Technical problems and complications of axillary artery cannulation.

OBJECTIVE: Cardiopulmonary bypass via the axillary artery is frequently used especially in aortic dissections. With an increased use of this technique problems were recognized too. We describe the technical problems and complications associated with axillary artery cannulation. METHODS: Sixty-five patients underwent cannulation of the axillary artery. The indication for operation was acute aortic dissection type A in 57%, chronic aortic dissection in 8%, aortic aneurysm in 18%, pseudoaneurysm in 3%, and others in 14%. RESULTS: Technical problems and complications occurred in 14%, and in 11% …

Axillary artery cannulation in acute ascending aortic dissections.

BACKGROUND: Standard cannulation of the femoral artery in preparation for repair of a dissection involving the ascending aorta carries a high risk of malperfusion. Arterial perfusion through the right axillary artery is more likely to perfuse the true lumen and should be advantageous in acute dissections involving the ascending aorta. METHODS: Thirteen patients underwent repair of acute ascending aortic dissections and were perfused through the right axillary artery. All had deep hypothermic circulatory arrest. RESULTS: There was one mild intraoperative cerebrovascular accident with complete…

Management of proximal axillary and subclavian artery injuries.

BACKGROUND: The aim of this study was to review the management and outcome of proximal axillary and subclavian artery injuries, and to estimate the prehospital mortality rate for subclavian injury through forensic pathology autopsy data. METHODS: Data were collected prospectively for 260 patients who presented between 1977 and 1996 with trauma to the proximal axillary and subclavian arteries. RESULTS: The majority of victims (214, 82 per cent) were admitted following stab injury. Some 154 patients (59 per cent) presented within 24 h of sustaining an injury and, of these, 59 (38 per cent) req

[Reflex dystrophy after reconstruction of the axillary artery].

Reflex sympathetic dystrophy (RSD) is a complex syndrome of pain, trophic changes and vasomotor instability affecting the limbs. Numerous theories have been suggested to explain the pathophysiology. None is universely accepted. In most of the patients reported, an antecedant event such as trauma or surgery is implicated in the initiation of symptoms. We describe a case of reflex sympathetic dystrophy developing after reconstruction of a. axillaris. To our knowledge there have only been a few previous descriptions of reflex dystrophy following vascular surgery. The mechanism of reflex sympathet

Axillary artery compression and thrombosis in throwing athletes.

A 28-year-old major league baseball pitcher sustained an axillary artery thrombosis which was successfully treated with intraarterial urokinase. Subsequent angiography and duplex scanning with the arm elevated in the pitching position demonstrated inducible compression of the axillary artery by the humeral head as well as compression at the thoracic outlet. To determine the incidence of axillary and subclavian artery compression and to investigate the mechanism of injury, brachial artery blood pressures and duplex scans of the subclavian and axillary arteries were performed in both the neutral…

[Intravascular monitoring by axillary artery cannulation in infants].

In patients with respiratory failure and shock serial arterial blood analyses are important to calculate adequate respiratory therapy. During circulatory insufficiency punction of a radial artery can be difficult or impossible, especially in very young patients. The case report of a 9-month old female infant with septic shock and Waterhouse-Friderichsen's syndrome illustrates these problems and is helpful to describe an alternative technique. Percutaneous axillary arterial catheterization with a disposable venous cannula proved to be a simple and quick method to obtain the possibility of arter

Axillary-contralateral brachial artery bypass for radiation-induced occlusion of the subclavian artery.

Symptomatic occlusion of the subclavian artery is a rare complication of radiation therapy for carcinoma of the breast. The first case of revascularization of this entity using saphenous vein was described in 1974. A total of only 24 patients have been reported in the English literature, of whom 14 have undergone successful arterial reconstruction. Three additional cases of occlusion of the subclavian artery 27, 18 and 7 years after radical mastectomy and postoperative radiotherapy are reported. These patients presented with pain, coldness, paresthesia and absence of pulses in the affected arm

Axillary artery access considerations in Impella 5.5 insertion: Insights from exclusive axillary approach for successful support.

BACKGROUND: The Impella 5.5® is commonly inserted via the axillary artery (AX) in patients with cardiogenic shock. The right AX has traditionally been preferred to avoid crossing the aortic arch, and a minimum diameter of 7 mm has been recommended to accommodate the device (21 Fr). There is limited data on choice of laterality of access and AX size required, both in terms of technicality of the procedure as well as outcomes. METHODS: We performed a single-center retrospective cohort analysis of patients who underwent Impella 5.5® implantation between December 2020 and February 2024 (N = 75). …

Contemporary utilization of the axillary artery in cardiac surgery.

INTRODUCTION: The axillary artery is a reliable inflow vessel when addressing pathology of the aortic root and aortic arch that may preclude standard central cannulation strategies. This narrative review examines the use of the axillary artery in cardiac surgery. Anatomy, indications for use, cannulation strategies, and potential complications will be discussed. METHODS: A comprehensive review of the current literature was performed using PubMed, Cochrane Review, and authoritative committee guidelines. A narrative review incorporating current available evidence was undertaken. COMMENT: Use o…

Bilateral Aneurysms of the Axillary Arteries and Marfan's Syndrome.

Aneurysms of the arteries of the upper limbs represent a rare pathology. Apart from the risk of rupture and according to their localization, they can be responsible for nervous or venous compression, for embolism and for ischemia. Axillary localization is particularly rare apart from an iatrogenic context. Open surgery and more recently endovascular techniques can be proposed, but real guidelines are lacking. We report the case of a patient presenting a Marfan syndrome and bilateral axillary aneurysms, with symptoms on the right side due to a nervous compression responsible for paresthesia. An

An unusual variation of axillary artery: a case report.

An unusual unilateral variation was observed in branching pattern and course of branches of the axillary artery of the left upper limb in an adult male cadaver. The superior thoracic branch of axillary artery had a very unusual course as it passed between the two divisions of the lateral cord of the brachial plexus and then descended down towards the first two intercostal spaces. A common trunk was seen arising from the third part of the axillary artery which divided into anterior and posterior circumflex humeral, subscapular and Profunda brachii artery. The ulnar and radial collaterals arose …

Covered stents for injuries of subclavian and axillary arteries.

INTRODUCTION: Injury to the subclavian and axillary arteries is uncommon. Exposure of these vessels is associated with significant morbidity, and mortality ranges from 5% to 30%. Endovascular methods may offer an alternative approach to these technically challenging injuries. METHODS: We retrospectively studied patients with blunt or penetrating (including iatrogenic) injuries to the subclavian or axillary artery between January 1, 1996 and July 30, 2002. Demographic data, mechanism of injury, concomitant injuries, angiographic findings, and treatment method and outcome were recorded. RESULT…

Percutaneous intervention for radiation damage to axillary arteries.

Radiation injury to subclavian and axillary arteries is a rare and late complication of radiotherapy. Until recently it has been treated almost exclusively by carotid-brachial bypass surgery. We report three cases who presented with severe upper limb ischaemia following previous axillary radiotherapy for breast carcinoma. One patient with an axillary artery occlusion failed angioplasty and required bypass surgery. A further patient with an axillary artery occlusion was successfully managed by the percutaneous placement of an arterial stent. The third patient with an isolated axillary artery st…

Axillary artery as a landmark in cannulating the subclavian vein.

A method of cannulation of the subclavian vein is described in which the axillary artery serves as the primary landmark for locating the vein. This technique results in a long skin tunnel and a very low incidence of pneumothorax or other complications.

Axillary artery injury in erect dislocation of the shoulder.

Two rare event occurred in one patient: luxation erecta, an unusual form of dislocation at the shoulder, and axillary artery injury, hitherto undescribed in this type of dislocation. The arterial injury consisted of rupture of the intima with secondary thrombosis and occlusion. Following resection of the contused artery and saphenous vein graft replacement normal circulation was restored.
