Epidemiological evidence on Balkan nephropathy as a viral disease.

Balkan nephropathy is a chronic kidney disease of a completely unknown etiology. Most epidemiologists believe that the disease has been caused by viruses, though all attempts to prove such a relationship have been fruitless. A common feature of most of the clues offered for elucidating the role of the viruses, is that they interfere with the epidemiological evidence on BN. Therefore, a hypothesis is put forward that the disease has been caused by slow viruses transmitted by rodents which contaminate food and articles in the house. Such an explanation fits most of the existing epidemiological d…

Evidence of a viral aetiology in endemic (Balkan) nephropathy.

Segemental and focal pathological changes were found in the glomeruli and tubules of postoperative renal-biopsy specimens from seven cases o … budding particles (80-200 nm) were found. These particles had the characteristics of a coronarivus. Balkan nephropathy occurs almost exclusively in people who have been in close contact with pigs. Coronaviruses have been isolated from pigs, and it is suggested that a slow coronavirus infection causes endemic nephropathy in man.

Balkan endemic nephropathy: an update on its aetiology.

Balkan endemic nephropathy (BEN) is a unique, chronic renal disease frequently associated with upper urothelial cancer (UUC). It only affects residents of specific farming villages located along tributaries of the Danube River in Bosnia-Herzegovina, Croatia, Macedonia, Serbia, Bulgaria, and Romania where it is estimated that ~100,000 individuals are at risk of BEN, while ~25,000 have the disease. This review summarises current findings on the aetiology of BEN. Over the last 50 years, several hypotheses on the cause of BEN have been formulated, including mycotoxins, heavy metals, viruses, and t

[Balkan nephropathy. Current problems of pathogenesis and carcinogenesis].

An important characteristic in the pathogenesis of Balkan nephropathy is the long latent period between the start of exposure and the clinical manifestation of the disease. The pathogenic mechanisms involved in this process are considered in both the adult and the child. Certain essential questions relating to carcinogenesis are also considered, urothelial cancer being very often associated with Balkan nephropathy.

[Clinical and morphologic stages in Balkan endemic nephropathy].

The first clinical classification of Balkan endemic nephropathy was elaborated by A. Puhlev et al in 1960, according to which two stages could be established in the course of the disease-compensated and decompensated. Gradually, many new data accumulated as regards the various aspects of the disease, that cannot be put into the frames of the first clinical classification. On the base of the author's own long-term complex clinical-laboratory, functional, instrumental and nephrobiopsic studies, he admits the existence of four stahes in the clinical and morphological course of the Balkan endemic

Evaluation of criteria for the diagnosis of Balkan endemic nephropathy.

BACKGROUND: The diagnosis of Balkan endemic nephropathy (BEN) is often made using Danilovic's criteria. The aim of this study was to determine the prevalence, sensitivity, specificity, and predictive value of Danilovic's criteria and several additional indices. METHODS: The study included 19 BEN patients, 23 BEN-suspected patients, 34 patients with other kidney diseases, and 23 healthy controls. The sensitivity and specificity of Danilovic's criteria was calculated, and these criteria, in addition to age, sex, blood pressure, creatinine clearance, glucosuria, urine osmolality, alkaline phosph

[Additional information on the history of Balkan endemic nephropathy].

The paper presents additional information to the bibliographic check up in the two papers of Asst. Prof. Dimitrov, published in the periodical "Vŭtreshni bolesti" in 1982 as regards the history of Balkan endemic nephropathy, being of importance for the early history of the disease. Some of the data not accurately cited in the two papers of Asst. Prof. Dimitrov are corrected. Critical notes are also presented about the erroneous interpretation of some of the early written scientific facts.

Population-genetic study of Balkan endemic nephropathy in Serbia.

The study of Balkan Endemic Nephropathy (BEN) in the affected localities of southern Serbia shows population-genetic difference between samples of BEN affected individuals and control group consisting of non-affected individuals from the same localities. Detailed population-genetic study in village Chepure, which includes 20 large families where BEN is present in 646 (from first to fourth degree) relatives of probants, shows familial character of disease as well as significant genetic influences in expression of the illness. Our study of genetic homozygosity degree includes an analysis of the

Balkan endemic nephropathy: role of ochratoxins A through biomarkers.

Several studies implicated mycotoxins, in endemic kidney disease geographically limited to Balkan region (Balkan endemic nephropathy (BEN)). In Bulgaria, much higher prevalence of ochratoxin A (OTA), exceeding 2 microg/L, was observed in the blood of affected population. OTA is found more often in the urine of people living in BEN-endemic villages. To confirm and quantify exposure to OTA in Vratza district, we followed up OTA intake for 1 month, OTA in blood and urine from healthy (20-30 years old) volunteers, from two villages with high risk for BEN disease. Food samples were collected daily,

Partial lecithin:cholesterol acyltransferase (LCAT) deficiency in Balkan endemic nephropathy.

The role of lipid abnormalities has been also implicated in the progression of renal diseases. The search for lipid abnormalities in Balkan endemic nephropathy (BEN) has roused sporadic interest and has not been fully elucidated. This study was performed in 54 healthy subjects from the families affected with BEN (group A), 18 members from non-affected families living in the same location (group B), and 25 control subjects (group C). Lipid profiles and lecithin:cholesterol acyltransferase (LCAT) were determined in each subject. The most striking distinction between the groups was that of the LC

Environmental renal disease: lead, cadmium and Balkan endemic nephropathy.

The similarity of lead and cadmium nephropathy to Balkan endemic nephropathy warrants careful reevaluation of the possibility that these nephrotoxic metals contribute to the production of the endemic renal disease. Low-level environmental exposure may result in a relationship between the concentration of the metals in tissue storage sites and biological fluids that differs from that encountered after occupational exposure. Urine and blood concentrations may therefore be inadequate measures of exposure. Lead is accumulated in the skeleton and cadmium in the liver and kidneys with biological hal

Spontaneous and induced chromosome aberrations in Balkan endemic nephropathy.

The aim of the present study was to investigate the spontaneous aberrations and chromosome breakages induced by X-rays and folic acid deficiency. In patients with Balkan endemic nephropathy (BEN) a higher frequency of spontaneous aberrations and chromosome lesions in medium TC 199 and radiation induced breakages were found compared with the healthy individuals. In BEN patients the 3q25 band was most frequently involved in the aberrations. These results support the idea that 3q25 may play a specific role and be a marker for BEN. Three of the additional five bands with increased frequencies of l
