenal Syndrome.

Ascites occurs in up to 70% of patients during the natural history of cirrhosis. Management of uncomplicated ascites includes sodium restriction and diuretic therapy, whereas that for refractory ascites (RA) is regular large-volume paracentesis with transjugular intrahepatic portosystemic shunt being offered in appropriate patients. Renal impairment occurs in up to 50% of patients with RA with type 1 hepatorenal syndrome (HRS) being most severe. Liver transplant remains the definitive treatment of eligible candidates with HRS, whereas combined liver and kidney transplant should be considered i…

Treatment for cirrhotic ascites.

Common complications of decompensated liver cirrhosis are esophageal varices, hepatic encephalopathy and ascites. After the onset of complications, the prognosis worsens. In patients with ascites, the 5-year mortality rate is 44%. Furthermore, hyponatremia, spontaneous bacterial translocation and hepatorenal syndrome also greatly worsen the prognosis. Effective treatment of cirrhotic ascites improves the quality of life and survival rate. Recently, the newly produced diuretic, tolvaptan (vasopressin V2 receptor antagonist), was reported to be effective in the treatment of refractory ascites in…

[Ascites drainage at home].

Ascites can lead to many symptoms, and often occurs in patients with an end-stage malignancy such as ovarian, pancreatic, colonic, or gastric cancer. Intermittent ascites drainage is applied in these patients as a palliative measure. As frequent drainage is necessary, a subcutaneously tunnelled permanent ascites catheter is a good alternative for intermittent drainage. The patient can open - and then re-close - the catheter when abdominal pressure increases. We inserted 35 subcutaneously permanent ascites catheters in the course of the past 3.5 years in the Leiden University Medical Centre. Th…

Management of cirrhotic ascites.

The most common complication to chronic liver failure is ascites. The formation of ascites in the cirrhotic patient is caused by a complex chain of pathophysiological events involving portal hypertension and progressive vascular dysfunction. Since ascites formation represents a hallmark in the natural history of chronic liver failure it predicts a poor outcome with a 50% mortality rate within 3 years. Patients with ascites are at high risk of developing complications such as spontaneous bacterial peritonitis, hyponatremia and progressive renal impairment. Adequate management of cirrhotic ascit…

Management of refractory ascites.

Ascites that does not respond or recurs after high-dose diuresis and sodium restriction should be considered refractory ascites. As cirrhosis advances, the escaping fluid overwhelms the lymphatic return. Decrease in renal plasma flow leads to increased sodium reabsorption at the proximal tubule leading to decreased responsiveness to loop diuretics and mineralocorticoid antagonists, which work distally. These complex hemodynamic alterations lead to refractory ascites. In refractory ascites, high-dose diuresis (400 mg of spironolactone and 160 mg of furosemide) and sodium restriction (<90 mmol/d…

Treatment of cirrhotic ascites.

Cirrhosis is the most common cause of ascites and accounts for almost 85% of all cases. It is the most common complication of cirrhosis, after development of ascites only 50% of patients will survive for 2 to 5 years. Successful treatment is dependent on accurate diagnosis of the cause of ascites. Because sodium and water retention is the basic abnormality leading to ascites formation, restriction of sodium intake and enhancing sodium excretion is the mainstay of the treatment of ascites. Patients with cirrhosis and ascites must limit sodium intake to 2 gram per day. Enhancement of sodium excr…

Dermatomyositis with massive ascites.

There have been few reports of dermatomyositis causing ascites. Here we report a case of a 63-year-old man complaining of general fatigue and gastromegaly. Abdominal examination revealed distension without tenderness. Serum myogenic enzyme was elevated. Electromyographic investigation indicated low amplitude signs, which were compatible with muscle disorder. Abdominal paracentesis on the fifth day yielded 2,500 ml clear, serous fluid. The specific gravity was 1.026 (range 1.005-1.015), with a positive Rivalta reaction, 3.4 g/dl total protein, and 1.59 g/dl albumin, suggesting exudate. Excludin…

Hepatic encephalopathy and ascites.

The first abnormality leading to sodium and water retention in cirrhosis is the renal tubular defect that is related to deteriorating liver function and hyperaldosteronism. With progression of liver disease and portal hypertension, renal blood flow declines because of the hepatorenal reflex, and is then maintained by the vasoactive hormonal systems. With increasing peripheral vasodilatation, intrarenal factors for maintenance of renal perfusion cause intense cortical vasoconstriction. The systemic vasoactive factors are predominantly compensatory; any attempts to counteract their action risk c

Pancreatic ascites in children.

We report on three children with pancreatic ascites confirmed by endoscopic retrograde cholangiopancreaticography (ERCP) and treated with surgery. The children presented with ascites, malnutrition and severe weight loss. Pancreatic ascites was diagnosed by elevated ascitic fluid and serum amylase levels. ERCP demonstrated a pseudocyst and the site of disruption of the pancreatic duct, but not the etiology of the pancreatitis. Following a period of nutritional support, surgery was carried out. Two of the children underwent a stented transgastric drainage of the pseudocyst; a recurrent pseudocys…

Ascites in liver diseases.

The clinical course of patients with cirrhosis of the liver is frequently complicated by progressive impairment of renal sodium handling leading to the formation of ascites. The occurrence of ascites is generally accompanied by the activation of several hormones and intrarenal autacoids and a complex derangement of systemic, portal and renal hemodynamics. The earliest "underfilling" theory of sodium retention proposes that ascites formation leads to hypovolemia and secondary sodium retention. According to the "overflow" theory, ascites formation is a secondary event with respect to sodium rete…

[Hepatic hydrothorax without ascites].

A case of hepatic hydrothorax without clinical ascites is reported. The diagnosis was confirmed by intraperitoneal injection of 99mTc-tin colloid: scintigraphic images taken between 5 min and 18 h p.i. demonstrated a one-way transdiaphragmatic flow into the pleural cavity. The therapy consisted of diuretic treatment, pleural drainage and chemical pleurodesis, and resulted in prompt clinical improvement.

Laparoscopy with artificial ascites.

Since September, 1985 we have performed laparoscopy with artificial ascites on 52 patients. First a laparoscopy with a pneumoperitoneum (2-31 CO2) is initiated, as usual under general anesthesia. After performing an orientating inspection, the CO2 is replaced by 2-31 warm physiological saline. The gas-containing intestinal loops, the air-containing stomach and the greater omentum float on the surface of the liquid, thus causing the mesenterium to spread out. By immersing the optical instrument in the liquid, accurate inspection of the intra-peritoneal organs becomes possible. By additionally i…

Bloody ascites in sarcoidosis.

We present the findings in two patients in whom sarcoidosis was manifested by bloody ascites. Granulomas were present on peritoneal surfaces in one case and on the serosal surface of the spleen and liver in the other. Five patients with abdominal sarcoidosis have previously been reported with nonbloody ascites. Although bloody ascites due to sarcoidosis is rare, it is presumably akin to bloody pleural and pericardial effusions which have been previously reported. The long-term prognosis of patients with peritoneal sarcoidosis and ascites appears good.

Management of pancreatic ascites.

Two patients with persistent pancreatic ascites are presented. Both were managed successfully by internal drainage. A trial of nonoperative treatment with nasogastric intubation and intravenous hyperalimentation is advocated for patients with pancreatic ascites. Patients who fail to improve usually have a pancreatic duct disruption and should have internal drainage operation and/or distal pancreatic resection.

Arteriovenous Fistula-Associated Ascites.

Dialysis-associated ascites (DAA) is a diagnosis of exclusion in end-stage kidney disease (ESKD). The prognosis is grim and typically obviates kidney transplantation. We report a case initially misdiagnosed as DAA, which was later correctly identified as cardiac ascites due to high-output heart failure (HOHF) resulting from an aneurysmal arteriovenous fistula (AVF). We propose the term "AVF-associated ascites" (AFAA) to describe this entity. The HOHF improved with AVF ligation with complete resolution of the ascites, thus making the patient eligible to receive a kidney transplant. This case em…

Studies on "Ascites sarcoma".

This review seeks to highlight and celebrate Professor Tomizo Yoshida's famous work on "Establishment and characterization of a rat ascites sarcoma, later named "Yoshida ascites sarcoma". Considering the tremendous contribution of this ascites tumor system to the subsequent promotion of research on cancer biology and cancer chemotherapy, his paper should be regarded as a monumental one in the cancer field. The research was carried out during 1943 and the results were submitted to this Journal in October 1944, when Japan was approaching a debilitating defeat in World War II in August 1945. In 1…
