Interatrial septum pacing: a new approach to prevent recurrent atrial fibrillation.

BACKGROUND: There are a variety of approaches to the prevention of atrial fibrillation (AF) with pacing. Aim of this study was to test the safety and feasibility of interatrial septum pacing at the posterior triangle of Koch for AF prevention and to exclude potential arrhythmic effects. MATERIAL AND METHODS: Interatrial septum pacing was performed in 34 patients (21 males, 13 females, mean age 69 +/- 12 years): 9 without a history and clinical evidence of atrial fibrillation (AF) (6 with sinus bradycardia, 2 with second-degree AV block, and 1 with carotid sinus hypersensitivity) and 25 with s…

Fetal stenting of the atrial septum: technique and initial results in cardiac lesions with left atrial hypertension.

BACKGROUND: Hypoplastic left heart syndrome with a highly restrictive or intact atrial septum (HLHS-RAS) has a very high mortality. Fetal left atrial (LA) hypertension results in abnormal lung development with lymphangiectasia and pulmonary vein muscularization. We report our initial experience with percutaneous ultrasound-guided stenting of the fetal atrial septum to decompress the LA. METHODS: Retrospective review of fetuses with HLHS-RAS or a variant that underwent active perinatal management from 2000 to 2012. RESULTS: Ten fetuses were identified. Two died in utero (33, 29 weeks). Four r…

Pulmonary overcirculation following left atrial decompression in hypoplastic left heart syndrome with intact/restrictive atrial septum.

Patients with hypoplastic left heart syndrome (HLHS) with intact or severely restrictive atrial septum have slow resolution of their elevated pulmonary vascular resistance after left atrial decompression. We report two neonates who required urgent surgical intervention because of unusually rapid fall in their pulmonary vascular resistance following left atrial decompression leading to pulmonary overcirculation and severe congestive heart failure. HLHS with intact or severely restrictive atrial septum is a predictor of poor outcome. The need for left atrial decompression can be predicted by wel…

The hypoplastic left heart syndrome with intact atrial septum: atrial morphology, pulmonary vascular histopathology and outcome.

OBJECTIVES: The purpose of this study was to investigate the outcome in infants with hypoplastic left heart syndrome and intact atrial septum and to evaluate the relationship of atrial morphology, left atrial decompression pathway and lung histopathology to outcome. BACKGROUND: In the hypoplastic left heart syndrome, severe restriction at the atrial level results in marked systemic hypoxemia after birth. Infants with intact atrial septum may be at high risk for mortality after Norwood operation. METHODS: Of 316 infants with hypoplastic left heart syndrome seen at our center over a 6.5-year p…

Dramatic Aneurysmal Atrial Septum Identified with Intracardiac Echo Complicating Transseptal Puncture.

• Atrial septal aneurysm is a largely underdiagnosed phenomenon. • Septal anatomy is best delineated using intracardiac echocardiography. • Septal anatomy is clinically relevant with procedures involving transseptal puncture. • Atrial septal aneurysms may be associated with supraventricular tachycardias.

Congenitally corrected transposition and mitral atresia complicated by restrictive atrial septum.

Congenitally corrected transposition (ccTGA), also known as L-transposition of the great arteries (L-TGA), is a rare cardiac malformation accounting for approximately 0.05% of congenital heart disease, characterized by ventricular inversion, discordant ventriculo-arterial connections with a normal visceroatrial relationship. It was first described by Baron Rokitansky in 1875, and prenatal sonographic diagnosis can be difficult. Symptomatology and clinical presentation of this malformation are related to the associated intracardiac defects. We report a rare case of severe, complex cardiac disea

Surgical outcome of hypoplastic left heart syndrome with intact atrial septum.

BACKGROUND: The surgical outcomes of hypoplastic left heart syndrome with intact atrial septum remains very poor in spite of the introduction of prenatal diagnosis before the Norwood operation. The hybrid operation consisting of bilateral pulmonary artery banding and balloon atrioseptectomy is one of the treatment strategies to potentially improve this patient population, however, the long-term outcomes are unknown. METHODS: Six consecutive patients with hypoplastic left heart syndrome with intact atrial septum, who underwent the hybrid operation between October 2006 and July 2014, were retro…

Anomalous pulmonary venous return with intact atrial septum: diagnosis and pathophysiology.

Twenty-one patients with partial anomalous pulmonary venous drainage with intact atrial septum have been studied. These include 13 patients not previously reported from our laboratories and eight patients with complete hemodynamics reported by others. Methods for identification of this abnormality and for identification of an intact atrial septum are described, including differential indicator dilution curves, catheter probing of the atrial septum and pulmonary angiography. Blood flow through anomalously draining lobes of the lung is usually higher than through normally drainage lobes attribut…

Hybrid transatrial stent insertion for left atrial decompression in hypoplastic left heart syndrome with intact atrial septum.

OBJECTIVES: To describe a novel strategy for the management of infants with hypoplastic left heart syndrome (HLHS) and intact atrial septum. Antenatally diagnosed infants are delivered in cardiac theatre and immediate left atrial decompression (LAD) using hybrid transatrial stent insertion (HTSI) via sternotomy is carried out. BACKGROUND: HLHS with intact atrial septum is a condition incompatible with life following placental separation. Despite a number of reported strategies the survival of these infants remains much worse than those born with an adequate atrial communication. Immediate pos…

Hypoplastic left heart syndrome with intact or highly restrictive atrial septum: outcome after neonatal transcatheter atrial septostomy.

BACKGROUND: Hypoplastic left heart syndrome (HLHS) with intact or very restrictive atrial septum is a highly lethal combination. We review our 13-year institutional experience treating this high-risk subgroup of patients with emergent catheter therapy. METHODS AND RESULTS: Infants with HLHS requiring catheter septostomy within the first 2 days of life were compared with a matched control group with adequate interatrial communication. Preoperative, early postoperative, and medium-term survival were evaluated. Earlier experience was compared with recent results to assess the effect of changes i…

Acquired atrial septal defects secondary to rupture of the atrial septum in dogs with degenerative mitral valve disease.

Acquired atrial septal defects (ASDs) secondary to atrial septal rupture are uncommon in dogs with degenerative mitral valve disease. Echocardiographic diagnosis of these defects has not previously been described. The echocardiographic features in two cases identified antemortem are presented in this report. ASDs can be difficult to identify on two-dimensional echocardiogram when relatively small in size and may be underreported. Acquired ASDs may alter the physiology, treatment plan and prognostic implications for the dogs in which they occur and their presence should be considered in all cas

[Lipomatous hypertrophy of the interatrial septum is a rare, benign cause of atrial arrhythmia].

Lipomatous hypertrophy of the interatrial septum is a pathological thickening of the interatrial septum characterized by a benign infiltration of fat. It usually occurs in older, obese persons and in most cases the diagnosis is made incidentally. Atrial arrhythmias may occur and in rare cases also superior vena cava syndrome caused by obstruction of venous inflow to the right atrium. We present a case of lipomatous hypertrophy of the interatrial septum in an 81-year-old woman with atrial arrhythmia.

Lipoma of the interatrial septum in a patient with chronic atrial fibrillation: A case report.

The authors report on a 70-year-old woman with chronic atrial fibrillation that was hospitalized for digitalis intoxication and incidentally was found to have an interatrial lipoma. The diagnosis was established by transesophageal echocardiography, computed tomography (CT) scanning, and magnetic resonance imaging (MRI). Owing to the asymptomatic character and the benign nature of the tumor, a decision for conservative management was made.
